On September 23, 1985, Henson Airlines Flight 1517 crashed in Grottoes, Virginia, while on approach to the Shenandoah Valley Regional Airport. The two pilots and twelve passengers were killed.
Henson Airlines Flight 1517 was a regular scheduled flight from Baltimore-Washington International Airport to Shenandoah Valley Regional Airport. The flight was operated on behalf of Piedmont Airlines as Piedmont Commuter.
At 9:59 am, Flight 1517 departed Baltimore bound for Shenandoah Valley. On board were twelve passengers and two crew members. The crew were Captain Martin Burns III (27), and First Officer Zilda Spadaro-Wolan (26).
The flight was routine until approach into Shenandoah Valley. At 10:14 am, Captain Burns told the Air Traffic Controller that he believed that the plane was too far west of his course. The controller tried several times to reach Flight 1517 by radio to no avail.
The search for Flight 1517 started at 10:25 am. At 6:42 pm, Civil Air Patrol located the crash site 6 miles east from the airport. Flight 1517 struck the southwest face of Hall Mountain at an elevation of 2,400 ft. [1]
Zilda Spadaro-Wolan was the first female commercial airline pilot to perish in an airplane crash in the United States.
The US National Transportation Safety Board determined that the probable cause of this accident was a navigational error by the flight crew resulting from their use of the incorrect navigational facility and their failure to adequately monitor the flight instruments. Factors which contributed to the flightcrew's errors were: (1) the nonstandardized navigational radio systems installed in the airline's Beech 99 fleet; (2) intracockpit communication difficulties associated with high ambient noise levels in the airplane; (3) inadequate training of the pilots by the airline; (4) the first officer's limited multiengine and instrument flying experience; (5) the pilots' limited experience in their positions in the Beech 99; and (6) stress-inducing events in the lives of the pilots. Also contributing to the accident was the inadequate surveillance of the airline by the Federal Aviation Administration which failed to detect the deficiencies which led to the accident. [2]
The passengers included playwright and actor Larry Shue and several executives of Beiersdorf, a personal care products company. [3]
Northwest Airlink was the brand name of Northwest Airlines' regional airline service, which flew turboprop and regional jet aircraft from Northwest's domestic hubs in Minneapolis, Detroit, and Memphis. Service was primarily to small-to-medium-sized cities and towns where larger aircraft might not be economical to operate and also to larger markets to either provide additional capacity or more frequent flights than could be justified using mainline aircraft. Beginning in July 2009, the Northwest Airlink trade name was phased out, and replaced by the Delta Connection trade name for Delta Air Lines as part of the Delta/Northwest merger.

Piedmont Airlines, Inc. is an American regional airline headquartered at the Salisbury Regional Airport in Wicomico County, Maryland, near the city of Salisbury. The airline is a wholly-owned subsidiary of the American Airlines Group and it is paid by fellow group member American Airlines to staff, operate and maintain aircraft used on American Eagle flights that are scheduled, marketed and sold by American Airlines. Piedmont also provides ground handling and customer service for airports in the northeastern and western United States.
On the evening of February 1, 1991, USAir Flight 1493, a Boeing 737-300, collided with SkyWest Airlines Flight 5569, a Fairchild Swearingen Metroliner turboprop aircraft, upon landing at Los Angeles International Airport (LAX). Though air traffic was not heavy at LAX, as Flight 1493 was on final approach, the local controller was distracted by a series of abnormalities, including a misplaced flight progress strip and an aircraft that had inadvertently switched off the tower frequency. The SkyWest flight was told to taxi into takeoff position, while the USAir flight was landing on the same runway.
USAir Flight 405 was a regularly scheduled domestic passenger flight between LaGuardia Airport in Queens, New York City, New York, and Cleveland, Ohio. On March 22, 1992, a USAir Fokker F28, registration N485US, flying the route, crashed in poor weather in a partially inverted position in Flushing Bay, shortly after liftoff from LaGuardia. The undercarriage lifted off from the runway, but the airplane failed to gain lift, flying only several meters above the ground. The aircraft then veered off the runway and hit several obstructions before coming to rest in Flushing Bay, just beyond the end of the runway. Of the 51 people on board, 27 were killed, including the captain and a member of the cabin crew.

