Air France Flight 066 was a scheduled international passenger flight from Paris Charles de Gaulle Airport to Los Angeles International Airport, operated by Air France and using an Airbus A380-861. On 30 September 2017, the aircraft suffered an uncontained engine failure and made an emergency landing at Goose Bay Airport, Canada. The outboard right-side Engine Alliance GP7200 engine failed and its fan hub and intake separated 150 kilometres (93 mi; 81 nmi) southeast of Paamiut, Greenland, while the aircraft was in cruise flight. A crack in the fan hub had been caused by metal fatigue.
This was the second of two uncontained engine failures suffered by the Airbus A380 type, following that of a Rolls-Royce Trent 900 engine on Qantas Flight 32 in 2010.
The aircraft diverted to CFB Goose Bay, a military air base also used for civilian flights, and landed at 15:42 UTC (12:42 local time) [1] [2] after suffering an uncontained failure on its number 4 (rightmost) engine while flying 150 kilometres (93 mi; 81 nmi) southeast of Paamiut, Greenland. [3] The engine had operated 3,527 cycles[ further explanation needed ] at the time of the incident. [4]
There were no reported injuries or fatalities among the 497 passengers and 24 crew on board. [1] [5] Passengers were not allowed to disembark from the A380 until another Air France aircraft and a chartered aircraft arrived the next morning (1 October), because the airport (located on a Royal Canadian Air Force base) is not equipped to accommodate a large number of international passengers from commercial aircraft. The Air France aircraft (a Boeing 777) landed at Atlanta, requiring a wait for its passengers to board another flight while the chartered Boeing 737 aircraft took passengers directly to Los Angeles with a refuelling stopover at Winnipeg. Pictures and video of the damaged engine were posted to social media by passengers; [7] [8] [9] and of the landing by an observer on the ground. [10]
The aircraft involved was a 7-year-old Airbus A380-861, [11] registration F-HPJE with the Manufacturer's Serial Number MSN 052, powered by four Engine Alliance GP7200 turbofan engines, having made its first flight on 10 August 2010, and was delivered to Air France on 17 May 2011. [12] At the time of the accident, the aircraft had accumulated a total of 27,184 flight hours. Eventually, the aircraft was repaired and went back to service on 15 January 2018, but was later retired in May 2020 due to the COVID-19 pandemic. As of June 2021, the aircraft has been returned to its lessor, the Dr. Peters Group, and is being stored at Tarbes–Lourdes–Pyrénées Airport. [13]
Air France issued a press release stating that an investigation was underway to determine the cause of the engine failure, including representatives of the Bureau of Enquiry and Analysis for Civil Aviation Safety (BEA, the French aviation accident investigation bureau), Airbus and Air France. [1] The Transportation Safety Board of Canada is responsible for investigating aviation accidents in Canada and planned to send investigators. [14] However, since the incident occurred over Greenland, the Danish Accident Investigation Board has jurisdiction over the investigation. [15]
On 3 October 2017 the Danish aviation authorities delegated the investigation to the BEA. Investigators from Denmark, the US and Canada joined the investigation. Advisors from Airbus, Air France and Engine Alliance (a partnership between General Electric and Pratt & Whitney) also flew to Goose Bay. The first observation was that the engine's fan hub had detached during the flight and dragged the air inlet with it. [16] Some six days later, debris from the aircraft's engine was recovered in Greenland. [17]
The BEA stated that "the recovery of the missing parts, especially of the fan hub fragments, was the key to supporting the investigation" and initiated a large search operation including synthetic-aperture radar overflights on a Dassault Falcon 20, but failed to locate the crucial components in 2018, before returning in 2019. [18] [19]
In July 2019, another missing piece of the engine, weighing 150 kilograms (330 lb), was located in Greenland and recovered. [20] The fan hub impact is believed to have made a small crater in the ice sheet that quickly filled with snow before initial search activities. The search and recovery for this missing fan hub took almost two years, with multiple search campaigns using different ice-penetrating radar and transient electromagnetic anomaly technologies. [21] It was ultimately recovered from 4 m depth in a large crevasse field. This search and recovery campaign was featured in the documentary "The Sky Detectives". [22]
The BEA released its final report in September 2020, indicating that the engine failed from a crack in the Ti-6-4 alloy fan hub caused by cold dwell fatigue cracking. [23]
