Eldon Leroy Foltz (March 28, 1919 - August 10, 2013) was born in Fort Collins, Colorado, Eldon L. Foltz grew up in East Lansing, Michigan. His father was Professor of Electrical Engineering at Michigan State University, and Foltz himself would go on to have a distinguished career as an academic and neurosurgeon.

Education

In 1941 Foltz graduated from Michigan State University (B.S., Magna Cum Laude). He went on to the University of Michigan Medical School in 1943 (M.D.), doing a combined scholar program with Michigan State.

Having graduated from medical school and done a surgical internship at the University of Michigan, Foltz joined the United States Navy Corps. He served for 30 months of active duty in the South Pacific.

Next Foltz did a year of general surgery residency at the University of Michigan and became a graduate student in neuroanatomy and neuropathology. His residency in neurosurgery was at Dartmouth Medical School, where he studied under Henry Heyl. In 1950 he finished residency at the University of Louisville, where he had studied with Dr. Glen Spurling.

In order to study the limbic system Foltz became a post-doctoral fellow at The National Institute of Mental Health. [1]

Career

Foltz’ initial research concerned "coma of head injury." He studied monkeys with electrodes implanted in the reticular system and also cortical electrodes. He used evoked responses to demonstrate a selective depression of electrical activity in the reticular formation by the cerebral concussion, with minimal effect on primary ascending sensory pathways. The effect was reversed (in part) by atropine therapy.

"Psychosomatic disease states in monkeys and the limbic system" was the result of Foltz’ investigation of chaired "executive" monkeys. They were trained to conditioned avoidance and "conditioned stress," and monitored as to agitate states by the degree of lever pulling and the effect on intestinal motility (direct contraction measurements). The study proved that cingulotomy reduced the agitated responses, thus modifying lever pulling to an efficient level and reducing the increased gut motility.

As he oversaw pediatric neurosurgical service at Children's Hospital in Seattle, WA, Foltz became profoundly interested in pediatric neurosurgery. Hydrocephalus became would become another major interest. Indeed, he’d work on it for the remainder of his career.

Accomplishments and awards

At the University of Washington, Foltz received a John R. and Mary Markle scholarship in Medical Science to support his coma and cingulum studies. During that time he became committed to academic neurosurgery. In 1965, Foltz He became a full Professor in Neurological Surgery at the university.

Foltz developed selective frontal leuotomy cingulotomy at the university, working with Dr. Arthur Ward. [2]

In 1969, Foltz became Chairman of Neurosurgery at the University of California, Irvine. There he did experimental work in hydrocephalus, his emphasis being on CSF molecular transport studies in dogs and cats. Foltz trained twenty neurosurgeons under the program.

Foltz held offices in the Western Neurosurgical Society, Society of Neurological Surgeons, Neurosurgical Society of America, and Society of Neurological Surgeons of Orange County. He became Emeritus Professor of Neurological Surgery in 1989.

Miscellany

After graduating from Medical School in 1943, Foltz married Katherine Crosby, a University of Michigan microbiologist. Foltz was fond of sailboat racing, backpacking, photography, classical music, and opera. He had five children, five grandchildren, and one great grandchild. [3]

Normal pressure hydrocephalus (NPH), also called malresorptive hydrocephalus, is a form of communicating hydrocephalus in which excess cerebrospinal fluid (CSF) builds up in the ventricles, leading to normal or slightly elevated cerebrospinal fluid pressure. The fluid build-up causes the ventricles to enlarge and the pressure inside the head to increase, compressing surrounding brain tissue and leading to neurological complications. Although the cause of idiopathicNPH remains unclear, it has been associated with various co-morbidities including hypertension, diabetes mellitus, Alzheimer's disease, and hyperlipidemia. Causes of secondary NPH include trauma, hemorrhage, or infection. The disease presents in a classic triad of symptoms, which are memory impairment, urinary frequency, and balance problems/gait deviations. The disease was first described by Salomón Hakim and Raymond Adams in 1965.

The study of neurology and neurosurgery dates back to prehistoric times, but the academic disciplines did not begin until the 16th century. The formal organization of the medical specialties of neurology and neurosurgery are relatively recent, taking place in the place in Europe and the United States only in the 20th century with the establishment of professional societies distinct from internal medicine, psychiatry and general surgery. From an observational science they developed a systematic way of approaching the nervous system and possible interventions in neurological disease.

Ralph Bingham Cloward was an American neurosurgeon, best known for his innovations in spinal neurosurgery. Cloward is known for the development of the Posterior Lumbar Interbody Fusion and Anterior Cervical Discectomy and Fusion. Cloward moved from Chicago to Hawaii in 1938, becoming the state's lone neurosurgeon. He is well known for his work treating victims of brain injuries after the Pearl Harbour attack in 1941.

A cerebral shunt is a device permanently implanted inside the head and body to drain excess fluid away from the brain. They are commonly used to treat hydrocephalus, the swelling of the brain due to excess buildup of cerebrospinal fluid (CSF). If left unchecked, the excess CSF can lead to an increase in intracranial pressure (ICP), which can cause intracranial hematoma, cerebral edema, crushed brain tissue or herniation. The drainage provided by a shunt can alleviate or prevent these problems in patients with hydrocephalus or related diseases.

Endoscopic third ventriculostomy (ETV) is a surgical procedure for treatment of hydrocephalus in which an opening is created in the floor of the third ventricle using an endoscope placed within the ventricular system through a burr hole. This allows the cerebrospinal fluid to flow directly to the basal cisterns, bypassing the obstruction. Specifically, the opening is created in the translucent tuber cinereum on the third ventricular floor.

An external ventricular drain (EVD), also known as a ventriculostomy or extraventricular drain, is a device used in neurosurgery to treat hydrocephalus and relieve elevated intracranial pressure when the normal flow of cerebrospinal fluid (CSF) inside the brain is obstructed. An EVD is a flexible plastic catheter placed by a neurosurgeon or neurointensivist and managed by intensive care unit (ICU) physicians and nurses. The purpose of external ventricular drainage is to divert fluid from the ventricles of the brain and allow for monitoring of intracranial pressure. An EVD must be placed in a center with full neurosurgical capabilities, because immediate neurosurgical intervention can be needed if a complication of EVD placement, such as bleeding, is encountered.

Vein of Galen aneurysmal malformations(VGAMs) and Vein of Galen aneurysmal dilations (VGADs) are the most frequent arteriovenous malformations in infants and fetuses. A VGAM consists of a tangled mass of dilated vessels supplied by an enlarged artery. The malformation increases greatly in size with age, although the mechanism of the increase is unknown. Dilation of the great cerebral vein of Galen is a secondary result of the force of arterial blood either directly from an artery via an arteriovenous fistula or by way of a tributary vein that receives the blood directly from an artery. There is usually a venous anomaly downstream from the draining vein that, together with the high blood flow into the great cerebral vein of Galen causes its dilation. The right sided cardiac chambers and pulmonary arteries also develop mild to severe dilation.

Endovascular coiling is an endovascular treatment for intracranial aneurysms and bleeding throughout the body. The procedure reduces blood circulation to the aneurysm through the use of microsurgical detachable platinum wires, with the clinician inserting one or more into the aneurysm until it is determined that blood flow is no longer occurring within the space. It is one of two main treatments for cerebral aneurysms, the other being surgical clipping. Clipping is an alternative to stenting for bleeding.
