Matteo Berrettini defeated Cameron Norrie in the final, 6–4, 6–7(5–7), 6–3, to win the singles tennis title at the 2021 Queen's Club Championships. Alex de Minaur was the reigning champion from when the tournament was last held in 2019, but chose not to participate this year.

Jannik Sinner defeated Mackenzie McDonald in the final, 7–5, 4–6, 7–5 to win the singles tennis title at the 2021 Citi Open. With the victory, Sinner became the youngest player and first teenager on the ATP Tour to win an ATP 500 title since the category was created in 2009.

John Millman was the defending champion, but lost in the quarterfinals to James Duckworth.
