Nikola Mektić and Antonio Veić were the defending champions but Mektić chose to not compete
Veić chose to compete with Franko Škugor and won the title, defeating Facundo Bagnis and Julio César Campozano 7–6(7–5), 4–6, [11–9] in the final.
Martin Kližan was the defending champion, but withdrew before the tournament began.
Marcelo Arévalo and Sergio Galdós were the defending champions but chose to defend their title with different partners. Arévalo partnered Miguel Ángel Reyes-Varela and successfully defended his title. Galdós partnered Roberto Maytín but lost in the first round to Arévalo and Reyes-Varela.
Łukasz Kubot and Marcelo Melo were the defending champions, but they were defeated in the second round by Jonathan Erlich and Marcin Matkowski.
Brian Baker and Nikola Mektić were the defending champions, but Baker chose not to participate this year. Mektić played alongside Alexander Peya, but lost in the first round to Wesley Koolhof and Artem Sitak.
Jürgen Melzer and Franko Škugor defeated Matwé Middelkoop and Frederik Nielsen in the final, 6–4, 7–6(8–6), to win the doubles tennis title at the 2019 Grand Prix Hassan II. They won their first title as a team, having never played together in a prior tournament. It marked Melzer's 15th individual doubles title and Škugor's fourth.

Cabal and Farah saved five match points en route to the title, against Jean-Julien Rojer and Horia Tecău in the quarterfinals. With the win, they jointly attained the ATP number one doubles ranking; Mike Bryan, Łukasz Kubot and Bruno Soares were also in contention for the top ranking.
Henri Kontinen and John Peers were the defending champions, but lost in the second round to Marcel Granollers and Horacio Zeballos.
Jean-Julien Rojer and Horia Tecău were the two-time defending champions, and chose not to participate this year.

Cabal and Farah retained the ATP no. 1 doubles ranking. Mike Bryan, Łukasz Kubot and Nicolas Mahut were also in contention for the top ranking at the start of the tournament.

Defending champion Nikola Mektić and his partner, Mate Pavić, defeated Dan Evans and Neal Skupski in the final, 6–3, 4–6, [10–7], to win the doubles tennis title at the 2021 Monte-Carlo Masters. It was the all-Croatian duo's second consecutive ATP Tour Masters 1000 title together and their fifth title overall of the year. The win also earned Mektić his first successful career title defense. The match was the second successive encounter between both teams in consecutive Masters 1000 finals.
Marcel Granollers and Horacio Zeballos defeated Nikola Mektić and Mate Pavić in the final, 1–6, 6–3, [10–8], to win the men's doubles tennis title at the 2021 Madrid Open. It was the third-seeded duo's third ATP Tour Masters 1000 doubles title together. Mektić and Pavić were contending to be the first doubles team in five years to win a season's first three Masters 1000 events.
