Body psychotherapy

Body psychotherapy, [1] [2] [3] [4] also called body-oriented psychotherapy, is an approach to psychotherapy [5] which applies basic principles of somatic psychology. It originated in the work of Pierre Janet, Sigmund Freud and particularly Wilhelm Reich who developed it as vegetotherapy. [6] Branches also were developed by Alexander Lowen, and John Pierrakos, both patients and students of Reich, like Reichian body-oriented psychotherapy [7] [8] [9] and Gerda Boyesen. [10]

History

Wilhelm Reich and the post-Reichians are considered the central element of body psychotherapy. [11] From the 1930s, Reich became known for the idea that muscular tension reflected repressed emotions, what he called 'body armour', and developed a way to use pressure to produce emotional release in his clients. [12] Reich was expelled from the psychoanalytic mainstream and his work found a home in the 'growth movement' of the 1960s and 1970s and in the countercultural project of 'liberating the body'. [12] Perhaps as a result, body psychotherapy was marginalised within mainstream psychology and was seen in the 1980s and 1990s as 'the radical fringe of psychotherapy'. [13] Body psychotherapy's marginal position may be connected with the tendency for charismatic leaders to emerge within it, from Reich onwards. [14]

Alexander Lowen in his Bioenergetic analysis [15] and John Pierrakos in Core energetics [16] extended Reich's finding of the segmented nature of body armour: "The muscular armour has a segmented arrangement...always transverse to the torso, never along it". [17] [18] Lowen claimed that "No words are so clear as the language of body expression". [19] Subsequently, the Chiron Centre for Body Psychotherapy added influences from Gestalt therapy to their approach. [20]

The early 2000s saw a 'renaissance of body psychotherapy' which was part of a broader increased interest in the body and embodiment in psychology and other disciplines including philosophy, sociology, anthropology and cultural studies. [21] Object relations theory has arguably opened the way more recently for a fuller consideration of the body-mind connection in psychotherapy. [22] [23] [24]

Branches

There are numerous branches of body psychotherapy, often tracing their origins to particular individuals: for example, 'Bioenergetic analysis' to the work of Lowen and Pierrakos; 'Radix' to the work of Chuck Kelley; Organismic Psychotherapy to the work of Malcolm and Katherine Brown; [25] 'Biosynthesis' to the work of David Boadella; [26] 'Biodynamic Psychology' to that of Gerda Boyesen; 'Rubenfeld Synergy' to Ilana Rubenfeld's work; [27] 'Body-Mind Centering' to Bonnie Bainbridge Cohen's work, and 'Body-mind Psychotherapy' to Susan Aposhyan; [28] the development of Jack Painter's 'Postural and Energetic Integration' into a psychotherapeutic modality. [29]

Many of these contributors to body psychotherapy were influenced by the work of Wilhelm Reich, while adding and incorporating a variety of other influences. [30] Syntheses of these approaches are also becoming accepted and recognised in their own right (e.g. The Chiron Approach: Chiron Association of Body Psychotherapists). [31]

Alongside the body psychotherapies built directly on the work of Reich, there is a branch of post-Jungian body psychotherapies, developed from Jung's idea of the 'somatic unconscious'. [32] While many post-Jungians dismiss Reich and do not work with the body, [33] contributors to Jungian derived body psychotherapy include Arnold Mindell with his concept of the 'dreambody' and the development of process oriented psychology. [34] [35] [36] [37] Process oriented psychology is known for its focus on the body and movement. [38] [39]

Body psychotherapy and dance movement therapy have developed separately and are professionally distinguished, however they have significant common ground and shared principles including the importance of non-verbal therapeutic techniques and the development of body-focused awareness. [27]

A review of body psychotherapy research finds there is a small but growing empirical evidence base about the outcomes of body psychotherapy, however it is weakened by the fragmentation of the field into different branches and schools. [40] The review reports that one of the strongest studies is longitudinal (2 year) outcome research conducted with 342 participants across 8 different schools (Hakomi Experiental Psychology, Unitive Body Psychotherapy, Biodynamic Psychology, Bioenergetic Analysis, Client-Centred Verbal and Body Psychotherapy, Integrative Body Psychotherapy, Body-Oriented Psychotherapy, and Biosynthesis). Overall efficacy was demonstrated in symptom reduction, however the study design limited further substantive conclusions. [41]

