Ocularist

An ocularist specializes in the fabrication and fitting of ocular prostheses for people who have lost an eye or eyes due to trauma or illness. [1] The fabrication process for a custom made eye typically includes taking an impression of the eye socket, shaping a plastic shell, painting the iris, and then fitting the ocular prostheses. Prefabricated ocular prostheses with different colored iris are also available. An ocularist may select the stock eye that is most closely matched to patient's iris color. However, due to better adaptation, comfort, and aesthetics, custom-made ocular prostheses are more accepted. In addition to creating the prosthetic eye, an ocularist shows the patient how to care for and handle the prosthesis.

Ocularists may develop their skills from various background disciplines, including medical, optometry, dental, nursing, biology, medical arts and illustration.

United States

Training and registration varies significantly worldwide.

There is not a specific training program of study to call a person an "ocularist" at the present time. Therefore, this field is practiced in many places by maxillofacial prosthodontists, dentists who have undergone 3–4 years of specialty residency training. There are also technicians who practice in this field after learning how to make artificial eyes through an apprenticeship program. There are also people who use the title of ocularist who have had no formal training in the fitting, manufacture, and delivery of ocular prosthetics.

The American Society of Ocularists teaches the fitting, fabrication, insertion, and maintenance of artificial eyes.

The National Examining Board of Ocularists (NEBO) is the certifying agency for ocularists in North America. NEBO designates the title of Board Certified Ocularist for Ocularists that have passed the NEBO examination. Those ocularists that have achieved and maintained certification by NEBO are designated as Board Certified Ocularists (BCO). An ocularist with this title has also pledged to make custom prosthetics as opposed to stock prosthetics.

Traditionally, ocularists hand paint the iris and sclera of an artificial eye. In the 1990s some ocularists began to use digital printing to enhance the natural appearance of the artificial eye. Digital artificial eyes are only made in a few ocularist offices.

Aniridia is the absence of the iris, a muscular structure that opens and closes the pupil to allow light into the eye. It is also responsible for eye color. Without it, the central eye appears all black. It can be congenital, in which both eyes are usually involved, or caused by a penetrant injury. Isolated aniridia is a congenital disorder that is not limited to a defect in iris development, but is a panocular condition with macular and optic nerve hypoplasia, cataract, and corneal changes. Vision may be severely compromised and the disorder is frequently associated with some ocular complications: nystagmus, amblyopia, buphthalmos, and cataract. Aniridia in some individuals occurs as part of a syndrome, such as WAGR syndrome, or Gillespie syndrome.

Prosthodontics, also known as dental prosthetics or prosthetic dentistry, is the area of dentistry that focuses on dental prostheses. It is one of 12 dental specialties recognized by the American Dental Association (ADA), Royal College of Surgeons of England, Royal College of Surgeons of Edinburgh, Royal College of Surgeons of Ireland, Royal College of Surgeons of Glasgow, Royal College of Dentists of Canada, and Royal Australasian College of Dental Surgeons. The ADA defines it as "the dental specialty pertaining to the diagnosis, treatment planning, rehabilitation and maintenance of the oral function, comfort, appearance and health of patients with clinical conditions associated with missing or deficient teeth or oral and maxillofacial tissues using biocompatible substitutes."

A palatal lift prosthesis is a prosthesis that addresses a condition referred to as palatopharyngeal incompetence. Palatopharyngeal incompetence broadly refers to a muscular inability to sufficiently close the port between the nasopharynx and oropharynx during speech and/or swallowing. An inability to adequately close the palatopharyngeal port during speech results in hypernasalance that, depending upon its severity, can render speakers difficult to understand or unintelligible. The potential for compromised intelligibility secondary to hypernasalance is underscored when consideration is given to the fact that only three English language phonemes – /m/, /n/, and /ng/ – are pronounced with an open palatopharyngeal port. Furthermore, an impaired ability to effect a closure of the palatopharyngeal port while swallowing can result in the nasopharyngeal regurgitation of liquid or solid boluses.

