West Wind Aviation Flight 280 was a domestic passenger flight from Fond-du-Lac Airport to Stony Rapids Airport, Canada. The aircraft was an ATR 42-320 registered C-GWEA. On 13 December 2017, shortly after taking off from Fond-du-Lac, the ATR-42 lost altitude and hit the ground. All 25 passengers and crew initially survived the crash, but one passenger later died of his injuries in hospital. Investigation on the cause of the crash is still ongoing.
The aircraft was an ATR 42-320 registered C-GWEA and was equipped with two PW121 turboprop engines produced by Pratt & Whitney Canada. [1] The aircraft entered service in 1991 with Aviación del Noroeste, and was later transferred to a few operators, including Zambia Airways, United Nations, and Fly540. The aircraft joined the fleet of West Wind Aviation in 2012, and was aged 26.8 years at the time of the crash. [2] This type of aircraft has previously been associated with accidents due to ice forming on the wing in freezing weather. The most notable accident was American Eagle Flight 4184. Experts say that changes in procedures and aircraft systems have solved the problem. [3]
The accident took place on December 13, 2017. According to the weather forecast, it was cloudy with temperature as low as −19 °C (−2 °F). [4] At 6:15 pm local time, the aircraft began its takeoff roll from Fond-du-Lac Airport. During the initial climb stage, the aircraft lost altitude and impacted the terrain 600 metres (2,000 ft) away from the runway. There was an 800 foot (240 m) trail of debris. The aircraft finally came to a stop in the upright position yet tilted to the right. The most serious damage was on the left side of the airframe, where it ruptured near seats Row 3. [5] There was no explosion nor fires at the crash site, but fuel leaks were found by nearby residents who rushed over for rescue work. [4]
Rescue work was immediately launched by local residents. Some of them followed the screams and rushed to the crash site near the airport, helping people to get out. [4] The passengers also struggled to save themselves. Four of them tried for half an hour and managed to open the emergency exit door. [6] Other passengers left the aircraft and guided local residents to the crash site. [7] People sent out alerts through Facebook, calling for more resources, and in 10–20 minutes more people arrived with blankets. Within a few hours, all aircraft occupants were rescued. Royal Canadian Mounted Police finally took over the crash site. [7]
There were 25 people on board, including 22 passengers, 2 pilots and 1 flight attendant. Nobody was killed initially, but six passengers and one crew member received serious injuries, at least five of whom were transported to hospital via air ambulance. The other 18 aircraft occupants received minor injuries. [4] [3] [8] A 19-year-old passenger died on 25 December 2017 as a result of his injuries. [9]
Transportation Safety Board of Canada launched the investigation. [4] BEA, ATR (aircraft manufacturer), Pratt & Whitney Canada (engine manufacturer), and Transport Canada also sent representatives to the site. Flight recorders were recovered and sent to the lab in Ottawa. [8] [10]
West Wind Aviation's air operator certificate was suspended on 22 December 2017 by Transport Canada, due to deficiencies in the company's operational control system. [11] They were allowed to fly again on May 8, 2018, after Transport Canada said West Wind had addressed the regulator's concerns about deficiencies in the company's operational control system. [12]
A year after the crash, the preliminary investigation made by the Canadian TSB suggested that icing might have largely contributed to the crash. The departure airport of Flight 280, Fond-du-Lac Airport, was not equipped with adequate de-icing equipment, with official commented it as "seriously inadequate". Polls conducted by TSB also revealed that at least 40% of pilots rarely or never have their aircraft de-iced in remote airports. Due to these findings, recommendation on better de-icing procedure throughout remote Canadian airports was issued to Transport Canada. Subsequently, investigators ruled out engine failure as likely cause of the crash. [13]
An aviation accident is defined by the Convention on International Civil Aviation Annex 13 as an occurrence associated with the operation of an aircraft, which takes place from the time any person boards the aircraft with the intention of flight until all such persons have disembarked, and in which a) a person is fatally or seriously injured, b) the aircraft sustains significant damage or structural failure, or c) the aircraft goes missing or becomes completely inaccessible. Annex 13 defines an aviation incident as an occurrence, other than an accident, associated with the operation of an aircraft that affects or could affect the safety of operation.
The ATR 42 is a regional airliner produced by ATR, with final assembly in Toulouse, France. On 4 November 1981, the aircraft was launched with ATR, as a joint venture between French Aérospatiale and Aeritalia . The ATR 42-300 performed its maiden flight on 16 August 1984 and type certification was granted during September 1985. Launch customer Air Littoral operated its first revenue-earning flight in December of that year.

