China Airlines Flight 358 was a Boeing 747-2R7F/SCD freighter that crashed on December 29, 1991, shortly after takeoff from Chiang Kai-shek International Airport near Taipei, Taiwan. All five crew members were killed.
The aircraft was a Boeing 747-2R7F/SCD, built in September 1980 for Cargolux as the City of Esch-sur-Alzette, registration LX-ECV, MSN 22390. It was acquired by China Airlines in June 1985 was given its registration of B-198. It had been in service for 11 years, 3 months. [1] The aircraft had clocked a total of 45,868 hours of flight time during its time in service. The last A-check maintenance had occurred on 21 December 1991, and the aircraft had accumulated 74 hours of flight time since that point. [1]
The aircraft was the same one involved in the China Airlines Flight 334 hijacking on 3 May 1986.
Several minutes after takeoff, the crew reported problems with the #2 engine, prompting Taipei air traffic control (ATC) to vector the flight into a left turn to return to the airport. Approximately two minutes later, the crew reported that they were unable to turn left, and ATC approved a right-hand turn instead. This was the last radio contact made by the crew. The crew lost control of the aircraft and it struck a hill, right wing first, near Wanli, Taipei. The crash occurred at approximately 3:05 p.m, at an altitude of 700 feet (210 m). [1] All five crew members died in the crash, and there were no injuries on the ground.
The subsequent investigation revealed that the number 3 engine and its pylon had separated from the aircraft and struck the number 4 engine, breaking it off the wing as well. [1] A more detailed investigation revealed that the pylon midspar fittings, which attach the pylon to the lower portion of the wing front spar, had failed. The search for the number 3 engine and its pylon, which landed in the sea, took several months.[ citation needed ]
Information from the investigation of this crash and the nearly identical crash of El Al Flight 1862 ten months later resulted in Boeing ordering pylon modifications to every 747 in use. [2]

American Airlines Flight 191 was a regularly scheduled domestic passenger flight from O'Hare International Airport in Chicago to Los Angeles International Airport. On the afternoon of May 25, 1979, the McDonnell Douglas DC-10 operating this flight was taking off from runway 32R at O'Hare International when its left engine detached from the wing, causing a loss of control. The aircraft crashed about 4,600 feet (1,400 m) from the end of runway 32R. All 271 occupants on board were killed on impact, along with two people on the ground. With a total of 273 fatalities, the disaster is the deadliest aviation accident to have occurred in the United States.
China Airlines Flight 611 was a regularly scheduled passenger flight from Chiang Kai-shek International Airport in Taiwan to Hong Kong International Airport in Hong Kong.
Singapore Airlines Flight 006 was an international scheduled passenger flight from Singapore Changi Airport to Los Angeles International Airport via Chiang Kai-shek International Airport near Taipei, Taiwan. On 31 October 2000, at 11:18pm Taipei local time, the Boeing 747-412 operating the flight attempted to take off from the wrong runway at Chiang Kai-shek International Airport during a typhoon. The aircraft crashed into construction equipment on the runway, killing 83 of the 179 people aboard. Ninety-eight occupants initially survived the accident, but two passengers died later from injuries in the hospital. This was the first fatal crash involving a Boeing 747-400, and also the first fatal accident in the history of Singapore Airlines.
As of January 2025, a total of 64 Boeing 747 aircraft, or just above 4% of the total number of 747s built, first flown commercially in 1970, have been involved in accidents and incidents resulting in a hull loss, meaning that the aircraft was either destroyed or damaged beyond economical repair. Of the 64 Boeing 747 aircraft losses, 32 resulted in no loss of life; in one, a hostage was murdered; and in one, a terrorist died.
On 4 October 1992, El Al Flight 1862, a Boeing 747 cargo aircraft of the Israeli airline El Al, crashed into the Groeneveen and Klein-Kruitberg flats in the Bijlmermeer neighbourhood of Amsterdam, the Netherlands. The accident is known in Dutch as the Bijlmerramp.

UPS Airlines Flight 6 was a scheduled international cargo flight operated by UPS. On September 3, 2010, the Boeing 747-400F flying the route between Dubai, United Arab Emirates, and Cologne, Germany, developed an in-flight fire, which caused the aircraft to crash, killing both crew members, the only people on board. It was the first fatal air crash for UPS Airlines. The accident prompted a re-evaluation of safety procedures protecting airliners from cockpit smoke.