On 12 October 2017, the American Federal Aviation Administration (FAA) issued an Emergency Airworthiness Directive (EAD) affecting all Engine Alliance GP7270, GP7272 and GP7277 engines. The EAD required a visual inspection of the fan hub within a timescale of two to eight weeks, depending on the number of cycles an engine had operated since new. [4] In June 2018 the FAA issued another Airworthiness Directive, requiring eddy-current testing of the fan hubs of GP7200 engines, to check for cracks in the slots in the hub that serve to attach the fan blades. [11]
In August 2019, the BEA announced that a part from the fan hub recovered from Greenland had been examined by the manufacturer Engine Alliance under BEA supervision. Metallurgical examination of the recovered titanium fan hub fragment identified a subsurface fatigue crack origin. The fracture was initiated in a microtextured area approximately in the middle of the slot bottom. Examination of the fracture was ongoing. Meanwhile, Engine Alliance informed affected A380 operators that an engine inspection campaign would be launched soon. [24]
Air France originally announced plans to ferry the aircraft back to Europe for repair, with an inoperable replacement engine installed, for reasons of weight and balance. [25] Such a flight requires special operating procedures, and thus rehearsal by the crew in a simulator. [25] That plan was revised and the aircraft was ferried back from Goose Bay Airport to Charles de Gaulle Airport [26] on 6 December 2017 using four operational engines and an Air France crew. [25] The damaged engine was flown to East Midlands Airport in the United Kingdom for examination by General Electric during the period 23–25 November 2017. [25] [27] The aircraft returned to service on 15 January 2018 [28] and continued to fly until Air France retired its A380 fleet in May 2020, due to the COVID-19 pandemic. F-HPJE's final flight was on 28 April from Charles de Gaulle Airport to Tarbes-Lourdes Airport as AF371V. [29] The aircraft is stored there along with two other Air France A380s and three former Singapore Airlines A380s. [30] In June 2021, the aircraft was re-registered as 9H-AIY by German-based investment firm Dr. Peters Group. [31]
Recovery of the fan hub from the Greenland ice sheet took place on 29–30 June 2019 after 20 months and four phases of complex aerial and ground search operations to locate the various elements from the engine. [32]

The Airbus A380 is a very large wide-body airliner, developed and produced by Airbus. It is the world's largest passenger airliner and the only full-length double-deck jet airliner. Airbus studies started in 1988, and the project was announced in 1990 to challenge the dominance of the Boeing 747 in the long-haul market. The then-designated A3XX project was presented in 1994; Airbus launched the €9.5–billion ($10.7–billion) A380 programme on 19 December 2000. The first prototype was unveiled in Toulouse on 18 January 2005, with its first flight on 27 April 2005. It then obtained its type certificate from the European Aviation Safety Agency (EASA) and the US Federal Aviation Administration (FAA) on 12 December 2006.
A wide-body aircraft, also known as a twin-aisle aircraft and in the largest cases as a jumbo jet, is an airliner with a fuselage wide enough to accommodate two passenger aisles with seven or more seats abreast. The typical fuselage diameter is 5 to 6 m. In the typical wide-body economy cabin, passengers are seated seven to ten abreast, allowing a total capacity of 200 to 850 passengers. Seven-abreast aircraft typically seat 160 to 260 passengers, eight-abreast 250 to 380, nine- and ten-abreast 350 to 480. The largest wide-body aircraft are over 6 m (20 ft) wide, and can accommodate up to eleven passengers abreast in high-density configurations.
The Pratt & Whitney PW4000 is a family of dual-spool, axial-flow, high-bypass turbofan aircraft engines produced by Pratt & Whitney as the successor to the JT9D. It was first run in April 1984, was FAA certified in July 1986, and was introduced in June 1987. With thrust ranging from 50,000 to 99,040 lbf, it is used on many wide-body aircraft.
Transaero, officially OJSC Transaero Airlines was a Russian airline that operated scheduled and charter flights to over 150 domestic and international destinations. The company is a 50/50 joint venture between GE Aerospace and Pratt & Whitney, a subsidiary of RTX. Engine Alliance was established in August 1996 to develop, manufacture, sell, and support a family of aircraft engines for new high-capacity, long-range aircraft.
The Engine Alliance GP7000 is a turbofan jet engine manufactured by Engine Alliance, a joint venture between General Electric and Pratt & Whitney. It is one of the powerplant options available for the Airbus A380, along with the Rolls-Royce Trent 900.
