Pan Am Flight 759 was a regularly scheduled domestic passenger flight from Miami to San Diego, with en route stops in New Orleans and Las Vegas. On July 9, 1982, the Boeing 727 flying this route crashed in the New Orleans suburb of Kenner after being forced down by a microburst shortly after takeoff. All 145 on board, as well as eight people on the ground, were killed. [1]
The aircraft involved, a 14-year-old Boeing 727-235, registration N4737, construction number 19457/518, was delivered to National Airlines on January 31, 1968. The aircraft was powered by three Pratt & Whitney JT8D-7B turbofan engines, [2] and was renamed from 37 Susan/Erica to Clipper Defiance after National was merged with Pan Am.
At the time of accident, the aircraft was carrying 137 passengers and one non-revenue passenger in the cockpit jumpseat, along with a crew of seven. The captain was 45-year-old Kenneth L. McCullers, who had 11,727 flying hours, including 10,595 hours on the Boeing 727. McCullers was described by others as an "above average" pilot, who was "comfortable" to fly with because of his excellent judgement and ability to exercise command. [1] : 78 The First Officer was 32-year-old Donald G. Pierce, who had 6,127 flying hours, including 3,914 hours on the Boeing 727. [1] : 78 Pierce was described by other captains as a conscientious pilot with excellent knowledge of aircraft systems and company flight procedures and techniques. The flight engineer was 60-year-old Leo B. Noone, who had 19,904 flying hours, including 10,508 hours on the Boeing 727. [1] : 78–79 All three flight crew, including the captain, the first officer and the second officer, were reported having no sleep or health problems, and had passed all proficiency checks without issues. The weather forecast issued at 07:40 on July 9 by the New Orleans National Meteorological Center contained thunderstorms, possible severe turbulence, icing, and wind shear. The weather chart at 18:00 local time identified a high pressure system located 60 nautical miles (69 mi; 110 km) off the Louisiana coast. No fronts or low pressure areas were within 100 nautical miles (120 mi; 190 km) of the airport. The forecast between 1200 and 2200 indicated "scattered clouds, variable to broken clouds at 3,000 feet (910 m), thunderstorms, and moderate rain showers." According to the NWS (National Weather Service), there were no severe weather warnings for the time and area of the accident. [1] : 1.7
Flight 759 began its takeoff from Runway 10 at the New Orleans International Airport (now Louis Armstrong New Orleans International), in Kenner, Louisiana at 16:07:57 central daylight time, bound for Las Vegas, Nevada. At the time of Flight 759's takeoff, there were thunderstorms over the east of the airport and east-northeast of the departure end of runway 10. The winds were reported to be "gusty and swirling." Flight 759 lifted off the runway, climbed to an altitude of between 95 and 150 feet (29 and 46 m), and then began to descend. About 2,376 feet (724 m) from the end of runway, the aircraft struck a line of trees at an altitude of about 50 feet (15 m). The aircraft continued descending for another 2,234 feet (681 m), hitting trees and houses. At 16:09:01, the aircraft crashed into the residential area of Kenner, about 4,610 feet (1,410 m) from the end of the runway. Flight 759 initially struck a house at 1624 Fairway Street, destroying it. The plane broke apart as it went across two and a half blocks, tearing diagonally across Hudson Street, 17th Street, and Taylor Street.[ citation needed ]
The aircraft was destroyed by the impact, explosion, and subsequent ground fire. [3] A total of 153 people were killed (all 145 passengers and crew on board and 8 on the ground). [1] : 1.1 Another four people on the ground sustained injuries. In one of the destroyed houses, a 16-month-old baby girl was discovered in a crib covered with debris that protected her from the flames, sustaining only minor burns. [4] The child's mother and 4-year-old sister were killed; the father was at work when the accident occurred. [5] In all, six houses were demolished, while five houses sustained significant damage. [6] [1] : 9 Most of the homes have since been rebuilt.
