The 2009 Bolivian dengue fever epidemic was an epidemic of dengue fever which struck Bolivia in early 2009, escalating into a national emergency by February. The BBC described it as the worst outbreak of dengue fever in the country's history. At least 18 people died and 31,000 were infected by the mosquito-transmitted arbovirus.

On 20 February, the Pan American Health Organization reported that eighty (80) cases of the more severe dengue hemorrhagic fever had occurred in Bolivia since January, of which 22% were fatal. [1] Bolivia requested outside assistance and foreign aid.

Background

Bolivia first declared a national emergency in early February 2009, when it alerted the world about the country's worst outbreak of dengue fever in twenty-two years. [2] By 3 February, five people had died in the east of the country and over 7,000 more were infected. [2] Worst hit is the Santa Cruz Department, near the Paraguayan border and the Amazon. [2] A number of military facilities, particularly in the city of Santa Cruz, were turned into temporary hospitals as the real hospitals struggled to cope with the conditions. [2] Thousands of soldiers were mobilized to assist medical workers. [2] The government then allocated funds to supply hospitals across the country; however, it was criticised in some quarters for the slowness of its actions. [1] [2] The infection was most widespread in the tropical eastern lowlands, where conditions led to a thriving mosquito population. [1] Bolivia's healthcare services had difficulty in coping with the outbreak, with experts from Venezuela, Cuba, Paraguay and the World Health Organization called in to assist. [1]

Mosquito transmission

Mosquitoes thrive in the high temperatures and humidity of the Bolivian lowlands and it was that region which saw the highest numbers of infected civilians. [1] There is currently no vaccine for dengue fever. [1] Those infected experience flu-like symptoms such as severe headaches, fevers and joint pain. [1] 1 of 4 people infected with dengue will develop symptoms. [3] There is no specific treatment for dengue, other than managing the symptoms. The best way to address dengue is to disrupt the mosquito's habitat. Mosquitos breed in stagnant water, i.e. open storage containers, discarded tires, and uncollected garbage. [4] The infected are advised by medical experts to drink plenty of fluids and obtain significant rest. [1] Dengue fever sufferers have an approximate 1% chance of progressing to the more severe dengue hemorrhagic fever. [1] 1 of 20 individuals who become ill with dengue will develop severe dengue. Severe dengue can lead to life-threatening condition including shock, internal bleeding, or death. [3] Other symptoms include hypothermia, vomiting, severe abdominal pain and confusion. The global average case-fatality ratio for dengue hemorrhagic fever is 5%. [1]

In a small proportion of cases, the disease develops into a more severe dengue hemorrhagic fever, resulting in bleeding, low levels of blood platelets and blood plasma leakage, or into dengue shock syndrome, where dangerously low blood pressure occurs.

Viral hemorrhagic fevers (VHFs) are a diverse group of animal and human illnesses. VHFs may be caused by five distinct families of RNA viruses: the families Filoviridae, Flaviviridae, Rhabdoviridae, and several member families of the Bunyavirales order such as Arenaviridae, and Hantaviridae. All types of VHF are characterized by fever and bleeding disorders and all can progress to high fever, shock and death in many cases. The virus has circulated in forested regions of sub-Saharan African in cycles involving nonhuman primate hosts and arboreal mosquito vectors. Phylogenetic studies indicate that the urban transmission cycle—the transmission of a pathogen between humans and mosquitoes that exist in urban environments—was established on multiple occasions from strains occurring on the eastern half of Africa in non-human primate hosts. This emergence and spread beyond Africa may have started as early as the 18th century. Currently, available data does not indicate whether the introduction of chikungunya into Asia occurred in the 19th century or more recently, but this epidemic Asian strain causes outbreaks in India and continues to circulate in Southeast Asia.

<i>Zika virus</i> Species of flavivirus

Zika virus is a member of the virus family Flaviviridae. It is spread by daytime-active Aedes mosquitoes, such as A. aegypti and A. albopictus. Its name comes from the Ziika Forest of Uganda, where the virus was first isolated in 1947. Zika virus shares a genus with the dengue, yellow fever, Japanese encephalitis, and West Nile viruses. Since the 1950s, it has been known to occur within a narrow equatorial belt from Africa to Asia. From 2007 to 2016, the virus spread eastward, across the Pacific Ocean to the Americas, leading to the 2015–2016 Zika virus epidemic.

<span class="mw-page-title-main">Dengue fever outbreaks</span> Disease outbreak

As of 2010, dengue fever is believed to infect 50 to 100 million people worldwide a year with 1/2 million life-threatening infections. It dramatically increased in frequency between 1960 and 2010, by 30 fold. This increase is believed to be due to a combination of urbanization, population growth, increased international travel, and global warming. The geographical distribution is around the equator with 70% of the total 2.5 billion people living in endemic areas from Asia and the Pacific. Many of the infected people during outbreaks are not virally tested, therefore their infections may also be due to chikungunya, a coinfection of both, or even other similar viruses.

