On 4 January 2013, a Britten-Norman Islander light passenger aircraft operated by Transaereo 5074 crashed during a domestic flight from Los Roques Airport, on the Los Roques archipelago, to Caracas, Venezuela, killing all six people on board. [1] Among the victims was Italian fashion entrepreneur Vittorio Missoni.

Discovery of the wreckage

On 27 June 2013, the Deep Sea oceanographic ship confirmed that the missing aircraft had been found in the Caribbean, north of the Los Roques archipelago. [2]

Investigation

During the course of the investigation, it emerged that the pilot had an expired medical certificate and the airline had not yet received the authorization to operate. [3] In a statement from Asdrubal Bermudez, president and owner of the company Transaereo 5074, even though the airline was unable to fly, the plane involved in the incident had met all safety certifications and was allowed to fly.[ citation needed ]

Adam Air Flight 574 2007 aviation accident

Adam Air Flight 574 was a scheduled domestic passenger flight operated by Adam Air between the Indonesian cities of Surabaya and Manado that crashed into the Makassar Strait near Polewali in Sulawesi on 1 January 2007. All 102 people on board died, making it the deadliest aviation accident involving a Boeing 737-400. A national investigation was launched into the disaster. The final report, released on 25 March 2008, concluded that the pilots lost control of the aircraft after they became preoccupied with troubleshooting the inertial navigation system and inadvertently disconnected the autopilot. Despite a series of safety incidents, which contributed to the shut down of Adam Air in June 2008, this was the only incident resulting in fatalities during the airline's 5-year existence.

2008 Los Roques archipelago Transaven Let L-410 crash

On 4 January 2008, a scheduled domestic Transaven flight from Simón Bolívar International Airport to Los Roques Airport, 118 kilometres (64 nmi) north of the departure airport and over water, radioed that both engines had failed and that they were descending through 3,000 feet. The crew was going to attempt a ditching as close as possible to the Los Roques archipelago. Shortly thereafter, radio contact was lost and the plane disappeared from radar.

Santa Bárbara Airlines Flight 518 2008 aviation accident in Venezuela

Santa Bárbara Airlines Flight 518 was an ATR 42–300 twin-turboprop aircraft, registration YV1449, operating as a scheduled domestic flight from Mérida, Venezuela, to Caracas that crashed into the side of a mountain on 21 February 2008, shortly after take-off. There were 43 passengers on board, with a crew consisting of two pilots and a flight attendant. The wreckage was discovered a day later with no survivors. It was the deadliest aviation accident involving an ATR 42 until Trigana Air Flight 267 crashed in Papua, Indonesia, in 2015 with 54 deaths.

LaMia Venezuelan-Bolivian charter airline

LaMia was a Bolivian charter airline headquartered in Santa Cruz de la Sierra, as an EcoJet subsidiary. It had its origins from the failed Venezuelan airline of the same name. Founded in 2015, LaMia operated three Avro RJ85 as of November 2016. The airline received international attention when one of its aircraft crashed in November 2016, killing many members of Brazilian football club Chapecoense. In the aftermath, LaMia's air operator's certificate was suspended by the Bolivian civil aviation authority.

RUTACA Airlines Flight 225 2001 aviation accident

RUTACA Airlines Flight 225(5R225/RUC225) was a domestic tourist passenger flight, operated by Venezuelan-airliner RUTACA Airlines from Canaima Airport to Santiago Mariño Caribbean International Airport that crashed during a refueling stop in Tomás de Heres Airport in Ciudad Bolívar, Venezuela on 25 January 2001. The aircraft was carrying 24 passengers and crewmembers. The aircraft involved in the crash, a Douglas DC-3, crashed into a shantytown shortly after take off from Ciudad Bolívar, killing everyone on board. Eyewitnesses stated that an engine failure had occurred.