Asiana Airlines Flight 991 was a cargo flight operated by Asiana Airlines. On 28 July 2011, the Boeing 747-400F flying from Seoul, South Korea, to Shanghai, China, crashed into the sea off Jeju Island after suffering an in-flight cargo fire. Both pilots, the only two people on board, were killed. The accident marked the second loss of a 747 freighter due to a cargo hold fire in less than a year, following the crash of UPS Airlines Flight 6 in Dubai in September 2010.
China Airlines Flight 334 was a Boeing 747-2R7F/SCD freighter aircraft that was hijacked by pilot Wang Hsi-chueh, a former military U-2 pilot, on May 3, 1986, while en route to Don Mueang, Thailand. Wang had left family members behind in China when he left for Taiwan in 1949 and had met some of them in Hong Kong in 1984. He decided to defect in order to reunite with his family in China. Wang managed to subdue the two other crew members and changed course to land the 747 in Guangzhou, where he defected to the People's Republic of China. The incident forced the Chiang Ching-kuo government in Taiwan to reverse its Three Noes policy in regard to contacting the communist government in mainland China, and Chiang dispatched several delegates to Hong Kong to negotiate with mainland officials for the return of the aircraft and crew. The incident was credited as a catalyst in renewing cross-strait relations between mainland China and Taiwan.
In aeronautics, loss of control (LOC) is the unintended departure of an aircraft from controlled flight and is a significant factor in several aviation accidents worldwide. In 2015 it was the leading cause of general aviation accidents. Loss of control may be the result of mechanical failure, external disturbances, aircraft upset conditions, or inappropriate crew actions or responses.
TransAsia Airways Flight 235 was a domestic flight from Taipei to Kinmen, Taiwan. On 4 February 2015, the aircraft serving the flight, a 10-month-old ATR 72-600, crashed into the Keelung River around 5 km from Taipei Songshan Airport, where the aircraft had just departed from. On board were 58 people, 15 of whom survived with injuries.
Centurion Air Cargo Flight 164 was a chartered international cargo flight, flying from Bogota's El Dorado International Airport while en route to Miami International Airport. The flight was operated by Kalitta Air and the aircraft was wet leased by Centurion Air Cargo. On 7 July 2008, the aircraft, a Boeing 747-209BSF registered as N714CK, crashed shortly after takeoff. All aboard suffered injuries, but none were killed. Two people on the ground were killed after the plane slammed into a farm. The crash was the second crash of a Boeing 747 in 2008 in Kalitta Air service, after a previous accident at Brussels in May.
Trans-Air Service Flight 671 was a cargo flight from Luxembourg Airport to Mallam Aminu Kano International Airport in Kano, Nigeria. While flying over France on 31 March 1992, the Boeing 707 operating the flight experienced an in-flight separation of two engines on its right wing. Despite the damage to the aircraft, the pilots were able to perform an emergency landing at Istres-Le Tubé Air Base in Istres, France. All five occupants of the aircraft survived; the aircraft was damaged beyond repair due to a fire on the right wing.
Kalitta Air Flight 207 (K4207/CKS207) was a scheduled cargo flight between John F. Kennedy Airport to Bahrain International Airport with a technical stopover at Brussels. On May 25, 2008, the Boeing 747-200 suffered a bird strike and overran runway 20 during takeoff at Brussels Airport, causing the aircraft to split into three large pieces. The occupants sustained minor injuries.
Omega Aerial Refueling Services Flight 70 was scheduled to provide aerial refueling to US Navy F/A-18 fighter jets. On May 18, 2011, it crashed on takeoff following engine separation. All 3 crew members on-board survived.
Japan Air Lines Cargo Flight 46E was a scheduled cargo flight on 31 March 1993, operated by Evergreen International Airlines, on behalf of Japan Air Lines, from Anchorage International Airport, in Anchorage, Alaska, to O'Hare International Airport, in Chicago. After departure, while climbing through 2,000 feet, the pylon for engine two detached, causing the whole engine to fall off the wing. The pilots managed to land the 747 back at Anchorage without further incident.