The Rolls-Royce Trent 900 is a high-bypass turbofan produced by Rolls-Royce plc to power the Airbus A380, competing with the Engine Alliance GP7000. Initially proposed for the Boeing 747-500/600X in July 1996, this first application was later abandoned but it was offered for the A3XX, launched as the A380 in December 2000. It first ran on 18 March 2003, made its maiden flight on 17 May 2004 on an A340 testbed, and was certified by the EASA on 29 October 2004. Producing up to 374 kN (84,000 lbf), the Trent 900 has the three shaft architecture of the Rolls-Royce Trent family with a 2.95 m (116 in) fan. It has a 8.5–8.7:1 bypass ratio and a 37–39:1 overall pressure ratio.

A turbine engine failure occurs when a gas turbine engine unexpectedly stops producing power due to a malfunction other than fuel exhaustion. It often applies for aircraft, but other turbine engines can also fail, such as ground-based turbines used in power plants or combined diesel and gas vessels and vehicles.
Air France has been in operation since 1933. Its aircraft have been involved in a number of major accidents and incidents. The deadliest accident of the airline occurred on June 1, 2009, when Air France Flight 447, an Airbus A330-203, flying from Rio de Janeiro to Paris crashed into the Atlantic Ocean with 228 fatalities. Qantas Flight 32 was a regularly scheduled passenger flight from London to Sydney via Singapore. On 4 November 2010, the aircraft operating the route, an Airbus A380, suffered an uncontained failure in one of its four Rolls-Royce Trent 900 engines. The failure occurred over the Riau Islands, Indonesia, four minutes after takeoff from Singapore Changi Airport. After holding for almost two hours to assess the situation, the aircraft made a successful emergency landing at Changi. No injuries occurred to the passengers, crew, or people on the ground, despite debris from the aircraft falling onto houses in Batam.
Richard Champion de Crespigny is an Australian Qantas pilot and author who served as pilot-in-command of Qantas Flight 32 and was widely praised for his cockpit resource management during the emergency with his crew.
A four-engined jet, sometimes called a quadjet, is a jet aircraft powered by four engines. The presence of four engines offers increased power and redundancy, allowing such aircraft to be used as airliners, freighters, and military aircraft. Many of the first purpose-built jet airliners had four engines, among which stands the De Havilland Comet, the world's first commercial jetliner. In the decades following their introduction, their use has gradually declined due to a variety of factors, including the approval of twin-engine jets to fly farther from diversion airports as reliability increased, and an increased emphasis on fuel efficiency.
Air Canada Flight 624 was a scheduled Canadian domestic passenger flight from Toronto Pearson International Airport to Halifax Stanfield International Airport in Halifax, Nova Scotia. During heavy snow and poor visibility, at 00:43 ADT on 29 March 2015, the Airbus A320-211 landed short of the runway and was severely damaged. Twenty-six people were injured.
EgyptAir Flight 804 was a regularly scheduled international passenger flight from Paris Charles de Gaulle Airport to Cairo International Airport, operated by EgyptAir. On 19 May 2016 at 02:33 Egypt Standard Time (UTC+2), the Airbus A320 crashed into the Mediterranean Sea, killing all 56 passengers, 3 security personnel, and 7 crew members on board.
Southwest Airlines Flight 1380 was a Boeing 737-700 that experienced a contained engine failure in the left CFM International CFM56 engine after departing from New York–LaGuardia Airport en route to Dallas Love Field on April 17, 2018. The engine cowl was broken in the failure, and cowl fragments damaged the fuselage, shattering a cabin window and causing explosive depressurization of the aircraft. Other fragments caused damage to the wing. The crew carried out an emergency descent and diverted to Philadelphia International Airport. One passenger was partially ejected from the aircraft and sustained fatal injuries, while eight other passengers sustained minor injuries. The aircraft was substantially damaged.

On 13 November 2020, Volga-Dnepr Airlines Flight 4066 suffered an uncontained engine failure on take-off from Tolmachevo Airport, Novosibirsk, Russia for Vienna International Airport, Vienna, Austria. The aircraft was severely damaged, and was further damaged when it overran the runway on landing at Tolmachevo airport. The investigation into the accident is currently ongoing.
LATAM Airlines Perú Flight 2213 was a scheduled domestic passenger flight in Peru from Lima to Juliaca. On 18 November 2022, the Airbus A320neo was taking off from Jorge Chávez International Airport when it collided with a fire engine that was crossing the runway, killing two firefighters and injuring a third, who died of his injuries seven months later. Forty passengers were injured. The aircraft was damaged beyond repair and written off, making it the first hull loss of the A320neo family.