It is also used as a freight aircraft and corporate transport, and by several governmental and military organizations. With customers favoring larger regional jets, Raytheon ended production in October 2002.
Continental Airlines Flight 1713 was a commercial airline flight that crashed while taking off in a snowstorm from Stapleton International Airport in Denver, Colorado, on November 15, 1987. The Douglas DC-9 airliner, operated by Continental Airlines, was making a scheduled flight to Boise, Idaho. Twenty-five passengers and three crew members died in the crash.
Southwest Airlines Flight 1248 was a scheduled passenger flight from Baltimore, Maryland, to Chicago, Illinois, continuing on to Salt Lake City, Utah, and then to Las Vegas, Nevada. On December 8, 2005, the airplane slid off a runway at Chicago-Midway while landing in a snowstorm and crashed into automobile traffic, killing a six-year-old boy.
The Beechcraft Model 99 is a civilian aircraft produced by Beechcraft. It is also known as the Beech 99 Airliner and the Commuter 99. The 99 is a twin-engine, unpressurized, 15 to 17 passenger seat turboprop aircraft, derived from the earlier Beechcraft King Air and Queen Air. It uses the wings of the Queen Air, the engines and nacelles of the King Air, and sub-systems from both, with a specifically designed nose structure.
Piedmont Airlines Flight 22 was a Piedmont Airlines Boeing 727-22 that collided with a twin-engine Cessna 310 on July 19, 1967, over Hendersonville, North Carolina, United States. Both aircraft were destroyed and all passengers and crew were killed, including John T. McNaughton, an advisor to U.S. Secretary of Defense Robert McNamara. The aircraft were both operating under instrument flight rules and were in radio contact with the Asheville control tower, though on different frequencies. The accident investigation was the first of a major scale conducted by the newly created National Transportation Safety Board. A review of the investigation conducted 39 years after the crash upheld the original findings that had placed primary responsibility on the Cessna pilot.

Comair Flight 5191 was a scheduled United States domestic passenger flight from Lexington, Kentucky, to Atlanta, Georgia. On the morning of August 27, 2006, at around 06:07 EDT, the Bombardier Canadair Regional Jet 100ER crashed while attempting to take off from Blue Grass Airport in Fayette County, Kentucky, 4 miles (6.4 km) west of the central business district of the city of Lexington.
Continental Airlines Flight 1404 was a Continental Airlines flight from Denver International Airport in Denver, Colorado, to George Bush Intercontinental Airport in Houston, Texas. On the evening of December 20, 2008, the flight crashed while taking off from Denver, resulting in two critical injuries, 36 noncritical injuries, and a hull loss of the Boeing 737-524 aircraft.
The 1990 Wayne County Airport runway collision involved the collision of two Northwest Airlines jetliners at Detroit Metropolitan Wayne County Airport on December 3, 1990. Flight 1482, a scheduled Douglas DC-9-14 operating from Detroit to Pittsburgh International Airport, taxied by mistake onto an active runway in dense fog and was hit by a departing Boeing 727 operating as Flight 299 to Memphis International Airport. One member of the crew and seven passengers of the DC-9 were killed.
On 22 December 2009, an American Airlines Boeing 737-800, operating American Airlines Flight 331 and carrying 148 passengers and six crew, overran runway 12 on landing at Kingston in poor weather. The plane continued on the ground outside the airport perimeter and broke apart on the beach, causing injuries.
On April 9, 1990, Atlantic Southeast Airlines Flight 2254, a scheduled passenger flight from Muscle Shoals, Alabama, to Gadsden, Alabama, to Atlanta, Georgia, was involved in a mid-air collision with a Cessna 172 over Gadsden. The collision resulted in the death of the pilot and passenger of the Cessna 172.
Northwest Airlink Flight 5719 was a flight from Minneapolis-Saint Paul International Airport to International Falls Airport in International Falls, Minnesota with a scheduled intermediate stop at Chisholm-Hibbing Airport in Hibbing, Minnesota. On December 1, 1993, the Jetstream 31, operated by Express Airlines I as Northwest Airlink, collided with a group of trees in a forest during final approach to Hibbing, and crashed into two ridges northwest of the airport, killing all sixteen passengers and the two pilots on board.
AVAir Flight 3378, was a scheduled flight under the American Eagle branding from Raleigh–Durham International Airport to Richmond International Airport which crashed after takeoff from Raleigh-Durham International Airport late on the night of February 19, 1988. All 12 people on board were killed in the accident.
Asiana Airlines Flight 214 was a scheduled transpacific passenger flight originating from Incheon International Airport near Seoul, South Korea. On the morning of July 6, 2013, the Boeing 777-200ER operating the flight stalled and crashed on final approach into San Francisco International Airport in the United States. Of the 307 people on board, three died; another 187 were injured, 49 of them seriously. Among the seriously injured were four flight attendants who were thrown onto the runway while still strapped in their seats when the tail section broke off after striking the seawall short of the runway. It was the first fatal crash of a Boeing 777 since the aircraft type entered service in 1995.
Trans-Colorado Airlines Flight 2286 was a scheduled domestic passenger flight from Denver, Colorado, to Durango, Colorado, operated for Continental Express by Trans-Colorado Airlines. On January 19, 1988, Flight 2286 crashed onto terrain near Bayfield, Colorado, while on approach to Durango-La Plata County Airport. Out of the seventeen people on board, nine were killed, including both crew members.
