Aeroflot Flight 068 was a regularly scheduled passenger flight operated by Aeroflot from Khabarovsk Novy Airport in Khabarovsk Krai to Pulkovo Airport in Saint Petersburg with intermediate stops at Tolmachevo Airport in Ob, Russia, then Koltsovo Airport in Yekaterinburg. On 16 March 1961, the Tupolev Tu-104B operating this flight crashed shortly after take off from Koltsovo Airport due to engine failure. Two crewmembers and 3 passengers along with two people on the ground, perished.
The Air Accident Investigation Commission concluded the cause of the accident was the failure of engine No. 2 (right) due a broken turbine blade.
After takeoff from Koltsovo Airport while climbing though 140-metre (460 ft) altitude as the crew reduced the engine power from takeoff setting, engine No. 2 began vibrating severely.
The crew was uncertain which engine was failing and were unable to use the engine instruments because the instrument panel was vibrating violently. A crew member decided to reduce power on engine No. 1 in an attempt to determine if it was the source of the shaking, but he inadvertently pulled the throttle lever too far back, shutting down that engine. With neither engine operating the airliner began descending rapidly and the crew decided there was not enough altitude to restart engine No. 1.
Searching for a suitable place to set down, the crew choose to make an emergency landing on a frozen pond 9,200 metres (9.2 km; 5.7 mi; 5.0 nmi) from the airport. Turning 90 degrees to the right and descending the crew lined up for the landing at a high speed with the flaps and landing gear retracted.
Passing beneath an overhead power line and severing it with the vertical stabilizer the Tupolev first struck the ice with its left wing tip, then crashed down on the ice traveling 870 metres (2,854 ft; 951 yd) before reaching the shore. Still moving at speed, the aircraft struck trees near the shore and continued on, destroying part of the ground floor of a two-story vacation home, killing two people inside.
Most of the right wing was torn from the aircraft and remained in the home. Simultaneously the left wing struck another home, tearing off half the wing and causing the home's complete destruction. The aircraft continued on, striking more trees, and came to a halt 350 metres (1,148 ft; 383 yd) from the pond and 1,220 metres (4,003 ft; 1,334 yd) from the initial contact with the ice. There was no fire and the fuselage broke into three main pieces, remaining close together at the crash site. [1] [2] [3]
Construction of the Tupolev Tu-104B involved, serial number 920805, was completed at the Kazan Aircraft Production Association aircraft factory on 24 July 1959. On 4 August 1959 the Tupolev was acquired by Aeroflot. At the time of the accident, the aircraft had sustained a total of 1,600 flight hours and 789 takeoff and landing cycles. [1] [2] [3]
The Air Accident Investigation Commission examining the wreckage discovered engine No. 2 suffered a contained failure and determined the main cause of the accident was the fracture of blade number 54 in the second stage of the turbine section. This blade suffered a fatigue crack along the first groove of the blade lock and detached from the turbine hub at high speed causing massive damage to the engine. The Commission also cited the crew members error in shutting down the No.1 engine as a secondary factor in the crash. [1] [3]
Agents of the Tupolev design bureau and the engine manufacturer stated that because the engine failure was contained the aircraft would have been able to return to the airport on one engine. Based on this opinion their position was that the accidental shut down of engine No. 1 was the main reason for the accident. [1] [3]

It was the second to enter regular service, behind the British de Havilland Comet and was the only jetliner operating in the world from 1956 to 1958, when the British jetliner was grounded due to safety concerns.

Aeroflot Flight 964 was a flight operated by Aeroflot from Kutaisi Airport, Georgia to Domodedovo Airport, Moscow, Russian SFSR. On 13 October 1973, the Tupolev Tu-104 operating on the route crashed during its approach to Moscow, killing all 122 passengers and crew on board. It remains the deadliest accident involving a Tupolev Tu-104.
Aeroflot Flight 3932 was a flight operated by Aeroflot from Koltsovo Airport to Omsk Tsentralny Airport. On 30 September 1973, the Tupolev Tu-104 operating the route crashed shortly after takeoff from Sverdlovsk, killing all 108 passengers and crew on board.
Aeroflot Flight 03 was a passenger flight from Vnukovo Airport to Petropavlovsk-Kamchatsky Airport via Khabarovsk Airport. On 3 September 1962 the Aeroflot Tupolev Tu-104 lost control after the airframe started vibrating, resulting in the plane rolling and yawing several times at an altitude of 4,500 meters before crashing. The aircraft crashed into a swamp, some 90 kilometers away from Khabarovsk. At the time, it was the deadliest crash in the history of Soviet aviation.

Twenty-two people survived the accident. The crash was caused by engine failure brought on by ice ingestion. On 8 May 1985 the Tupolev Tu-134A was officially written off.
Aeroflot Flight 6502 was a Soviet domestic passenger flight operated by a Tupolev Tu-134A from Sverdlovsk to Grozny, which crashed on 20 October 1986; 70 of the 94 passengers and crew on board were killed. The accident occurred when, on a bet, the pilot attempted to make an instrument-only approach with curtained cockpit windows. Investigators determined the cause of the accident was pilot negligence.
Aeroflot Flight 3519 was a Tupolev Tu-154B-2 airline flight on a domestic route from Krasnoyarsk to Irkutsk on 23 December 1984. Shortly after takeoff, the No. 3 engine caught fire, and the airplane crashed during an emergency landing. This killed 110 people; there was only one survivor, and the aircraft was destroyed. The engine fire was caused by a manufacturing defect in the compressor disk.

On 17 February 1966 at 1:38 am local time a Tupolev Tu-114 crashed during take-off from Sheremetyevo International Airport in Moscow, killing 21 of the 47 passengers and 19 crew members on board. This was the only fatal incident involving a Tu-114. A committee investigating the accident found that the crash was due to multiple crew and ATC failures.

Aeroflot Flight 1912 was a scheduled domestic Aeroflot passenger flight on the Odessa-Kiev (Kyiv)-Chelyabinsk-Novosibirsk-Irkutsk-Khabarovsk-Vladivostok route that crashed on 25 July 1971, making a hard landing at Irkutsk Airport. It touched down 150 metres (490 ft) short of the runway, breaking the left wing and catching fire. Of the 126 people on board the aircraft, 29 survived.
Aeroflot Flight 04 was a scheduled domestic passenger flight from Khabarovsk to Moscow with a stopover in Irkutsk that crashed on 15 August 1958, killing all 64 passengers and crew aboard the aircraft. It was the first fatal accident involving a Tupolev Tu-104.

The 1978 Yegoryevsk Tu-144 crash occurred during a test flight of a Tupolev Tu-144 on 23 May 1978. The aircraft suffered a fuel leak, which led to an in-flight fire in the right wing, forcing the shutdown of two of the aircraft's four engines. One of the two remaining engines subsequently failed, forcing the crew to make a belly landing in a field near Yegoryevsk, Moscow Oblast. Two flight engineers were killed in the ensuing crash, but the remaining six crew members survived. The accident prompted a ban on passenger flights of the Tu-144, which had already been beset by numerous problems, leading to a lack of interest that ultimately resulted in the Tu-144 program's cancellation.