Air France Flight 358 was a regularly scheduled international flight from Charles de Gaulle Airport in Paris, France, to Toronto Pearson International Airport in Ontario, Canada. On the afternoon of 2 August 2005, while landing at Pearson Airport, the Airbus A340-313E operating the route overran the runway and crashed into nearby Etobicoke Creek, approximately 300 m (980 ft) beyond the end of the runway. All 309 passengers and crew on board the Airbus survived, but twelve people sustained serious injuries. The accident highlighted the vital role played by highly trained flight attendants during an emergency.
Air Ontario Flight 1363 was a scheduled Air Ontario passenger flight which crashed near Dryden, Ontario, on 10 March 1989 shortly after takeoff from Dryden Regional Airport. The aircraft was a Fokker F28-1000 Fellowship twin jet. It crashed after only 49 seconds because it was not able to attain sufficient altitude to clear the trees beyond the end of the runway, due to ice and snow on the wings.

American Eagle Flight 4184 was a scheduled domestic passenger flight from Indianapolis, Indiana to Chicago, Illinois, United States. On October 31, 1994, the ATR 72 performing this route flew into severe icing conditions, lost control and crashed into a field. All 68 people aboard were killed in the high-speed impact.
Comair Flight 3272 was a Comair flight from Cincinnati to Detroit on Thursday, January 9, 1997. While on approach for landing, the Embraer EMB 120 Brasilia aircraft crashed nose-down 18 miles (29 km) southwest of Detroit Metropolitan Wayne County Airport at 15:54 EST. All 29 aboard, 26 passengers and three crew members, were killed.
Cougar Helicopters Flight 91 was a scheduled flight of a Cougar Sikorsky S-92A which ditched on 12 March 2009 en route to the SeaRose FPSO in the White Rose oil field and Hibernia Platform in the Hibernia oilfield off the coast of Newfoundland 55 kilometres (34 mi) east-southeast of St. John's, Newfoundland. Of the eighteen aboard, only one survived.

On 4 November 2010, the ATR 72 operating the route crashed in the central Cuban province of Sancti Spíritus, killing all 61 passengers and 7 crew members aboard.
First Air Flight 6560 was a domestic charter flight that crashed on landing at Resolute, Nunavut, Canada, on 20 August 2011. Of the 15 people on board, 12 were killed and the remaining three were severely injured. The Boeing 737-200 of First Air was operating a service from Yellowknife, Northwest Territories, when it struck a hill in cloud near Resolute Bay Airport.
Si Fly Flight 3275 (KSV3275) was a non-scheduled international passenger flight, operated by Italian airline Si Fly using an ATR 42-300 series from Rome Leonardo da Vinci–Fiumicino Airport to Pristina International Airport in Pristina, Kosovo. The flight had been chartered by the United Nations World Food Programme in response to the ongoing war in Kosovo. On 12 November 1999, Flight 3275 struck a mountain during an approach to Pristina Airport on Kosovo's capital. The crash killed all on board.
Lao Airlines Flight 301 was a scheduled domestic passenger flight from Vientiane to Pakse, Laos. On 16 October 2013, the ATR 72-600 aircraft operating the flight crashed into the Mekong River near Pakse, killing all 49 people on board. The accident was the first involving an ATR 72-600 and the deadliest ever to occur on Laotian soil.
Air Canada Flight 624 was a scheduled Canadian domestic passenger flight from Toronto Pearson International Airport to Halifax Stanfield International Airport in Halifax, Nova Scotia. During heavy snow and poor visibility, at 00:43 ADT on March 29, 2015, the Airbus A320-211 landed short of the runway and was severely damaged. Twenty-six people were injured.
On 29 March 2016, a Mitsubishi MU-2 operated by Aero Teknic, a Canadian aircraft maintenance company, crashed on approach to Îles-de-la-Madeleine Airport, killing former Canadian politician Jean Lapierre, several members of his family, and the two pilots. Lapierre was flying to his father's funeral with his wife and three siblings. While one man survived the crash, he died of a heart attack after being pulled from the wreckage.
Iran Aseman Airlines Flight 3704 was a scheduled Iranian domestic passenger flight from Iranian capital Tehran Mehrabad International Airport to Yasuj in southwest Iran. On 18 February 2018, during its approach to Yasuj, the aircraft serving the flight, an ATR 72-212 operated by Iran Aseman Airlines, crashed into Mount Dena in the Zagros Mountains near Noqol village in Semirom county, Isfahan Province. All 60 passengers and 6 crew members on board were killed.
