Air Tahoma Flight 185 was a scheduled cargo flight from Memphis to Cincinnati/Northern Kentucky International Airport conducted by Air Tahoma as part of a contract to freight parcels for courier firm DHL. On August 13, 2004, the flight crashed during approach to landing just one mile short of the runway. The Convair 580, which is a twin engine turboprop, was destroyed upon impact. The first officer was killed and the captain received minor injuries. [1] [2]
On August 13, 2004, at about 00:49 Eastern Daylight Time, Air Tahoma, Inc., Flight 185 crashed about one mile south of Cincinnati/Northern Kentucky International Airport (CVG), in Florence, Kentucky while on approach to runway 36R. The crash was heard by a nearby Florence Police officer. Shortly after, first responders found the plane. First Officer Michael Gelwicks (37) was killed and Captain Bruno Pichelli (49) received minor injuries. The airplane was destroyed by impact forces. The flight was operating as a cargo flight for DHL Express from Memphis International Airport to CVG. Visual meteorological conditions prevailed for the flight, which operated on an instrument flight rules flight plan. [1]
The National Transportation Safety Board determined that the probable cause of this accident was fuel starvation resulting from the captain's decision not to follow approved fuel crossfeed procedures. Different output pressure settings on the fuel boost pumps coupled with the open crossfeed valve resulted in both engines drawing fuel from the left tank. All of the fuel from the airplane's left tank that was not used by the engines was transferred into the right tank due to the pressure differential between the boost pumps. During the airplane's descent to landing, the fuel in the left fuel tank became exhausted. Both engine-driven fuel pumps drew air from the exhausted left tank into the fuel system, resulting in a dual-engine flameout. [1]
Contributing to the accident were the captain's inadequate preflight planning, his subsequent distraction during the flight, and his late initiation of the in-range checklist. Further contributing to the accident was the flight crew's failure to monitor the fuel gauges and to recognize that the airplane's changing handling characteristics were caused by a fuel imbalance. The Convair 880 is a retired American narrow-body jet airliner produced by the Convair division of General Dynamics. It was designed to compete with the Boeing 707 and Douglas DC-8 by being smaller but faster, a niche that failed to create demand. When it was first introduced, some in aviation circles claimed that at 615 mph (990 km/h), it was the fastest jet transport in the world. Only 65 Convair 880s were produced over the lifetime of the production run from 1959 to 1962, and General Dynamics eventually withdrew from the airliner market after considering the 880 project a failure. The Convair 990 Coronado was a stretched and faster variant of the 880.
Cincinnati/Northern Kentucky International Airport is a public international airport located in Boone County, Kentucky, United States, around the community of Hebron. The airport serves the Cincinnati tri-state area. The airport's code, CVG, is derived from the nearest city at the time of the airport's opening, Covington, Kentucky. The airport covers an area of 7,700 acres (3,100 ha). It is included in the Federal Aviation Administration (FAA) National Plan of Integrated Airport Systems for 2023–2027, in which it is categorized as a medium-hub primary commercial service facility.
United Airlines Flight 232 was a regularly scheduled United Airlines flight from Stapleton International Airport in Denver to O'Hare International Airport in Chicago, continuing to Philadelphia International Airport. On July 19, 1989, the DC-10 serving the flight crash-landed at Sioux Gateway Airport in Sioux City, Iowa, after suffering a catastrophic failure of its tail-mounted engine due to an unnoticed manufacturing defect in the engine's fan disk, which resulted in the loss of many flight controls. Of the 296 passengers and crew on board, 112 died during the accident, while 184 people survived. 13 of the passengers were uninjured. It was the deadliest single-aircraft accident in the history of United Airlines.
Air Tahoma was an American cargo airline based in Columbus, Ohio, United States. It was established and started operations in 1996 in San Diego then later moved to Indianapolis in 1998 and to its last location at Rickenbacker International Airport, Columbus. Air Tahoma operated contract cargo flights internationally to the Caribbean, Mexico, Vietnam, Philippines, and the United States. Air Tahoma ceased operations in 2009.
American Airlines Flight 157, a Douglas DC-6, departed on November 29, 1949, from New York City bound for Mexico City with 46 passengers and crew. After one engine failed in mid-flight, a series of critical mistakes by the flight crew caused the pilot to lose control of the plane during the final approach to a routine stopover at Love Field in Dallas, Texas. The airliner slid off the runway and struck a parked airplane, a hangar, and a flight school before crashing into a business across from the airport. 26 passengers and two flight attendants died. The pilot, co-pilot, flight engineer, and 15 passengers survived.
In an internal combustion engine, fuel starvation is the failure of the fuel system to supply sufficient fuel to allow the engine to run properly, for example due to blockage, vapor lock, contamination by water, malfunction of the fuel pump or incorrect operation, leading to loss of power or engine stoppage. There is still fuel in the tank(s), but it is unable to get to the engine(s) in sufficient quantity. By contrast, fuel exhaustion is an occurrence in which the vehicle in question becomes completely devoid of usable fuel, with results similar to those of fuel starvation.
TWA Flight 128 was a regularly scheduled Trans World Airlines passenger flight from Los Angeles to Boston, with intermediate stops in Cincinnati and Pittsburgh. On November 20, 1967, Flight 128 crashed on final approach to Greater Cincinnati Airport; 70 of the 82 people aboard the Convair 880 were killed in the tragic crash.

British Airways Flight 38 was a scheduled international passenger flight from Beijing Capital International Airport in Beijing, China, to London Heathrow Airport in London, United Kingdom, an 8,100-kilometre trip. On 17 January 2008, the Boeing 777-200ER aircraft operating the flight crashed just short of the runway while landing at Heathrow. No fatalities occurred; of the 152 people on board, 47 sustained injuries, one serious. It was the first time in the aircraft type's history that a Boeing 777 was declared a hull loss, and subsequently written off.

On November 8, 1961, the aircraft crashed as it attempted to land at Byrd Field, near Richmond, Virginia. This was the second deadliest accident in American history for a single civilian aircraft.
Allegheny Airlines Flight 485 was a regularly scheduled domestic passenger flight between Washington, D.C. and Newport News, Virginia, United States, with three stop-overs, two in Connecticut and a third in Pennsylvania. On June 7, 1971, the Allegheny Airlines Convair CV-580 operating the flight crashed on approach to Tweed New Haven Regional Airport, New Haven County, Connecticut.
On December 27, 1968, North Central Airlines Flight 458 crashed into a hangar while attempting a night landing in poor weather at O'Hare International Airport in Chicago, Illinois, in the United States. Of the 41 passengers and four crew members, only 17 passengers and one crew member survived. One person was killed and six were injured on the ground.
Allegheny Airlines Flight 736 was a regularly scheduled flight that crashed while attempting to land at Bradford Regional Airport in Bradford, Pennsylvania, on December 24, 1968. Of the 47 occupants on board, 20 were killed.

RED Air Flight 203 (L5203/REA203) was a scheduled international commercial passenger flight from Santo Domingo in the Dominican Republic to Miami International Airport by RED Air. On 21 June 2022, the McDonnell Douglas MD-82 aircraft operating the service suffered a left landing gear collapse and runway excursion, causing the left wing of the aircraft to impact an antenna structure, followed by a subsequent fire on the right side of the airplane. The incident caused three people to be hospitalized with minor injuries.
