Lucky iron fish are fish-shaped cast iron ingots used to provide dietary supplementation of iron to individuals affected by iron-deficiency anemia. The ingots are placed in a pot of boiling water to leach elemental iron into the water and food. They were developed in 2008 by Canadian health workers in Cambodia, and in 2012 a company, The Lucky Iron Fish Project, was formed to develop the iron fish on a larger scale, promote them among rural areas, and distribute them to non-governmental organization partners.

Research published in 2017 found that the iron ingot had no effect on anemia caused by factors other than iron deficiency. It was therefore not recommended for use in Cambodia and other countries where the majority of anaemia is not due to iron deficiency and the prevalence of genetic hemoglobin disorders is high. [1]

Motivation

About 60% of pregnant Cambodian women are (as of 2010) anemic as a result of dietary iron deficiency, resulting in premature labour and childbirth hemorrhaging. [2] [3] Babies have an increased incidence of brain development problems. [4] Iron deficiency is the "most widespread nutritional disorder" in Cambodia, [5] affecting 44% of the population and resulting in a GDP loss of about $70 billion annually. [6]

Cast-iron cookware is known to transmit iron to food through cooking, but the cost to obtain it is prohibitive for individuals living in poverty in rural Cambodia. [7]

Development

University of Guelph student Christopher Charles received a grant to conduct epidemiological health research in Cambodia from the Canadian International Development Agency (CIDA) after completing an undergraduate degree in biomedical science. [2] The research group distributed an iron disc to women in the village, asking them to place the disc in their pot while making soup or boiling water. The women were reluctant to use the chunk of iron while cooking, and "almost no one used it". [7] Charles stated that it was a "challenge in social marketing". [8] Several weeks before returning to Canada to start his Master's program, Charles called his doctoral advisor Alastair Summerlee to postpone his hormone research project. [8] [2] Summerlee told him to focus his research on preventing iron deficiency anemia, which eventually "spiralled into a PhD project". [2]

Charles and others distributed iron ingots in the shape of a lotus flower, but that was also rejected by the villagers. [8] During discussions with village elders, Charles learned about a fish species deemed a symbol of good luck, health, and happiness in local folklore. [9] The group created fish-shaped iron ingots, which were received more positively by the villagers and led to immediate increases in blood iron levels amongst the villagers. [8] Charles would later state that "You can have the best treatment in the world, but if people won’t use it, it won't matter." [8]

Evaluation

A trial conducted from September 2008 to February 2009 found that the fish increased blood iron levels in individuals for at least three months, but that continued use was found to have negligible long-term effects on blood iron levels. [10] The research spanned the late monsoon season and a dry season that began in December. The villagers relied on harvested monsoon water during the former period, and tube well water during the latter. [11] In the study area, tube well water has high concentrations of arsenic and manganese ions, which act as complexing agents in the presence of iron. [11] This may have resulted in the sequestration of the iron released by the ingot as well as that normally available from the food. [11] The researchers interpreted the difference in water quality as a confounding variable, and conducted a second study to control for it. [12]

Research on iron-arsenic chemistry in the local aquifer sands shows that ferric oxides are associated with low-arsenic groundwater, whereas ferrous oxides indicate higher arsenic concentrations. [13]

The subsequent trial found that, compared to the base blood iron rate at the beginning of the trial, individuals using the iron fish had increased levels of blood iron after 12 months, and the rate of anemia decreased by 43%. [6]

A randomized control trial in 2017 found that the iron fish did not increase hemoglobin concentrations in a sample group of 340 Cambodian women. These women did have anemia, but it was not attributed to low levels of iron. The conclusion of the study stated "We do not recommend the use of the fish-shaped iron ingot in Cambodia or in countries where the prevalence of iron deficiency is low and genetic hemoglobin disorders are high". [1]

Production

In December 2012, University of Guelph biomedical science doctoral student Gavin Armstrong established The Lucky Iron Fish Project, a company founded to commercialize the concept. [6] [14] The company employs "hard-to-employ Cambodians"[ clarification needed ] to manufacture the fish-shaped iron ingots using recycled scrap iron from a nearby factory. [2] [14] The company began promoting the product in a few rural villages in October 2013, and began a full marketing campaign in Kandal Province in January 2014. [14]

In May 2014, the company announced that it had obtained CA$ 860,000 in financing, including CA$ 500,000 from Grand Challenges Canada, for scale production of 60,000 iron fish at a cost of about CA$ 5 per fish. [15] [16] It also received $1.1 million from private investors. [6] The company had delivered about 11,000 units to non-governmental organization partners in the first five months of 2014. [3] [6] It sells the units when possible, as it is more likely that individuals will use it if it is purchased, but will also distribute if for free where needed. [6] It also planned to sell units in Canada for $25, each purchase accompanied by the donation of three units in Cambodia. [6] As of November 2015, the company donates one unit for each unit purchased. [17]

On 28 April 2014, the company announced in a press release that it had obtained the B Corporation certification issued by the nonprofit organisation B Lab. [18]

Use

The smiling iron fish is 3 inches (7.6 cm) long and weighs about 200 grams (7.1 oz). [3] It is placed in a cooking vessel with 1 or 2 drops of lemon juice for 10 minutes in 1 litre of boiling water. [2] [4] [12] The lemon juice improves intestinal iron absorption. [2] The iron fish is also used while cooking rice or stews. [3] Approximately 92% of villagers who have an iron fish use it regularly, and many of them recommend it to friends and family as a symbol of luck. [7]

