On 29 July 2011, a Boeing 777 on a scheduled passenger flight from Cairo, Egypt, to Jeddah, Saudi Arabia, flying as Egyptair Flight 667, suffered a cockpit fire on the ground at Cairo Airport, while preparing to depart. There were no fatalities, but seven people were treated for smoke inhalation. The aircraft was damaged beyond economic repair. [1]
The subsequent investigation found that the fire had originated in the crew's emergency oxygen system storage area, but was unable to conclusively determine the source of ignition, nor the cause of the oxygen leak that fuelled the fire.
The EgyptAir Boeing 777 was parked at gate F7 of Terminal 3 at Cairo airport on 29 July 2011, with preparations underway to operate flight 667. As the last few of the 307 passengers were boarding, the flight crew in the cockpit heard a bang and a hissing sound coming from the right side console, immediately followed by smoke and flames. The first officer jumped out of his seat and was ordered by the captain to leave the cockpit, while the captain unsuccessfully attempted to put out the fire using the on-board fire extinguisher. According to the cockpit voice recorder, noises occur at 9:11:38 that the final report characterizes as "a pop is heard, followed by a hissing sound similar to the escape of pressurized gas". [2] : 24 Three seconds later, the captain instructs the first officer to "get up, get out now." [2] : 24 Twenty-one seconds later, the captain says "fire, fire, call for fire". [2] : 24 The fire erupted about 30 minutes after the first officer performed a test of the oxygen mask's function during the standard pre-flight checklist. [2] : 97, 109 The passengers were immediately evacuated through the two connected jet bridges. [2] : 86
Airport fire crews arrived on the scene about three minutes after the alarm was raised and quickly extinguished the fire. Two firefighters and five passengers and crew were hospitalized for smoke inhalation. [3]
The aircraft was subsequently written off, having sustained severe structural damage in the cockpit area as well as extensive heat and smoke damage in the cabin. [3] This was the second hull loss of a Boeing 777 after British Airways Flight 38.[ citation needed ]
The aircraft involved was a Boeing 777-266ER named Nefertiti, registered as SU-GBP with serial number 28423 (line number 71). It was manufactured by Boeing Commercial Airplanes in 1997 and in its 14 years of service, it had accumulated 48,281 hours and 27 minutes of flight time in 11,448 flight cycles. [2] : 20
The captain was a 49-year-old who had logged 16982 hours and 37 minutes of flying time, 5314 hours and 22 minutes of which were logged on the Boeing 777. His co-pilot was a 25-year-old who had logged 2247 hours and 1 minute of flying time, including 198 hours and 10 minutes logged on the Boeing 777. [2] : 16–18
Since the area where the fire originated has no fuel lines, oil lines, or hydraulic lines, the investigation focused on the crew oxygen supply system as the root cause or a major contributing factor. [2] : 59 In the days after the fire, the area of the cockpit where the fire originated on SU-GBP was examined on all EgyptAir 777-200 and 777-300 aircraft. According to the investigation's final report, the wiring found in the oxygen light plate did not match the original Boeing design: [2] : 60
The 777-200 aircraft were found to differ from Boeing’s current design. In particular, the wiring to the first officer’s oxygen mask light plate differed in the follow aspects: a wire clamp was missing, the wiring was not sleeved, and a large loop of unsupported wire was found. All of EgyptAir’s 777-200 had a similar wiring configuration at the first officer’s oxygen mask location. The captain’s side wiring was similar, except that sleeving was present on all airplanes inspected. ... On one of the 777-200 aircraft, the outer layer of wiring insulation was found damaged, although the inner layer was intact and the conductor was not exposed.
