Bemethyl, also commonly referred to in literature as bemitil, is a synthetic actoprotector which is also antihypoxant (combating conditions of hypoxia), antioxidant, and antimutagenic. Bemethyl is primarily classified as an actoprotector; a synthetic adaptogen with significant capacity to increase physical performance. [1]
It is approved for use in Ukraine as a dietary supplement. Bemethyl is commonly used in preparing for international competitions by Ukrainian national sport teams. [1]
Bemethyl is formulated as a hydrobromide salt. Its parent compound is 2-ethylsulfanyl-1H-benzimidazole. [2]
Metabolic modulation
Bemethyl also has a positive effect on metabolic processes, allowing adaptation to conditions causing frequent hypoxia, and the obtained effect is long-lasting and also occurs after the end of dosage. [3]
Anti-hypoxant
Bemethyl has been shown to preserve both physical and mental capacity in high-altitude, low-oxygen environments, particularly by its effect in helping control excess serum levels of cholesterol and bilirubin, which are known to have negative effects especially during adjustment to high-altitude environments. [4]
Recovery from trauma
Bemethyl has also been shown to prevent permanent hearing loss and facilitate recovery of hearing after mine-explosion trauma, when treatment is initiated immediately after injury. [5]
In one study, bemethyl was shown to prevent the mutagenic effect of white asbestos in mice and in cultured human whole blood. [6]
A study using mice showed bemethyl to reduce mutation induced by certain mutagenic drugs. [7]
Another study using cells from human donors showed Bemethyl to be anticlastogenic (able to minimize chromosome breakages). [8]
Anti-asthenic
A study using mice established that both single and continuous dosing of bemethyl increases work capacity and accelerates rehabilitation after exhaustive exertion. [1]
The mechanistic underpinnings of bemethyl's effects are not well understood, but some evidence implicate alterations in protein synthesis and glucose metabolism, in addition to antioxidant activity, as relevant mechanisms of action. [1]
Bemethyl resists metabolization and is long-lived, accumulating in tissues over course of treatment. In one study involving rats, long-term administration of Bemethyl was accompanied by a 1.38-fold increase in drug concentration in the brain, and a 1.68-fold increase in its concentration in skeletal muscles. [9]
Bemethyl was developed in the 1970s by the Department of Pharmacology of the St. Petersburg State Military Medical Academy under the direction of Professor Vladimir Vinogradov. Professor Vinogradov and his research team earned the USSR State Prize for this accomplishment. [1]
First used with Soviet cosmonauts, Bemethyl was also used to prepare athletes of the USSR national team for the Moscow 1980 Olympic Games. In the 1990s, bemethyl saw use as a basic medicinal agent in many of the corps of the Soviet and Russian armies, including Soviet troops in Afghanistan, as bemethyl facilitated increased endurance for soldiers over long marches, as well as an enhanced work capacity and stability to hypoxia and high temperatures. Bemethyl was also used to enhance the physical and mental capacities of workers deployed in the wake of the 1986 Chernobyl disaster. [1]
In 2018, bemethyl was added to the Monitoring Program of the World Anti-Doping Agency. [10] As of 2021 it remains in the Monitoring Program, [11] and therefore is not on the Prohibited List; presently only being monitored to potentially detect patterns of misuse in sport. The WADA Monitoring Program is also recognized by the U.S. Anti-Doping Agency, [12] as bemethyl continues not to be a "banned substance" in sports.
Erythropoietin, also known as erythropoetin, haematopoietin, or haemopoietin, is a glycoprotein cytokine secreted mainly by the kidneys in response to cellular hypoxia; it stimulates red blood cell production (erythropoiesis) in the bone marrow. Low levels of EPO are constantly secreted in sufficient quantities to compensate for normal red blood cell turnover. Common causes of cellular hypoxia resulting in elevated levels of EPO include any anemia, and hypoxemia due to chronic lung disease.

Headquartered in Colorado Springs, Colorado, USADA is a signatory to the World Anti-Doping Code, which harmonizes anti-doping practices around the world, and is widely considered the basis for the strongest and strictest anti-doping programs to prevent doping in sport.
In competitive sports, doping is the use of banned athletic performance-enhancing drugs by athletic competitors, as a way of cheating. The term doping is widely used by organizations that regulate sporting competitions. Trimetazidine is a drug for angina pectoris sold under many brand names. Trimetazidine is described as the first cytoprotective anti-ischemic agent developed and marketed by Laboratoires Servier (France). It is an anti-ischemic (antianginal) metabolic agent of the fatty acid oxidation inhibitor class, meaning that it improves myocardial glucose utilization through inhibition of fatty acid metabolism.
Phenylpiracetam, is a phenylated analog of the drug piracetam. It was developed in 1983 as a medication for Soviet Cosmonauts to treat the prolonged stresses of working in space. Phenylpiracetam was created at the Russian Academy of Sciences Institute of Biomedical Problems in an effort led by psychopharmacologist Valentina Ivanovna Akhapkina. In Russia it is now available as a prescription drug. Research on animals has indicated that phenylpiracetam may have anti-amnesic, antidepressant, anticonvulsant, anxiolytic, and memory enhancement effects.
Performance-enhancing substances, also known as performance-enhancing drugs (PEDs), are substances that are used to improve any form of activity performance in humans. A well-known example of cheating in sports involves doping in sport, where banned physical performance-enhancing drugs are used by athletes and bodybuilders. Athletic performance-enhancing substances are sometimes referred as ergogenic aids. Cognitive performance-enhancing drugs, commonly called nootropics, are sometimes used by students to improve academic performance. Performance-enhancing substances are also used by military personnel to enhance combat performance.