Dengue fever is an important infectious disease in Pakistan with increasingly frequent epidemics. Despite the efforts of the Government of Pakistan, especially in Punjab, the high cost of prevention has limited the ability of Pakistan to control epidemics. In Pakistan, in the summer of 2011, more than 300 people died of Dengue fever. The prevalence of the disease was over 14,000. The outbreaks occurred mostly in the Lahore area, Punjab, Pakistan.

In the 2013 dengue outbreak in Singapore, a significant rise in the number of dengue fever cases was reported in Singapore. The outbreak began in January, with the number of infections beginning to surge in April, before eventually reaching a peak of 842 dengue cases in the week of 16–22 June 2013. This figure was far beyond the highest number of cases per week in the previous three years. Although there were concerns that the rate of infection could exceed 1,000 per week, these fears did not materialize.

<span class="mw-page-title-main">Ebola</span> Viral hemorrhagic fever of humans and other primates caused by ebolaviruses

Ebola, also known as Ebola virus disease (EVD) and Ebola hemorrhagic fever (EHF), is a viral hemorrhagic fever in humans and other primates, caused by ebolaviruses. Symptoms typically start anywhere between two days and three weeks after infection. The first symptoms are usually fever, sore throat, muscle pain, and headaches. These are usually followed by vomiting, diarrhoea, rash and decreased liver and kidney function, at which point some people begin to bleed both internally and externally. It kills between 25% and 90% of those infected – about 50% on average. Death is often due to shock from fluid loss, and typically occurs between six and 16 days after the first symptoms appear. Early treatment of symptoms increases the survival rate considerably compared to late start. An Ebola vaccine was approved by the US FDA in December 2019.

<span class="mw-page-title-main">2015–16 Zika virus epidemic</span> Widespread epidemic of Zika fever

An epidemic of Zika fever, caused by Zika virus, began in Brazil and affected other countries in the Americas from April 2015 to November 2016. The World Health Organization (WHO) declared the end of the epidemic in November 2016, but noted that the virus still represents "a highly significant and long term problem". It is estimated that 1.5 million people were infected by Zika virus in Brazil, with over 3,500 cases of infant microcephaly reported between October 2015 and January 2016. The epidemic also affected other parts of South and North America, as well as several islands in the Pacific.

<span class="mw-page-title-main">2006–2007 East Africa Rift Valley fever outbreak</span> East Africa fever outbreak

East Africa had a regional outbreak of Rift Valley fever in late 2006 that affected Kenya, Somalia, and Tanzania. During outbreak, 1062 people were infected with Rift Valley fever and 394 people died between December 2006 and December 2007. Rift Valley fever (RVF) is caused by a phlebovirus in the Bunyavirales order which is transmitted by mosquito bite and contact with infected animal blood; it mainly infects livestock that come into infectious contact with a viral reservoir but human beings can also be infected. The outbreak began after a heavy El Niño rain season across East Africa left greater than usual breeding ground for Aedes aegypti mosquitos, with particularly heavy rainfall over eastern Kenya, central Tanzania, and southern Somalia. While most people infected with the virus experience a relatively mild, flu-like illness without hospitalization, around 8% will develop a severe illness that can include eye disease, encephalitis, hemorrhagic fever and death. During this outbreak, of the 1,062 hospitalized, laboratory-confirmed RVF cases assays, 37% died.

<span class="mw-page-title-main">2019 dengue outbreak in Bangladesh</span>

The 2019 dengue outbreak in Bangladesh is a nationwide occurrence of dengue fever in Bangladesh that began primarily in April 2019 and is still ongoing. According to Directorate General of Health Services (DGHS), 14 people have died and 19,513 people have been affected as of 1 August 2019, majority of whom are children. Although other sources are reporting the death toll has already passed 50. On 1 August 2019, DGHS confirmed the outbreak affecting all districts of the country. Additionally, it reported 1,712 newly infected patients in the preceding 24 hours which is also the largest number of infected people on record in a single day. According to official source, roughly 71 more people are being infected in dengue fever every hour. Dhaka is the worst-hit city in the country and the districts in Dhaka Division are among the most affected regions. Experts feared that the situation would deteriorate as a lot of people traveled from major cities to rural areas to celebrate Eid al-Adha holidays in August. The Institute of Epidemiology, Disease Control and Research (IEDCR) expects the outbreak to continue till September 2019.

The 2019–2020 dengue fever epidemic was an epidemic of the infectious disease dengue fever in several countries of Southeast Asia, including the Philippines, Malaysia, Vietnam, and Bangladesh, Pakistan, India, Thailand, Singapore, and Laos. The spread of the disease was exacerbated by falling vaccination levels in certain areas, and by a growing population of mosquitoes, which are the primary carriers of the disease, and which are able to reproduce in larger numbers where humans have littered the environment with plastic containers, which provide an ideal breeding ground for mosquitoes. Affected countries have sought to control the epidemic through increased vaccination efforts, and through efforts to control the mosquito population.
