Firefighter rehabilitation is a vital firefighting service, providing firefighters and other emergency personnel with immediate medical attention including rehydration, treatment for smoke inhalation, and the prevention of life-threatening conditions such as heatstroke and heart attack [1] after working at the scene of an incident. [2] Firefighter rehabilitation can include a variety of things from a simple check up to deciding whether or not the firefighter needs to see a doctor. The rehabilitation area is set up in a safe location near the incident so that it can be accessible to any emergency responders who may need it.

Purpose

Firefighter rehabilitation is designed to ensure that the physical and mental well-being of members operating at the scene of an emergency (or a training exercise) don't deteriorate to the point where it affects the safety of any other members. Firefighting is inherently dangerous in the best of circumstances, and any additional physical or mental stress increases the danger. Studies have shown that core body temperature and heart rate can rise dramatically during firefighting operations. [3] Without intervention, firefighters may be at risk for serious acute illness from the extreme conditions and high levels of exertion. [4]

The primary mission for personnel responsible for firefighter rehabilitation is to identify, examine, and evaluate the physical and mental status of fire-rescue personnel who have been working during the emergency incident or a training exercise. Following a proper survey, it should be determined what additional treatment, if any, may be required. The need to implement rehabilitation is based on many factors including, but not limited to, outside temperature, type of work being performed by firefighters, and use of Self-Contained Breathing Apparatus (SCBA). [5] [6]

Designating a Rehabilitation Area

A specially designated Rehabilitation Area is established at the discretion of the Incident Commander. If the Incident Commander determines that Rehab is necessary, firefighters may be instructed to "self-rehab" in the case of smaller-scale incidents, or qualified Paramedics or EMTs should be designated to manage the Rehab Area under the command of a fire or EMS officer or supervisor. [5] The site should be located away from any environmental hazards, or by-products of the fire, such as smoke, gases or fumes. During hot months, the ideal location might include a shady, cool area distant from the incident. In winter, a warm, dry area is preferred. Regardless of the season, the area should be readily accessible to EMS-Rescue personnel and their equipment, so they may restock the sector with supplies, or in the event that ambulance transport is required. [5] [7]

Rehab sites can take many forms, including lobbies of nearby buildings, parking lots, or grassy areas such as yards, parks, or fields. [6] Specially designed rehab vehicles are also used by many fire departments and EMS agencies. [8]

. During large-scale incidents, like multi-alarm fires, multiple Rehab Areas may be necessary. [6]

Coordination and manning

Command of the Rehab Area is assigned to a fire chief or company level officer, who is designated as the Rehab Officer under most Incident Command structures. [6] A minimum of two trained EMS personnel should initially be assigned to monitor and assist firefighters in the Rehab Sector, but more personnel may be necessary for larger incidents. Volunteer canteen or auxiliary members often assist EMS personnel in making firefighters as comfortable as possible. [9] [10]

It is important for command and company level officers to continually monitor personnel for telltale signs of exhaustion, stress, and or physical trauma. [6] Individual members are encouraged to report to the Rehab Sector at any time that he or she feels the need to do so. Symptoms may include weakness, dizziness, chest pain, muscle cramps, nausea, altered mental status, difficulty breathing, and others. [6] The U.S. Fire Administration recommends all fireground personnel (regardless of physical well-being) should report to the Rehab Sector immediately following strenuous activity, the use and depletion of two SCBA bottles (or failure of an SCBA), or forty (40) minutes of intense physical activity. [6]

Medical monitoring

When firefighters get to the rehabilitation tent, they are monitored to prevent life-threatening conditions such as heat stroke and heart attack. Medical monitoring includes: [11]

Heart rate should be measured as early as possible in the rest period. If the firefighter's heart rate exceeds 110 beats per minute, it is recommended that an oral temperature be taken. If body core temperature exceeds 100.6F, the firefighter should not be permitted to wear protective equipment or re-enter the active work environment, until temperature has been reduced and heart rate decreased. [12]

They are typically used in firefighting and industry. The term self-contained means that the SCBA is not dependent on a remote supply of breathing gas. If designed for use under water, it is also known as a Scuba set. When not used underwater, they are sometimes called industrial breathing sets. Unofficial names include air pack, air tank, oxygen cylinder or simply pack, which are mostly used in firefighting.

<span class="mw-page-title-main">Volunteer fire department</span> Fire department composed of volunteers

A volunteer fire department (VFD) is a fire department of volunteers who perform fire suppression and other related emergency services for a local jurisdiction. Volunteer and retained (on-call) firefighters are expected to be on call to respond to emergency calls for long periods of time, and are summoned to the fire station when their services are needed. They are also expected to attend other non-emergency duties as well.