The reason for the missing clamps could not be determined. [2] : 60 Approximately 380 early 777 aircraft did not have sleeves on the wire in the oxygen light. [2] : 35 Boeing issued a service bulletin in October 2011 recommending that the oxygen light plate wiring be inspected and, if necessary, have sleeving installed and damaged wires replaced. [2] : 61
Previous testing by the US National Transportation Safety Board following a 2008 cockpit fire on a Boeing 767 had found that flexible oxygen supply hoses could ignite if anti-kink springs along the oxygen supply hose were exposed to an electric current. [4] The hoses in SU-GBP were composed of two layers of silicone with the spring embedded in the outer layer, unlike the hose in the NTSB tests. Testing performed by Boeing on 777 crew oxygen hoses found two of the seven tested to be conductive. Further testing in conditions that increased the likelihood of ignition showed that 5 volts of direct current was not sufficient to destroy the hose. However, the final report notes that these tests were conducted on new hoses and that Boeing planned to conduct similar tests on older hoses from in-service aircraft to see if any age-related changes could have affected the results. The same test was also performed at voltages and amperage of aircraft wiring found elsewhere in the cockpit. With air in the hose, applying 28 volts DC power at 5 amps of current or 115 volts AC power at 2.5 amps did not rupture the hose or cause ignition, but 28 V DC at 6 amps or 115 V AC at 5 amps resulted in a small leak with "some incandescence". [2] : 42 With oxygen in the tube, 5 V DC at 1.2 amps or 28 V DC at 2.5 amps did not result in a rupture, but 28 V DC at 5 amps caused a leak "followed by ignition and complete rupture of hose [ sic ]." [2] : 42 Boeing examined the design of the oxygen supply to determine possible sources of electricity: [2] : 42
With the exception of the oxygen mask microphone wire (which was found to carry only milliamp level signal currents), all of the wiring in the area of the captain's and first officer's oxygen system followed the design requirements for separation. However, it was determined that contact between aircraft wiring and oxygen system components may be possible if multiple wire clamps are missing or fractured or if wires are incorrectly installed.
Adiabatic heating from the sudden release of pressurized oxygen into the hose was another possible source of ignition, but ruled out. Window grease in an oxygen-rich environment was another source of ignition that was investigated, but testing determined that the oxygen level did not impact the flashpoint, which was over 200 °F above the maximum theoretical temperature in that part of the cockpit. The investigation did not conclusively determine the cause of the fire, only that the fire originated near the first officer's oxygen mask supply tubing and that oxygen from the first officer's supply mask "is suspected to have contributed to the fire's intensity and speed", [2] : 65 although it is not known whether a breach of the oxygen system provided a flammable environment for the fire to start or whether the oxygen system breached as a result of the fire, and an electrical fault, possibly in anti-kink springs in flexible oxygen supply hoses, may have caused the fire. Trans World Airlines Flight 800 (TW800/TWA800) was a scheduled international passenger flight from New York to Rome, with a stopover in Paris. On July 17, 1996, at approximately 8:31 p.m. EDT, 12 minutes after takeoff, the Boeing 747-100 serving the flight exploded and crashed into the Atlantic Ocean near East Moriches, New York.
The Boeing 777, commonly referred to as the Triple Seven, is an American long-range wide-body airliner developed and manufactured by Boeing Commercial Airplanes. The 777 is the world's largest twinjet and the most-built wide-body airliner. The jetliner was designed to bridge the gap between Boeing's other wide body airplanes, the twin-engined 767 and quad-engined 747, and to replace aging DC-10 and L-1011 trijets. Developed in consultation with eight major airlines, the 777 program was launched in October 1990, with an order from United Airlines. The prototype aircraft rolled out in April 1994, and first flew in June of that year. The 777 entered service with the launch operator United Airlines in June 1995. Longer-range variants were launched in 2000, and first delivered in 2004.
EgyptAir Flight 648 was a regularly scheduled international flight between Athens Ellinikon International Airport (Greece) and Cairo International Airport (Egypt). On 23 November 1985, a Boeing 737-266 airliner, servicing the flight was hijacked by the Palestinian terrorist organization Abu Nidal. The subsequent raid on the aircraft by Egyptian troops killed 56 of the 86 passengers, 2 of the 3 hijackers and 2 of the 6 crew, making the hijacking of Flight 648 one of the deadliest such incidents in history.
