Anatomical terminology

The tail of Spence (Spence's tail, axillary process, axillary tail) has historically been described as an extension of the tissue of the upper outer quadrant of the breast traveling into the axilla. [1] The "axillary tail" has been reported to pass into the axilla through an opening in the deep fascia called foramen of Langer. The "tail of Spence" was named after the Scottish surgeon James Spence, who served as a President of the Royal College of Surgeons in Edinburgh in the latter half of the 19th Century. [2] [3] [4]

A recent publication has presented an updated description of the anatomy of the breast and upper outer chest, calling into question the concept of an axillary tail. The report does not challenge that lymphatic drainage consistently extends from the primary breast into the axilla through the foramen of Langer, but does demonstrate that a superolaterally oriented "tail" of breast fat (with or without ductal tissue) is rarely if ever present. Instead, upper lateral chest anatomy is consistently composed of the primary breast itself, a discrete axillary breast mound, and a lateral chest wall tail that never travels superiorly. [5] A review of historical literature shows that Spence himself never wrote that adipose or breast-tissue extends into the axilla. He only published that surgeons should not operate on breast cancer if they found "an undefined tail-like projection creeping up from the breast towards the axilla”, as though referring to the tumor tissue itself. [6] Spence's peers interpreted his remarks and published with differing descriptions, blurring anatomic understanding. [7] [8] Since 1871, a notion evolved of a tail-like extension of fatty tissue originating from the upper outer portion of the breast and traveling into the axilla. Over the next 150 years, the concept became engrained in medical parlance and literature, even though there has never been a detailed anatomic description or published anatomic data to support the anecdotal mention made by Spence and described in various ways by other authors.

At present, the breast is divided into quadrants for clinical reporting and oncological management purposes. It has been solidly established that tumor extension through lymphatics that travel in a contiguous chain from the primary breast into the axilla remains a poor oncologic prognosticator, especially when tumor originates in the upper outer quadrant of the breast. [9] However, for anatomic clarity, oncologists and surgeons may want to consider that the adjacent but separate axillary mound is not a tissue extension of the primary breast. Instead, there is a growing awareness that separate focal vestigial breast mounds are consistently present in adults, located in pairs running down the curved lines of the embryological mammary ridges. [10] It may be of great oncologic and surgical benefit if breast cancer formation and metastasis were reinterpreted in light of this new anatomic understanding. For example, it appears that the axillary mound is more likely to contain accessory ductal tissue than any of the other accessory fatty mounds along each mammary chain, perhaps more commonly serving as a nidus for breast cancer formation, though incidence of tumor formation in the other vestigial breast mounds has not been established.

Breast cancer

Breast cancer can develop in the tail of Spence. One form is referred to as carcinoma of the axillary tail of Spence (CATS). Various studies have calculated CATS as representing 0.1-1% of breast cancers. [11] [9] These cancers may actually represent de novo tumors forming in the anatomically separate axillary breast mounds that were previously thought to be tail-like extensions of the primary breast. [5]

Lumpectomy is a surgical removal of a discrete portion or "lump" of breast tissue, usually in the treatment of a malignant tumor or breast cancer. It is considered a viable breast conservation therapy, as the amount of tissue removed is limited compared to a full-breast mastectomy, and thus may have physical and emotional advantages over more disfiguring treatment. Sometimes a lumpectomy may be used to either confirm or rule out that cancer has actually been detected. A lumpectomy is usually recommended to patients whose cancer has been detected early and who do not have enlarged tumors. Although a lumpectomy is used to allow for most of the breast to remain intact, the procedure may result in adverse affects that can include sensitivity and result in scar tissue, pain, and possible disfiguration of the breast if the lump taken out is significant. According to National Comprehensive Cancer Network guidelines, lumpectomy may be performed for ductal carcinoma in situ (DCIS), invasive ductal carcinoma, or other conditions.

In the context of human evolution, human vestigiality involves those traits occurring in humans that have lost all or most of their original function through evolution. Although structures called vestigial often appear functionless, a vestigial structure may retain lesser functions or develop minor new ones. In some cases, structures once identified as vestigial simply had an unrecognized function. Vestigial organs are sometimes called rudimentary organs. Many human characteristics are also vestigial in other primates and related animals.

A micrometastasis is a small collection of cancer cells that has been shed from the original tumor and spread to another part of the body through the lymphovascular system. Micrometastases are too few in size and quantity to be picked up in a screening or diagnostic test, and therefore cannot be seen with imaging tests such as a mammogram, MRI, ultrasound, PET, or CT scans. These migrant cancer cells may group together to form a second tumor, which is so small that it can only be seen under a microscope. Approximately 90 per cent of people who die from cancer die from metastatic disease, since these cells are so challenging to detect. It is important for these cancer cells to be treated immediately after discovery, in order to prevent the relapse and the likely death of the patient.

Breast cancer management takes different approaches depending on physical and biological characteristics of the disease, as well as the age, over-all health and personal preferences of the patient. Treatment types can be classified into local therapy and systemic treatment. Local therapy is most efficacious in early stage breast cancer, while systemic therapy is generally justified in advanced and metastatic disease, or in diseases with specific phenotypes.

Breast cancer classification divides breast cancer into categories according to different schemes criteria and serving a different purpose. The major categories are the histopathological type, the grade of the tumor, the stage of the tumor, and the expression of proteins and genes. As knowledge of cancer cell biology develops these classifications are updated.

<span class="mw-page-title-main">Male breast cancer</span> Medical condition

Male breast cancer (MBC) is a cancer in males that originates in their breasts. Males account for less than 1% of new breast cancers with about 20,000 new cases being diagnosed worldwide every year. Its incidence rates in males vs. females are, respectively, 0.4 and 66.7 per 100,000 person-years. The worldwide incidences of male as well as female breast cancers have been increasing over the last few decades. Currently, one of every 800 men are estimated to develop this cancer during their lifetimes.

Mammary secretory carcinoma (MSC), also termed secretory carcinoma of the breast, is a rare form of the breast cancers. MSC usually affects women but in a significant percentage of cases also occurs in men and children. Indeed, McDvitt and Stewart first described MSC in 1966 and termed it juvenile breast carcinoma because an increased number of cases were at that time diagnosed in juvenile females. MSC is the most common form of breast cancer in children, representing 80% of childhood breast cancers, although it accounts for less than 0.15% of all breast cancers.

Secretory carcinoma was once used exclusively as a term for rare, slowly growing breast tumors without reference to their location in the breast. It is now termed mammary secretory carcinoma because secretory carcinoma has sometimes been used to name tumors which develop in non-breast tissues but have the microscopic appearance of, and a critical gene abnormality found in, mammary secretory carcinoma. This genetic abnormality is a balanced genetic translocation that forms a ETV6-NTRK3 fusion gene which appears involved in promoting the development and/or progression of all these tumors. Papillary carcinomas of the breast (PCB), also termed malignant papillary carcinomas of the breast, are rare forms of the breast cancers. The World Health Organization (2019) classified papillary neoplasms of the breast into 5 types: intraductal papilloma, papillary ductal carcinoma in situ (PDCIS), encapsulated papillary carcinoma (EPC), solid-papillary carcinoma (SPC), and invasive papillary carcinoma (IPC). The latter four carcinomas are considered here; intraductal papilloma is a benign neoplasm. The World Health Organization regarded solid papillary carcinoma as having two subtypes: in situ and invasive SPC.