In firefighting, the policy of two-in, two-out refers to United States Occupational Safety and Health Administration (OSHA) policy 29 CFR 1910.134(g)(4)(i). The respiratory protection standard requires that workers engaged in fighting interior structural fires work in a buddy system; at least two workers must enter the building together, so that they can monitor each other's whereabouts as well as the work environment. There must also be at least two standby personnel outside the fire area prepared to rescue the inside firefighters should the need arise. One of these outside firefighters must actively monitor the status of the inside fighters but the second outside firefighter may perform a variety of other duties, such as pump operations, incident commander or outside hose line operation. There are no provisions in the standard to waive the requirements for either the "two-inside firefighters" or the "two-outside firefighters", although the circumstances under which this provision applies are more limited than generally understood.

<span class="mw-page-title-main">Vehicle extrication</span>

Vehicle extrication is the process of removing a vehicle from around a person who has been involved in a motor vehicle collision, when conventional means of exit are impossible or inadvisable. A delicate approach is needed to minimize injury to the victim during the extrication. This operation is usually accomplished by using chocks and bracing for stabilization and powered rescue tools and equipment, including the Jaws of Life. Standards and regulations for organizations can be found in NFPA 1670 and for individual members in 1006.

<span class="mw-page-title-main">PASS device</span> Device used to set off an alarm when a firefighter is in distress

A PASS device, also known as a Distress Signal Unit (DSU) or ADSU, is a personal safety device used primarily by firefighters entering a hazardous or Immediately Dangerous to Life and Health (IDLH) environment such as a burning building. The PASS device sounds a loud (95 decibel) audible alert to notify others in the area that the firefighter is in distress. On a fireground, the sound of an activated PASS device indicates a true emergency and results in an immediate response to rescue the firefighter(s) in distress. In the United States, the National Fire Protection Association sets standards for PASS devices in NFPA 1982.

Firefighting jargon includes a diverse lexicon of both common and idiosyncratic terms. One problem that exists in trying to create a list such as this is that much of the terminology used by a particular department is specifically defined in their particular standing operating procedures, such that two departments may have completely different terms for the same thing. For example, depending on whom one asks, a safety team may be referred to as a standby, a RIT or RIG or RIC, or a FAST. Furthermore, a department may change a definition within its SOP, such that one year it may be RIT, and the next RIG or RIC.

<span class="mw-page-title-main">Hazmat suit</span> Protective suit against chemical, bacteriological, and nuclear risks

A hazmat suit is a piece of personal protective equipment that consists of an impermeable whole-body garment worn as protection against hazardous materials. Such suits are often combined with self-contained breathing apparatus (SCBA) to ensure a supply of breathable air. Hazmat suits are used by firefighters, emergency medical technicians, paramedics, researchers, personnel responding to toxic spills, specialists cleaning up contaminated facilities, and workers in toxic environments.

A firefighter assist and search team (FAST), also known as a rapid intervention team/rapid intervention crew/rapid intervention dispatch (RIT/RIC/RID), is a team of two or more firefighters dedicated solely to the search and rescue of other firefighters in distress. Firefighter Assist and Search Team personnel shall have no other operational assignment during any incident. Multiple alarm fires may require multiple FAST/RIC teams.

<span class="mw-page-title-main">Rescue vehicle</span> Specialized vehicle with technical life saving equipment

A rescue vehicle is a specialised vehicle designed to transport and provide the equipment necessary for technical rescue. Vehicles carry an array of special equipment such as the jaws of life, wooden cribbing, generators, winches, hi-lift jacks, cranes, cutting torches, circular saws and other forms of heavy equipment unavailable on standard trucks. This capability differentiates them from traditional pumper trucks or ladder trucks designed primarily to carry firefighters and their entry gear as well as on-board water tanks, hoses and equipment for fire extinguishing and light rescue. Most rescue vehicles lack on-board water tanks and pumping gear, owing to their specialized role. A rescue vehicle is typically operated by a rescue squad, but in some areas it may be integrated with emergency medical services or fire departments.

<span class="mw-page-title-main">Light and air unit</span>

An air and light unit, also known as a breathing support unit, is a specialized piece of firefighting apparatus used by firefighters to provide supplemental lighting and SCBA air bottles at the scene of an emergency. During prolonged emergencies, particularly structure fires, where firefighters must remain on air, these air bottles will need to be replaced and refilled. The air and light unit has the ability to refill the SCBA bottles while in the field using onboard air compressors (cascade). It also carries a supply of spare air cylinders. These units are highly customizable and can vary greatly between departments. These multifunctional units are also equipped with diesel generators which supply electricity to power portable lights and overall scene illumination equipment. This is usually done via a roof-mounted telescoping light bank. The unit also has the ability to supply electrical power in an emergency to a shelter, base camp, or medical facility.

A Special operations firefighter, also known as Fire Service Special Operations, is a specialist firefighter who has been specially trained to execute tasks other than standard firefighting operations. The National Fire Protection Association's Standard for the Organization and Deployment of Fire Suppression Operations, Emergency Medical Operations, and Special Operations to the Public by Career Fire Departments defines special operations as "Those emergency incidents to which the fire department responds that require specific and advanced training and specialized tools and equipment". The NFPA 1710 further defined special operations as "Special operations include water rescue, extrication, hazardous materials, confined space entry, high­angle rescue, aircraft rescue and fire fighting, and other operations requiring specialized training".