Flash Airlines was a private charter airline operating out of Cairo, Egypt that was part of the Flash Group tourism company. The airline operated two Boeing 737-3Q8 aircraft manufactured in 1993 on non-scheduled commercial passenger flights on both international and domestic routes.
Japan Air Lines Flight 123 was a scheduled domestic passenger flight from Tokyo to Osaka, Japan. On August 12, 1985, the Boeing 747 flying the route suffered a severe structural failure and decompression 12 minutes into the flight. After flying under minimal control for a further 32 minutes, the 747 crashed in the area of Mount Takamagahara, 100 kilometres from Tokyo.
Gameel Al-Batouti was a pilot for EgyptAir and a former officer for the Egyptian Air Force. On 31 October 1999, he and 216 passengers and crew on board EgyptAir Flight 990 were killed when he possibly crashed into the Atlantic Ocean about 60 miles (100 km) southeast of Nantucket Island, Massachusetts. The US National Transportation Safety Board (NTSB) concluded and stated that the crash was caused by a series of deliberate flight control inputs to the aircraft made by Al-Batouti, while being alone in the cockpit and in the position of relief first officer. The NTSB went on to state that the reason for his inputs were "not determined".
Air Canada Flight 797 was an international passenger flight operating from Dallas/Fort Worth International Airport to Montréal–Dorval International Airport, with an intermediate stop at Toronto Pearson International Airport.
Cabin pressurization is a process in which conditioned air is pumped into the cabin of an aircraft or spacecraft in order to create a safe and comfortable environment for humans flying at high altitudes. For aircraft, this air is usually bled off from the gas turbine engines at the compressor stage, and for spacecraft, it is carried in high-pressure, often cryogenic, tanks. The air is cooled, humidified, and mixed with recirculated air by one or more environmental control systems before it is distributed to the cabin.
Helios Airways Flight 522 was a scheduled passenger flight from Larnaca, Cyprus, to Prague, Czech Republic, with a stopover in Athens, Greece. Shortly after take-off on 14 August 2005, Nicosia air traffic control (ATC) lost contact with the pilots operating the flight, named Olympia; it eventually crashed near Grammatiko, Greece, killing all 121 passengers and crew on board. It is the deadliest aviation accident in Greek history.
On October 25, 1999, a chartered Learjet 35 business jet was scheduled to fly from Orlando, Florida, United States to Dallas, Texas, United States. Early in the flight, the aircraft, which was climbing to its assigned altitude on autopilot, lost cabin pressure, and all six on board were incapacitated by hypoxia, a lack of oxygen in the brain and body. The aircraft continued climbing past its assigned altitude, then failed to make the westward turn toward Dallas over North Florida and continued on its northwestern course, flying over the southern and midwestern United States for almost four hours and 1,500 miles (2,400 km). The plane ran out of fuel over South Dakota and crashed into a field near Aberdeen after an uncontrolled descent, killing all six on board.
Aircraft emergency oxygen systems or air masks are emergency equipment fitted to pressurized commercial aircraft, intended for use when the cabin pressurisation system has failed and the cabin altitude has climbed above a safe level. It consists of a number of individual yellow oxygen masks stored in compartments near passenger seats and near areas like lavatories and galleys, and an oxygen source, like a centralized gaseous cylinder or decentralized chemical oxygen generator.
British Airways Flight 38 was a scheduled international passenger flight from Beijing Capital International Airport in Beijing, China, to London Heathrow Airport in London, United Kingdom, an 8,100-kilometre trip. On 17 January 2008, the Boeing 777-200ER aircraft operating the flight crashed just short of the runway while landing at Heathrow. No fatalities occurred; of the 152 people on board, 47 sustained injuries, one serious. It was the first time in the aircraft type's history that a Boeing 777 was declared a hull loss, and subsequently written off.