The National Transportation Safety Board (NTSB) determined that the probable cause of the accident was the aircraft's encounter with microburst-induced wind shear during the liftoff, which imposed a downdraft and a decreasing headwind, the effects of which the pilot would have had difficulty recognizing and reacting to in time for the aircraft's descent to be stopped before its impact with trees. [7] Contributing to the accident was the limited capability of then current wind shear detection technology. [1] The investigation noted the failure of the US Government to "put out proper weather information that day and to maintain wind shear detection devices at the airport." [8] The New York Times reported that:
According to witnesses, a wind shear alert was mentioned on New Orleans Airport radio frequencies on July 9, before Flight 759 took off. But the flight crew had been briefed with a recorded weather advisory that was two hours old, though airport routine is for hourly recordings of weather information. There were no procedures at the airport for advising flight crews that updated weather announcements were available. [9]
As a result, millions of dollars were paid out as compensation to various families affected by the crash. [10] [11] Flight 759, along with Delta Air Lines Flight 191 which crashed due to similar circumstances three years later, led to the development of the Airborne wind shear detection and alert system and the Federal Aviation Administration mandate to install windshear detection systems at airports and on board aircraft in the U.S. by 1993. Louis Armstrong New Orleans International Airport is an international airport under Class B airspace in Kenner city, Jefferson Parish, Louisiana, United States. It is owned by the city of New Orleans and is 11 miles (18 km) west of downtown New Orleans. A small portion of Runway 11/29 is in unincorporated St. Charles Parish. Armstrong International is the primary commercial airport for the New Orleans metropolitan area and southeast Louisiana.
Delta Air Lines Flight 191 was a regularly scheduled Delta Air Lines domestic service from Fort Lauderdale, Florida, to Los Angeles, California, with an intermediate stop at Dallas/Fort Worth International Airport (DFW). On August 2, 1985, the Lockheed L-1011 TriStar operating Flight 191 encountered a microburst while on approach to land at DFW. The aircraft impacted ground just over one mile (1.6 km) short of the runway, struck a car near the airport, collided with two water tanks and disintegrated. Out of the 163 occupants on board, 137 people died and 25 others were injured in the accident.
American Airlines Flight 1420 was a flight from Dallas/Fort Worth International Airport (DFW) to Little Rock National Airport in the United States. On June 1, 1999, the McDonnell Douglas MD-82 operating as Flight 1420 overran the runway upon landing in Little Rock and crashed. Nine of the 145 people aboard were immediately killed—the captain and eight passengers. Two more passengers died in the hospital in the following weeks.
In meteorology, a downburst is a strong downward and outward gushing wind system that emanates from a point source above and blows radially, that is, in straight lines in all directions from the area of impact at surface level. It originates under deep, moist convective conditions like cumulus congestus or cumulonimbus. Capable of producing damaging winds, it may sometimes be confused with a tornado, where high-velocity winds circle a central area, and air moves inward and upward. These usually last for seconds to minutes. Downbursts are particularly strong downdrafts within thunderstorms . Downbursts are most often created by an area of significantly precipitation-cooled air that, after reaching the surface (subsiding), spreads out in all directions producing strong winds.
Eastern Air Lines Flight 66 was a regularly scheduled flight from New Orleans to New York City that crashed on June 24, 1975, while on approach to New York's John F. Kennedy International Airport, killing 113 of the 124 people on board. The crash was determined to be caused by wind shear caused by a microburst, but the failure of the airport and the flight crew to recognize the severe weather hazard was also a contributing factor.
American Airlines Flight 625, a Boeing 727-100, crashed at St. Thomas, U.S. Virgin Islands on April 27, 1976, while on a domestic scheduled passenger flight originating at T. F. Green Airport in Rhode Island and ending at Saint Thomas, United States Virgin Islands, with an intermediate stop at John F. Kennedy International Airport. 37 out of the 88 passengers on board died in the accident.
USAir Flight 1016 was a regularly scheduled flight in the southeastern United States, between Columbia, South Carolina, and Charlotte, North Carolina. On July 2, 1994, the flight encountered heavy thunderstorms and microburst-induced windshear while attempting to land, and crashed into heavy trees and a private residence near the airport. The crash and ensuing fire caused 37 fatalities and seriously injured 20 others.