The review of outcome research across different types of body-oriented psychotherapy concludes that the best evidence supports efficacy for treating somatoform/psychosomatic disorders and schizophrenia, [42] [ full citation needed ] while there is also support for 'generally good effects on subjectively experienced depressive and anxiety symptoms, somatisation and social insecurity.' [43] [ full citation needed ] A more recent review found that results in some of these domains were mixed or might have resulted from other causes (for example, somatic symptoms in one study improved even after therapy had ended, suggesting that the improvements may have been unrelated to the therapy). [44]

Trauma

Body psychotherapy is one modality used in a multi-modal approach to treating psychological trauma, particularly post-traumatic stress disorder (PTSD) and complex post-traumatic stress disorder (C-PTSD). [45] [46] [47] [48]

Recovering a sense of physical boundaries through sensorimotor psychotherapy is an important part of re-establishing trust in the traumatised. [49] Blending somatic and cognitive awareness, such an approach reaches back for inspiration to the pioneering work of Janet, as well as employing the more recent work of António Damásio. [50]

The necessity of often working without touch with traumatised victims presents a special challenge for body psychotherapists. [51]

Organizations

The European Association for Body Psychotherapy (EABP) and The United States Association for Body Psychotherapy (USABP) are two professional associations for body psychotherapists. [52]

The EABP was founded in 1988 to promote the inclusion of body psychotherapy within a broader process of professionalisation, standardisation and regulation of psychotherapy in Europe, driven by the European Association for Psychotherapy (EAP). [37] The EABP Board committed to meeting the EAP standards for establishing the scientific validity of psychotherapy modalities and achieved this in 1999/2000 for body psychotherapy as a whole, with various individual modalities subsequently also achieving this recognition. [53] It was accepted as a European-Wide Accrediting Organisation in 2000. [54]

EABP has a bi-annual conference; organises a Council of ten National B-P Associations; supports a FORUM of Body-Psychotherapy Organisations, which accredits more than 18 B-P training organisations in 10 different countries; the EABP website also provides a list of research papers; a searchable bibliography of body-psychotherapy publications, containing more than 5,000 entries. [55]

The USABP was formed in June 1996 [56] to provide professional representation for body psychotherapy practitioners in the United States. The USABP launched a peer-reviewed professional journal in 2002, the USA Body Psychotherapy Journal, which was published twice-yearly from 2002 to 2011. [57] In 2012, the sister organisations, EABP and USABP, together launched the International Body Psychotherapy Journal. [58]

There is also an Australian Association of Somatic Psychotherapy Australia. [59]

Cautions

The importance of ethical issues in body psychotherapy has been highlighted on account of the intimacy of the techniques used. [20]

The term bioenergetic has a well established meaning in biochemistry and cell biology. [60] Its use in RBOP (Reichian body-oriented psychotherapy) has been criticized as "ignoring the already well established universal consensus about energy existing in Science." [61]

There is a group of psychotherapists who believe that psychotherapy should be thought of as a craft and evaluated based on the effectiveness of the treatment, rather than evaluated based on scientific validity. [62] [63] However, efficacy studies of body psychotherapy have been few in number and, although the results are supportive of the use of body psychotherapy in some contexts, this trend "is not overwhelming". [64]

Orgone is a pseudoscientific concept variously described as an esoteric energy or hypothetical universal life force. Originally proposed in the 1930s by Wilhelm Reich, and developed by Reich's student Charles Kelley after Reich's death in 1957, orgone was conceived as the anti-entropic principle of the universe, a creative substratum in all of nature comparable to Mesmer's animal magnetism (1779), to the Odic force (1845) of Carl Reichenbach and to Henri Bergson's élan vital (1907). Orgone was seen as a massless, omnipresent substance, similar to luminiferous aether, but more closely associated with living energy than with inert matter. It could allegedly coalesce to create organization on all scales, from the smallest microscopic units—called "bions" in orgone theory—to macroscopic structures like organisms, clouds, or even galaxies.

Bodymind is an approach to understand the relationship between the human body and mind where they are seen as a single integrated unit. It attempts to address the mind–body problem and resists the Western traditions of mind–body dualism. The term bodymind is also typically seen and encountered in disability studies, referring to the intricate and often inseparable relationship between the body and the mind, and how these two units might act as one. The field of psychosomatic medicine investigates the embodied manifestations of psychological processes.