The flattened ingot was designed to maximize the exposed surface area during cooking, thus maximizing the amount of iron leached from the ingot to the food or water in the pot. [11] Its use results in the average villager obtaining about 75% of the daily recommended dietary iron. [6]

Armstrong states that the iron does not change the taste of the water or food into which it is leached. [4] The project targets youth in Cambodia to promote the project because of the high level of adult illiteracy in its rural areas. [14]

Iron-deficiency anemia is anemia caused by a lack of iron. Anemia is defined as a decrease in the number of red blood cells or the amount of hemoglobin in the blood. When onset is slow, symptoms are often vague such as feeling tired, weak, short of breath, or having decreased ability to exercise. Anemia that comes on quickly often has more severe symptoms, including confusion, feeling like one is going to pass out or increased thirst. Anemia is typically significant before a person becomes noticeably pale. Children with iron deficiency anemia may have problems with growth and development. There may be additional symptoms depending on the underlying cause.

Hypochromic anemia is a generic term for any type of anemia in which the red blood cells are paler than normal. A normal red blood cell has a biconcave disk shape and will have an area of pallor in its center when viewed microscopically. In hypochromic cells, this area of central pallor is increased. This decrease in redness is due to a disproportionate reduction of red cell hemoglobin in proportion to the volume of the cell. Clinically the color can be evaluated by the mean corpuscular hemoglobin (MCH) or mean corpuscular hemoglobin concentration (MCHC). The MCHC is considered the better parameter of the two as it adjusts for effect the size of the cell has on its amount of hemoglobin. Iron supplements, also known as iron salts and iron pills, are a number of iron formulations used to treat and prevent iron deficiency including iron deficiency anemia. For prevention they are only recommended in those with poor absorption, heavy menstrual periods, pregnancy, hemodialysis, or a diet low in iron. Prevention may also be used in low birth weight babies. They are taken by mouth, injection into a vein, or injection into a muscle. While benefits may be seen in days, up to two months may be required until iron levels return to normal.

Anemia is a deficiency in the size or number of red blood cells or in the amount of hemoglobin they contain. This deficiency limits the exchange of O2 and CO2 between the blood and the tissue cells. Globally, young children, women, and older adults are at the highest risk of developing anemia. Anemia can be classified based on different parameters, and one classification depends on whether it is related to nutrition or not so there are two types: nutritional anemia and non-nutritional anemia. Nutritional anemia refers to anemia that can be directly attributed to nutritional disorders or deficiencies. Examples include Iron deficiency anemia and pernicious anemia. It is often discussed in a pediatric context.

Congenital hemolytic anemia (CHA) is a diverse group of rare hereditary conditions marked by decreased life expectancy and premature removal of erythrocytes from blood flow. Defects in erythrocyte membrane proteins and red cell enzyme metabolism, as well as changes at the level of erythrocyte precursors, lead to impaired bone marrow erythropoiesis. CHA is distinguished by variable anemia, chronic extravascular hemolysis, decreased erythrocyte life span, splenomegaly, jaundice, biliary lithiasis, and iron overload. Immune-mediated mechanisms may play a role in the pathogenesis of these uncommon diseases, despite the paucity of data regarding the immune system's involvement in CHAs.

Latent iron deficiency (LID), also called iron-deficient erythropoiesis, is a medical condition in which there is evidence of iron deficiency without anemia. It is important to assess this condition because individuals with latent iron deficiency may develop iron-deficiency anemia. Additionally, there is some evidence of a decrease in vitality and an increase in fatigue among individuals with LID.

Iron(III)-hydroxide polymaltose complex is a medication used to treat iron deficiency / iron deficiency anemia and belongs to the group of oral iron preparations. The preparation is a macromolecular complex, consisting of iron(III) hydroxide (trivalent iron, Fe3+, Fe(OH)3·H2O) and the carrier polymaltose and is available in solid form as a film-coated or chewable tablet and in liquid form as a syrup, drinkable solution, or drops. It is used for treating iron deficiency without anemia (latent iron deficiency) or with anemia (apparent iron deficiency). Prior to administration, the iron deficiency should be diagnostically established and verified via laboratory tests (e.g., low ferritin concentration, low transferrin saturation).

Anemia is a condition in which blood has a lower-than-normal amount of red blood cells or hemoglobin. Anemia in pregnancy is a decrease in the total red blood cells (RBCs) or hemoglobin in the blood during pregnancy. Anemia is an extremely common condition in pregnancy world-wide, conferring a number of health risks to mother and child. While anemia in pregnancy may be pathologic, in normal pregnancies, the increase in RBC mass is smaller than the increase in plasma volume, leading to a mild decrease in hemoglobin concentration referred to as physiologic anemia. Maternal signs and symptoms are usually non-specific, but can include: fatigue, pallor, dyspnea, palpitations, and dizziness. There are numerous well-known maternal consequences of anemia including: maternal cardiovascular strain, reduced physical and mental performance, reduced peripartum blood reserves, increased risk for peripartum blood product transfusion, and increased risk for maternal mortality.