American Airlines Flight 383 was a nonstop flight from New York City to Cincinnati on November 8, 1965. The aircraft was a Boeing 727, with 57 passengers, and 5 crew on board. The aircraft crashed on final approach to the Cincinnati/Northern Kentucky International Airport located in Hebron, Kentucky, United States. Only three passengers and one flight attendant survived the accident.
ADC Airlines Flight 053 (ADK053) was a scheduled passenger flight operated by ADC Airlines from Nigeria's capital of Abuja to Sokoto. On 29 October 2006, the Boeing 737-2B7 crashed onto a corn field shortly after take-off from Nnamdi Azikiwe International Airport in Abuja, killing 96 out of 105 people on board.
A low-level windshear alert system (LLWAS) measures average surface wind speed and direction using a network of remote sensor stations, situated near runways and along approach or departure corridors at an airport. Wind shear is the generic term for wind differences over an operationally short distance which encompass meteorological phenomena including gust fronts, microbursts, vertical shear, and derechos.
On July 10, 1991, a L'Express Airlines Beechcraft C99, flying as Flight 508 originating in New Orleans, and in transit from Mobile to Birmingham, crashed while attempting to make an ILS approach to Runway 5 at Birmingham Municipal Airport in Birmingham, Alabama. The plane crashed in the Fairview area near Five Points West in the Ensley neighborhood and subsequently injured four persons on the ground, as well as destroying two homes. Of the 15 occupants on board, there were 13 fatalities. The cause of the crash was attributed to the captain's decision to attempt an instrument approach into severe thunderstorms resulting in a loss of control of the airplane. To date it is the deadliest commercial aviation accident in Alabama history.
The airborne wind shear detection and alert system, fitted in an aircraft, detects and alerts the pilot both visually and aurally of a wind shear condition. A reactive wind shear detection system is activated by the aircraft flying into an area with a wind shear condition of sufficient force to pose a hazard to the aircraft. A predictive wind shear detection system is activated by the presence of a wind shear condition ahead of the aircraft. In 1988, the U.S. Federal Aviation Administration (FAA) mandated that all turbine-powered commercial aircraft must have on-board wind shear detection systems by 1993. Airlines successfully lobbied to have commercial turbo-prop aircraft exempted from this requirement.
Pan Am Flight 806 was an international scheduled flight from Auckland, New Zealand, to Los Angeles, California, with intermediate stops at Pago Pago, American Samoa and Honolulu, Hawaii. On January 30, 1974, the Boeing 707 Clipper Radiant crashed on approach to Pago Pago International Airport, killing 87 passengers and ten crew members.
On December 3, 1990, two Northwest Airlines jetliners collided at Detroit Metropolitan Wayne County Airport. Flight 1482, a scheduled Douglas DC-9-14 operating from Detroit to Pittsburgh International Airport, taxied by mistake onto an active runway in dense fog and was hit by a departing Boeing 727 operating as Flight 299 to Memphis International Airport. One member of the crew and seven passengers of the DC-9 were killed.
Aeroméxico Connect Flight 2431 (SLI2431/5D2431) was a Mexican domestic scheduled passenger flight bound for Mexico City that crashed on takeoff from Durango International Airport on July 31, 2018. Shortly after becoming airborne, the plane encountered sudden wind shear caused by a microburst. The plane rapidly lost speed and altitude and impacted the runway, detaching the engines and skidding to a halt about 1,000 feet (300 m) beyond the runway. The plane caught fire and was destroyed. All 103 people on board survived, but 39 passengers and crew members were injured.
American Airlines Flight 102 was a regularly scheduled flight operated by a McDonnell Douglas DC-10 from Honolulu International Airport to Dallas/Fort Worth International Airport. On 14 April 1993, upon landing, it was raining at Dallas-Ft Worth International Airport, and there were numerous thunderstorms in the area. Shortly after touchdown on runway 17L, the pilot lost directional control as the aircraft began to weathervane, and it departed the right side of the runway. All 202 occupants on board survived, with two passengers suffering serious injuries during the emergency evacuation. The aircraft was badly damaged and was written off.
The Las Vegas-bound 727 crashed in the middle of a ferocious thunderstorm, less than a minute after it took off, killing all on board, including eight on the ground. The plane tore through the Morningside Park neighborhood in south Kenner, destroying 15 homes.