Arnold Mindell is an American author, therapist, and teacher in the fields of transpersonal psychology, body psychotherapy, social change, and spirituality. He is known for extending Jungian dream analysis to body symptoms, promoting ideas of ‘deep democracy,’ and interpreting concepts from physics and mathematics in psychological terms. Mindell is the founder of process oriented psychology, also called Process Work, a development of Jungian psychology influenced by Taoism, shamanism, and physics.

Process-oriented psychology, also called process work, is a depth psychology theory and set of techniques developed by Arnold Mindell and associated with transpersonal psychology, somatic psychology and post-Jungian psychology. Process oriented psychology has been applied in contexts including individual therapy and working with groups and organisations. It is known for extending dream analysis to body experiences and for applying psychology to world issues including socioeconomic disparities, diversity issues, social conflict and leadership.

The Hakomi Method is a form of mindfulness-centered somatic psychotherapy developed by Ron Kurtz in the 1970s.

Vegetotherapy is a form of Reichian psychotherapy that involves the physical manifestations of emotions.

Somatic experiencing (SE) is a form of alternative therapy aimed at treating trauma and stress-related disorders, such as PTSD. The primary goal of SE is to modify the trauma-related stress response through bottom-up processing. The Clients’ attention is directed toward internal sensations,, rather than to cognitive or emotional experiences. The method was developed by Peter A. Levine.

Somatic psychology is a pseudoscientific form of psychotherapy that focuses on somatic experience, including therapeutic and holistic approaches to the body. Somatic psychology seeks to bridge the mind-body dichotomy. Wilhelm Reich is the first who tried to develop a clear psychodynamic approach that included the body, but he soon found out that it could not be done. Several types of body-oriented psychotherapies trace their origins back to Reich, though there have been many subsequent developments and other influences on body psychotherapy and somatic psychology is of particular interest in trauma work.

Gerda Boyesen was the founder of Biodynamic Psychology, a branch of Body Psychotherapy.

Postural Integration is a type of bodywork purporting to draw on "energy" and allow access to the past. It was devised in the late 1960s by Jack Painter (1933–2010) in California, US, after exploration in the fields of humanistic psychology and the human potential movement.

Reichian therapy can refer to several schools of thought and therapeutic techniques whose common touchstone is their origins in the work of psychoanalyst Wilhelm Reich (1897–1957). Some examples are:

Body-centred countertransference involves a psychotherapist's experiencing the physical state of the patient in a clinical context. Also known as somatic countertransference, it can incorporate the therapist's gut feelings, as well as changes to breathing, to heart rate and to tension in muscles.

Alexander Lowen was an American physician and psychotherapist.

Biodynamic massage is a complementary therapy developed by Gerda Boyesen in Norway during the 1950s.

Stanley Keleman was an American writer and therapist, who created the body psychotherapy approach known as "formative psychology". He was one of the leaders of the body psychotherapy movement nationally and internationally. His methodology rested on an anatomical base and incorporates an evolutionary, philosophical and mythological perspective; within this formative paradigm the human is capable of learning voluntary self-influence of instinctual and emotional expression as a way to manage dilemmas of daily living and to form personal choices for creating a future. Keleman started developing and articulating his concepts in 1957. In 1971, he published the first of 10 books.

John Pierrakos was an American physician and psychiatrist. A student of Wilhelm Reich, he developed bioenergetic analysis, a form of mind-body psychotherapy, with his then-colleague Alexander Lowen. Pierrakos was the founder and director of the Institute of Core Energetics (1973-2001), co-founder of the Bioenergetics Institute in New York, NY with Lowen (1955-1970), and co-founder of The Pathwork Center, Phoenicia, NY, in association with his wife, Eva Pierrakos.

The British Psychotherapy Foundation, Bpf, is the successor organisation to three former long-established British psychotherapy providers and clinical training institutions which merged in April 2013. The original constituents are the British Association of Psychotherapists, BAP (1951), The Lincoln Clinic and Centre for Psychotherapy (1968) and the London Centre for Psychotherapy, LCP, (1976). It is unique in the United Kingdom for providing treatment services for children and adults in all the psychoanalytic modalities, that is of Freudian and Jungian inspiration. It is also unique in providing professional training in those modalities within one institution and is regulated by the British Psychoanalytic Council. It has charitable status.
