On 21 February 2017, at 8:59 am local time, a Beechcraft B200 King Air aircraft operating a charter flight, carrying a pilot and four passengers bound for King Island, crashed seconds after taking off from Essendon Airport in Melbourne, Australia. [2] [1]
Four American passengers and the Australian pilot died in the crash.
The pilot made two mayday calls [3] before the aircraft clipped the roof of a building housing the DFO Essendon. The aircraft then impacted the ground a few metres away and ignited, starting a fire. [4] [5] The DFO complex, located on the airport grounds, had not yet begun the day's trading. [6] All five people on board were killed in the crash, including the four passengers who were American tourists on their way to King Island to play golf. [5] Victorian Premier Daniel Andrews said it was the worst civil aviation accident in Victoria in 30 years. [7]
Media sources reported that the aircraft crashed as a result of an engine failure on take-off. [5]
Some local residents and aviation groups have stated that the crash shows buildings have been constructed too close to the airport. [8] [9] [10]
The official investigation's initial findings include that there were no pre-existing aircraft faults, that the pilot made a mayday call, that the voice recorder failed, and that witnesses said the take-off took "longer than normal". Engine tear-downs show that both powerplants were free to rotate and both propellers showed evidence of rotation, although only the left propeller left slash damage to the building roof. Further findings would be made public as the investigation was concluded. [11] [12]
The aircraft was a Beechcraft B200 King Air, serial number BB-1544 and registered as VH-ZCR; it was manufactured in 1996 [2] and had accumulated just under 7000 hours of service time. [1]
The Executive Director of the Australian branch of the Aircraft Owners and Pilots Association (AOPA) criticised the practice of allowing commercial developments, such as the Essendon DFO complex, at Australian airports, saying these reduced the options available to pilots in emergencies. He stated that AOPA had been calling on governments for two decades to stop such developments. [13] A spokesman for the Australian Federation of Air Pilots also criticised commercial development of land at Essendon Airport. [14] Anti-airport campaigners renewed calls for the airport to be closed. [14]
An investigation by the Australian Transport Safety Bureau determined the accident was a result of one of the flight control trim tabs being set incorrectly. Failure of the pilot to realise the rudder trim was set fully to the left caused a longer than normal takeoff, and caused the aircraft to slip and yaw to the left once airborne. Ultimately the uncorrected configuration caused a loss of control of the aircraft. The checklist used by the pilot did not contain a specific check for the trim tab position before takeoff. The investigation was hindered by the lack of data from the cockpit voice recorder, which had stopped working on a previous flight. [note 1] Furthermore, the weight of the aircraft at takeoff exceeded its maximum takeoff weight, but this factor was not determined to be a contributing factor to the crash. [1] [15]
In September 2021, it was reported that the pilot failed a proficiency test two years prior to the crash. [16]
Moorabbin Airport is a mostly general aviation airport for light aircraft located in between the southern Melbourne suburbs of Heatherton, Cheltenham, Dingley Village and Mentone. It also receives commercial airline service. The airport grounds are treated as their own suburb, and share the postcode 3194 with the neighboring suburb of Mentone. With a total of 274,082 aircraft movements, Moorabbin Airport was the second busiest airport in Australia for the calendar year 2011.
On August 16, 1987 a McDonnell Douglas MD-82, operating as Northwest Airlines Flight 255, crashed shortly after takeoff from Detroit Metropolitan Airport, about 8:46 pm EDT, resulting in the deaths of all six crew members and 148 of the 149 passengers, along with two people on the ground. The sole survivor was a 4-year-old girl who sustained serious injuries. It was the second-deadliest aviation accident at the time in the United States. It is also the deadliest aviation accident to have a sole survivor.
Essendon Fields Airport, colloquially known by its former name Essendon Airport, is a 305 ha public airport serving scheduled commercial, corporate-jet, charter and general aviation flights. It is located next to the intersection of the Tullamarine and Calder Freeways, in the north western suburb of Essendon Fields of Melbourne, Victoria, Australia. The airport is the closest to Melbourne's City Centre, approximately an 11 km (6.8 mi) drive north-west from it and 8 km (5.0 mi) south-east from Melbourne–Tullamarine Airport. In 1970, Tullamarine Airport replaced Essendon as Melbourne's main airport.

Advance Airlines Flight 4210 was a scheduled passenger flight which crashed at Sydney Airport on 21 February 1980, killing all 13 people on board the Advance Airlines Beech Beechcraft King Air 200. After taking off on runway 25 for a scheduled flight the aircraft's left (port) engine failed and the pilot requested an emergency landing on runway 34. The plane crashed into the sea wall while attempting the emergency landing. The accident caused the greatest number of fatalities in a civil aircraft crash in Australia since MacRobertson Miller Airlines Flight 1750, a Vickers Viscount that crashed near Port Hedland in Western Australia on 31 December 1968 killing all 26 on board.
Qantas Flight 30, on 25 July 2008, a Boeing 747-438 operated by Qantas, construction number 25067, registration VH-OJK, was a scheduled flight from London Heathrow to Melbourne with a stopover in Hong Kong. The flight was interrupted on the Hong Kong leg by an exploding oxygen tank that ruptured the fuselage just forward of the starboard wing root. 53-year-old Captain John Bartels and his co-pilots, Bernd Werninghaus and Paul Tabac, made an emergency descent to a breathable altitude of about 10,000 feet and diverted to Ninoy Aquino International Airport, Metro Manila, Philippines. There were no injuries.
On the night of September 19, 2008, a Learjet 60 business jet operating for Global Exec Aviation crashed during take-off from Columbia Metropolitan Airport in South Carolina.