EgyptAir Flight MS990 (MSR990) was a scheduled flight from Los Angeles International Airport to Cairo International Airport, with a stop at John F. Kennedy International Airport, New York City. On October 31, 1999, the Boeing 767-300ER operating the route crashed into the Atlantic Ocean about 60 miles (100 km) south of Nantucket Island, Massachusetts, killing all 217 passengers and crew on board, making it the deadliest aviation disaster for EgyptAir, and also the second-deadliest aviation accident involving a Boeing 767 aircraft, behind Lauda Air Flight 004.
Trans World Airlines Flight 800 was an international scheduled passenger service from Kansas City, Missouri to Cairo, Egypt via Chicago, New York City, Paris, Milan, Rome, and Athens. The Boeing 707 caught fire following a rejected take off on runway 25 at Leonardo da Vinci–Fiumicino Airport, Rome at 13:09 GMT on a flight to Athens International Airport, Greece on November 23, 1964, killing 49 of the 74 people on board.
On September 3, 2010, UPS Airlines Flight 6, the Boeing 747-400F flying the route between Dubai, United Arab Emirates, and Cologne, Germany, developed an in-flight fire, which caused the aircraft to crash, killing both crew members, the only people on board. It was the first fatal air crash for UPS Airlines. The accident prompted a re-evaluation of safety procedures protecting airliners from cockpit smoke.
Aeroflot Flight 2306 was a scheduled domestic passenger flight from Vorkuta to Moscow in the Soviet Union, with a stopover in Syktyvkar. The Tupolev Tu-134 operated by Aeroflot crashed on 2 July 1986 during an emergency landing after it departed Syktyvkar, killing 54 of 92 passengers and crew on board.
British Airways Flight 2276 was a scheduled international passenger service from Las Vegas to London. On 8 September 2015, the Boeing 777-200ER operating the flight suffered an uncontained engine failure and fire in the left (#1) GE90 engine during take-off from Las Vegas-McCarran International Airport, prompting an aborted take-off and the evacuation of all passengers and crew. All 170 people on board survived, but 20 were injured.
EgyptAir Flight 804 was a regularly scheduled international passenger flight from Paris Charles de Gaulle Airport to Cairo International Airport, operated by EgyptAir. On 19 May 2016 at 02:33 Egypt Standard Time (UTC+2), the Airbus A320 crashed into the Mediterranean Sea, killing all 66 occupants on board.
On February 13, 2018, around noon local time, a Boeing 777-222 airplane, operating as United Airlines Flight 1175 (UA1175), experienced an in-flight separation of a fan blade in the No. 2 (right) engine while over the Pacific Ocean en route from San Francisco International Airport to the Daniel K. Inouye International Airport, Honolulu, Hawaii. During level cruise flight shortly before beginning a descent from flight level 360, and about 120 miles from HNL, the flight crew heard a loud bang, followed by a violent shaking of the airplane, followed by warnings of a compressor stall. The flight crew shut down the failed engine, declared an emergency, and began a drift-down descent, proceeding direct to HNL where they made a single-engine landing without further incident at 12:37 local time. There were no reported injuries to the 378 passengers and crew on board and the airplane damage was classified as minor under National Transportation Safety Board (NTSB) criteria.
On February 20, 2021, United Airlines Flight 328 (UA328/UAL328), a scheduled U.S. domestic passenger flight from Denver to Honolulu, suffered what was technically ruled a contained engine failure despite shedding large pieces of debris, approximately four minutes after takeoff from Denver International Airport (DEN). Parts departing from the engine cowling of the Boeing 777-222 aircraft resulted in a debris field at least 1 mile (1.6 km) long over suburban residential areas of Broomfield, Colorado. Falling debris was recorded by eyewitnesses using smartphone cameras and a dash cam. Debris fell through the roof of a private home and significantly damaged a parked vehicle.