Breast prostheses are breast forms intended to look like breasts. They are often used temporarily or permanently by women after mastectomy or lumpectomy procedures, but may also be used by for aesthetic purposes. There are a number of materials and designs; although, the most common construction is gel in a plastic film meant to feel similar to a person's skin. Prostheses may be purchased at a surgical supply store, pharmacy, custom lingerie shop, or even through private services that come to a person's home. There are many types of ready made breast prostheses including full or standard prostheses, partial prostheses such a shell prostheses, and stick on prostheses. Customized options are also available from specialty shops, which are moulded to fit an individual's chest by taking an impression of the breast(s). The areola and nipple may be replicated as part of the breast form or as separate nipple prosthesis. Both custom made and off-the shelf breast prostheses come in varieties that are designed to either be held in a pocket in a specially designed mastectomy bra or attached to the skin via adhesive or other methods and worn with a standard bra. There are many factors to consider when selecting breast prostheses such as different types and the care they require, insurance coverage, and psychosocial effects.

A Prosthetist and Orthotist, as defined by The World Health Organization, is a healthcare professional with overall responsibly of Prosthetics & Orthotics treatment, who can supervise and mentor the practice of other personnel. They are clinicians trained to assess the needs of the user, prescribe treatment, determine the precise technical specifications of prosthesis and Orthosis, take measurements and image of body segments, prepare model of the evaluation, fit devices and evaluate treatment outcome.

Anaplastology is a branch of medicine dealing with the prosthetic rehabilitation of an absent, disfigured or malformed anatomically critical location of the face or body. The term anaplastology was coined by Walter G. Spohn and is used worldwide.

Craniofacial prostheses are prostheses made by individuals trained in anaplastology or maxillofacial prosthodontics who medically help rehabilitate those with facial defects caused by disease, trauma or birth defects. They have the ability to replace almost any part of the face, but most commonly the ear, nose or eye/eyelids. An ocular prosthesis and hair prosthesis can also be classified as craniofacial prostheses. Prostheses are held in place either by biocompatible drying adhesives, osseointegrated implants, magnets, or another mechanical means such as glasses or straps. Prostheses are designed to be as similar as possible to the natural anatomy of each individual. Their purpose is to cover, protect, and disguise facial disfigurements or underdevelopments.

An ocular prosthesis, artificial eye or glass eye is a type of craniofacial prosthesis that replaces an absent natural eye following an enucleation, evisceration, or orbital exenteration. The prosthesis fits over an orbital implant and under the eyelids. Though often referred to as a glass eye, the ocular prosthesis roughly takes the shape of a convex shell and is made of medical grade plastic acrylic. A few ocular prostheses today are made of cryolite glass. A variant of the ocular prosthesis is a very thin hard shell known as a scleral shell which can be worn over a damaged or eviscerated eye. Makers of ocular prosthetics are known as ocularists. An ocular prosthesis does not provide vision; this would be a visual prosthesis. A nose prosthesis is a craniofacial prosthesis for someone who no longer has their original nose. Nose prostheses are designed by anaplastologists who have their patients referred to them by ear, nose, and throat doctors and plastic surgeons.

A visual prosthesis, often referred to as a bionic eye, is an experimental visual device intended to restore functional vision in those with partial or total blindness. Many devices have been developed, usually modeled on the cochlear implant or bionic ear devices, a type of neural prosthesis in use since the mid-1980s. The idea of using electrical current to provide sight dates back to the 18th century, discussed by Benjamin Franklin, Tiberius Cavallo, and Charles LeRoy.

<span class="mw-page-title-main">Kapampangan Development Foundation</span>

In the Philippines, the Kapampangan Development Foundation (KDF) is an organization of Kapampangans living outside the region. It was founded in 1987 by 35 people who were inspired by President Corazon Aquino; in 2012, 11 of its founders remained. The KDF has created a rural-industrialization program known as RICH and has provided social and health services to poor residents of Pampanga through its HELP program, with assistance from international and non-governmental organizations and government agencies.

<span class="mw-page-title-main">Nipple prosthesis</span>

Nipple/Areola prostheses are made of silicone by breast prosthesis manufacturers and anaplastologists for breast cancer survivors who were treated for breast cancer with a mastectomy. Prostheses can be worn weeks after a mastectomy, breast reconstruction, or even nipple reconstruction. As an inexpensive and convenient alternative to surgery, patients may choose to wear them anytime during treatment. Patients who ultimately find nipple prostheses thought that they should be informed of them during the consultation prior to mastectomy.

An artificial iris is an intraocular implant that is used as both a cosmetic and to treat those with aniridia or other eye trauma. People with this condition experience photophobia, or increased sensitivity to light. The artificial iris, made from silicone, acts as a replacement iris. The artificial iris is implanted in the eye using different surgery techniques depending on the patient's eye trauma. Most of the major artificial iris manufacturers are European and the treatment is used sparingly in the United States because of limited FDA approval, in part due to possibility of vision loss and other risks.