On 4 September 2000, a chartered Beechcraft 200 Super King Air departed Perth for a flight to the mining town of Leonora, Western Australia. The aircraft crashed near Burketown, Queensland, Australia, resulting in the deaths of all eight occupants. During the flight, the aircraft climbed above its assigned altitude. When air traffic control (ATC) contacted the pilot, the pilot's speech had become significantly impaired, and he was unable to respond to instructions. Three aircraft intercepted the Beechcraft, but were unable to make radio contact. The aircraft continued flying on a straight north-easterly heading for five hours, before exhausting its fuel and crashing 40 mi (65 km) south-east of Burketown. The crash became known in the media as the "ghost flight".
Emirates Flight 407 was a scheduled international passenger flight operated by Emirates from Auckland to Dubai with a stopover in Melbourne, operated by an Airbus A340-500 aircraft. On 20 March 2009, the flight failed to take off properly at Melbourne Airport, hitting several structures at the end of the runway before climbing and then returning to the airport for a safe landing. Although no fatalities or injuries resulted, damage to the aircraft was severe enough for the event to be classified by Australian Transport Safety Bureau as an "accident". It was subsequently determined that a data-entry error resulted in insufficient engine thrust during take off. It has been described "as close as we have ever come to a major aviation catastrophe in Australia" by aviation officials.
The 2009 Pel-Air Westwind ditching or Norfolk Island ditching was an aircraft accident on 18 November 2009 near Norfolk Island, Australia. A Westwind II jet operated by Pel-Air was conducting an air ambulance flight for CareFlight International when it was forced to ditch after being unable to land in bad weather and not having sufficient fuel to divert to an alternate destination.
Qantas Flight 32 was a regularly scheduled passenger flight from London to Sydney via Singapore. On 4 November 2010, the aircraft operating the route, an Airbus A380, suffered an uncontained failure in one of its four Rolls-Royce Trent 900 engines. The failure occurred over the Riau Islands, Indonesia, four minutes after takeoff from Singapore Changi Airport. After holding for almost two hours to assess the situation, the aircraft made a successful emergency landing at Changi. No injuries occurred to the passengers, crew, or people on the ground, despite debris from the aircraft falling onto houses in Batam.
On 5 November 2010, JS Air Flight 201, a Beechcraft 1900 passenger aircraft on a charter service from Karachi to the Bhit Shah gas field in Sindh, Pakistan, crashed near Karachi's Jinnah International Airport, after suffering an engine malfunction at take-off. All 21 people on board were killed.
Angel Flight is the name used by a number of groups whose members provide free air transportation for passengers in need of medical treatment far from home and perform other missions of community service. Such a non-profit organization may be located in the United States, Europe, Australia, or Canada. Transportation is provided by volunteer pilots, often using their own general aviation aircraft. In most of Canada, the Volunteer Pilot Program of Hope Air provides a similar service, along with Angel Flight of Vancouver.
On 31 January 1945 a Stinson Model A aircraft departed from Melbourne for a flight of 127 nautical miles (235 km) to Kerang, Victoria—the first leg of an Australian National Airways regular scheduled service to Broken Hill, New South Wales. It crashed 50 nmi (93 km) from Melbourne. All ten occupants were killed in the accident. The aircraft was one of four Stinsons imported in 1936 by Airlines of Australia (AoA). Three had now crashed with the loss of 17 lives, and the fourth would not be permitted to fly again.
Caloundra Airport is a public general aviation airport located in Caloundra West serving the Sunshine Coast in the Australian state of Queensland. The airport is located on a 130 ha site, of which 74 ha is occupied by the current facilities. Further growth and expansion of the airport is limited by urban encroachment as much of the surrounding land has been developed into residential estates. The airport has been managed by Sunshine Coast Airports, a business unit of the Sunshine Coast Regional Council which is also responsible for the operations of the Sunshine Coast Airport at Maroochydore since 2008.
On 1 October 2012, Riama, a 1934 vintage de Havilland DH.84 Dragon passenger aircraft, crashed in Queensland, Australia, while flying from Monto to Caboolture. Radio contact was lost about an hour after the pilot reported to be in cloud with zero visibility. The wreckage was found in heavily wooded, hilly terrain two days later. The pilot and five passengers were killed in the accident.
Whyalla Airlines Flight 904 was a scheduled commuter flight, operated by a Piper PA-31 Navajo which crashed while attempting to ditch in the Spencer Gulf, South Australia after suffering failures of both engines on the evening of 31 May 2000. All 8 people on board the aircraft were killed as a result of the accident. The findings of a subsequent investigation by the Australian Transport Safety Bureau highlighting the airline's operating procedures as a key factor leading to the accident were later overturned after they were contradicted by evidence presented at a coronial inquiry into the deaths of those on board the flight. The safety implications arising from the accident were subject to a recall by engine manufacturer Textron Lycoming which saw close to 1000 aircraft grounded worldwide while defects were rectified at an estimated cost of $A66 million. Australia's aviation regulator, the Civil Aviation Safety Authority also mandated lifejackets be carried on all aircraft operating over water after the crash.
The 1994 South Pacific Airmotive DC-3 crash took place on 24 April 1994, when a Douglas DC-3 airliner operated by South Pacific Airmotive, tail number VH-EDC, ditched into Botany Bay shortly after takeoff from Sydney Airport in Sydney, New South Wales, Australia. The cause of the crash was determined by the Bureau of Air Safety Investigation to have been a power loss in the aircraft's left engine caused by an inlet valve being stuck in the open position, compounded by inadequate action on the part of the pilots; Rod Lovell, the pilot in command of the flight, has disputed BASI's conclusions.