5-Aminoimidazole-4-carboxamide ribonucleotide (AICAR) is an intermediate in the generation of inosine monophosphate. AICAR is an analog of adenosine monophosphate (AMP) that is capable of stimulating AMP-dependent protein kinase (AMPK) activity. The drug has also been shown as a potential treatment for diabetes by increasing the metabolic activity of tissues by changing the physical composition of muscle.
Tuaminoheptane is a sympathomimetic agent and vasoconstrictor which was formerly used as a nasal decongestant. It has also been used as a stimulant.
Erythropoiesis-stimulating agents (ESA) are medications which stimulate the bone marrow to make red blood cells. They are used to treat anemia due to end stage kidney disease, chemotherapy, major surgery, or certain treatments in HIV/AIDS. In these situations they decrease the need for blood transfusions. The different agents are more or less equivalent. They are given by injection.
Efaproxiral (INN) is an analogue of the cholesterol drug bezafibrate developed for the treatment of depression, traumatic brain injury, ischemia, stroke, myocardial infarction, diabetes, hypoxia, sickle cell disease, hypercholesterolemia and as a radio sensitiser.
Bromantane, sold under the brand name Ladasten, is an atypical psychostimulant and anxiolytic drug of the adamantane family related to amantadine and memantine which is used in Russia in the treatment of neurasthenia. Although the effects of the bromantane have been determined to be dependent on the dopaminergic and possibly serotonergic neurotransmitter systems, its exact mechanism of action is unknown, and it is distinct in its properties relative to typical psychostimulants such as amphetamine. Because of its unique aspects, bromantane has sometimes been described instead as an adaptogen and actoprotector.
Fabomotizole is an anxiolytic drug launched in Russia in the early 2000s. It produces anxiolytic and neuroprotective effects without any sedative or muscle relaxant actions. Its mechanism of action remains poorly defined however, with GABAergic, NGF- and BDNF-release-promoting, MT1 receptor agonism, MT3 receptor antagonism, and sigma agonism suggested as potential mechanisms. Fabomotizole was shown to inhibit MAO-A reversibly and there might be also some involvement with serotonin receptors. Clinical trials have shown fabomotizole to be well tolerated and reasonably effective for the treatment of anxiety.
Emoxypine (2-ethyl-6-methyl-3-hydroxypyridine), also known as Mexidol or Mexifin when used as the succinate salt, is an antioxidant actoprotector manufactured by Pharmasoft Pharmaceuticals. Its chemical structure resembles that of pyridoxine (a type of vitamin B6).
Meldonium is a limited-market pharmaceutical, developed in 1970 by Ivars Kalviņš at the USSR Latvia Institute of Organic Synthesis, and now manufactured by the Latvian pharmaceutical company Grindeks and several generic manufacturers. It is primarily distributed in Eastern European countries as an anti-ischemia medication.
Actoprotectors or synthetic adaptogens are compounds that enhance an organism's resilience to physical stress without increasing heat output. Actoprotectors are distinct from other performance-enhancing substances in that they increase physical and psychological resilience via non-exhaustive action. The term "actoprotector" is used to describe synthetic and isolated compounds possessing adaptogenic properties. By contrast, the term "adaptogen" is most often use to describe a natural herb as a whole, which can contain hundreds if not thousands of biologically active components.
Doping in figure skating involves the use of illegal performance-enhancing drugs (PEDs), specifically those listed and monitored by the World Anti-Doping Agency (WADA). Figure skaters occasionally have positive doping results but it is not common. Bans can be enforced on figure skaters by the International Skating Union (ISU) and each country's individual skating federation. These bans can often be career ending due to the competitive nature of figure skating. A ban may be revoked if it can be proved that the skater tested positive for a prescribed medication. Some figure skaters will use PEDs to help with recovery time, allowing them to train harder and longer. Figure skating is an aesthetic sport that combines both athleticism and artistic licence, where weight-loss substances will have little effect on athletic performance but skaters may be perceived as more graceful and sleek, which is required for an athlete to be competitive.
Gene doping is the hypothetical non-therapeutic use of gene therapy by athletes in order to improve their performance in those sporting events which prohibit such applications of genetic modification technology, and for reasons other than the treatment of disease. As of April 2015, there is no evidence that gene doping has been used for athletic performance-enhancement in any sporting events. Gene doping would involve the use of gene transfer to increase or decrease gene expression and protein biosynthesis of a specific human protein; this could be done by directly injecting the gene carrier into the person, or by taking cells from the person, transfecting the cells, and administering the cells back to the person.

The World Anti-Doping Code (Article 4.5) states: "WADA, in consultation with Signatories and governments, shall establish a monitoring program regarding substances which are not on the Prohibited List, but which WADA wishes to monitor in order to detect patterns of misuse in sport."
