In aeronautics, loss of control (LOC) is the unintended departure of an aircraft from controlled flight and is a significant factor in several aviation accidents worldwide. In 2015 [1] it was the leading cause of general aviation accidents. [2] Loss of control may be the result of mechanical failure, external disturbances, aircraft upset conditions, or inappropriate crew actions or responses. [2]

Causes

Loss of control causes aircraft to depart from normal flight and possibly reach altitudes or encounter situations from which it can be difficult or impossible to recover, such as a stall or a spin. [1] Due to the certification and design processes, it is extremely rare for aircraft to experience a loss of control without extreme mishandling or a technical defect. [ citation needed ] [3]

A NASA study focused on identifying causal factors and addressing mitigation challenges related to aircraft loss of control had developed a preliminary list of contributing factors through a process that involved interviews, reviews of accident reports, and team analysis of available data. Causal factors in the list were segregated into three categories: (1) pilot- or human-induced; (2) environmentally-induced; and (3) systems-induced. The following list does not order the causal factors by frequency of occurrence or importance. [4]

Aerosucre S.A. is a cargo airline based in Bogotá, Colombia. It began operation in 1969 and operates scheduled international and domestic cargo services throughout Latin America and the Caribbean. Its home base is El Dorado International Airport, Bogotá. Aerosucre has been involved in a number of accidents and incidents during its lifetime, and more recently, internet videos have emerged showcasing reckless behavior by its pilots.

Pilot error generally refers to an accident in which an action or decision made by the pilot was the cause or a contributing factor that led to the accident, but also includes the pilot's failure to make a correct decision or take proper action. Errors are intentional actions that fail to achieve their intended outcomes. The Chicago Convention defines the term "accident" as "an occurrence associated with the operation of an aircraft [...] in which [...] a person is fatally or seriously injured [...] except when the injuries are [...] inflicted by other persons." Hence the definition of "pilot error" does not include deliberate crashing.

<span class="mw-page-title-main">Copa Airlines Flight 201</span> 1992 aviation accident in Panama

Copa Airlines Flight 201 was a regularly scheduled passenger flight from Tocumen International Airport in Panama City, Panama, to Alfonso Bonilla Aragón International Airport in Cali, Colombia. On 6 June 1992, the Boeing 737-204 Advanced operating the route rolled, entered a steep dive, disintegrated in mid-air, and crashed into the jungle of the Darién Gap 29 minutes after takeoff, killing all 47 people on board. The in-flight break-up was caused by faulty instrument readings and several other contributing factors, including incomplete training.

ADC Airlines Flight 053 (ADK053) was a scheduled passenger flight operated by ADC Airlines from Nigeria's capital of Abuja to Sokoto. On 29 October 2006, the Boeing 737-2B7 crashed onto a corn field shortly after take-off from Nnamdi Azikiwe International Airport in Abuja, killing 96 out of 105 people on board.

<span class="mw-page-title-main">Boeing 747 hull losses</span>

As of July 2020, a total of 60 Boeing 747 aircraft, or just under 4% of the total number of 747s built, first flown commercially in 1970, have been involved in accidents and incidents resulting in a hull loss, meaning that the aircraft was either destroyed or damaged beyond economical repair. Of the 60 Boeing 747 aircraft losses, 32 resulted in no loss of life; in one, a hostage was murdered; and in one, a terrorist died. Some of the aircraft that were declared damaged beyond economical repair were older 747s that sustained relatively minor damage. Had these planes been newer, repairing them might have been economically viable, although with the 747's increasing obsolescence, this is becoming less common. Some 747s have been involved in accidents resulting in the highest death toll of any civil aviation accident, the highest death toll of any single airplane accident, and the highest death toll of a midair collision. As with most airliner accidents, the root of cause(s) in these incidents involved a confluence of multiple factors that rarely could be ascribed to flaws with the 747's design or its flying characteristics.

Air France has been in operation since 1933. Its aircraft have been involved in a number of major accidents and incidents. The deadliest accident of the airline occurred on June 1, 2009, when Air France Flight 447, an Airbus A330-203, flying from Rio de Janeiro to Paris crashed into the Atlantic Ocean with 228 fatalities. The 1983 TAME Boeing 737 crash occurred when A Boeing 737-200, operated by Ecuador's national airline TAME, flying on a domestic route from the now-closed Mariscal Sucre International Airport in Quito to Mariscal Lamar Airport in Cuenca, crashed into a hill during final approach just 1 mile from its final destination, killing all 119 people on board.

Tatarstan Airlines Flight 363 was a scheduled domestic passenger flight, operated by Tatarstan Airlines on behalf of Ak Bars Aero, from Moscow to Kazan, Russia. On 17 November 2013, at 19:24 local time (UTC+4), the Boeing 737-500 crashed during an aborted landing at Kazan International Airport, killing all 44 passengers and 6 crew members on board, making it 2013's worst plane crash.
