Permanent teeth or adult teeth are the second set of teeth formed in diphyodont mammals. In humans and old world simians, there are thirty-two permanent teeth, consisting of six maxillary and six mandibular molars, four maxillary and four mandibular premolars, two maxillary and two mandibular canines, four maxillary and four mandibular incisors. The first permanent tooth usually appears in the mouth at around 5-6 years of age, and the mouth will then be in a transition time with both primary (or deciduous dentition) teeth and permanent teeth during the mixed dentition period until the last primary tooth is lost or shed. [2]

The first of the permanent teeth to erupt are the permanent first molars, right behind the last 'milk' molars of the primary dentition. These first permanent molars are important for the correct development of a permanent dentition. Up to thirteen years of age, 28 of the 32 permanent teeth will appear.

The full permanent dentition is completed much later during the permanent dentition period. [3] The four last permanent teeth, the third molars, usually appear between the ages of 17 and 21 years; they are considered wisdom teeth. [4]

Pathology

It is possible to have extra, or "supernumerary", teeth. This phenomenon is called hyperdontia and is often erroneously referred to as "a third set of teeth." These teeth may erupt into the mouth or remain impacted in the bone. Hyperdontia is often associated with syndromes such as cleft lip and cleft palate, tricho-rhino-phalangeal syndrome, cleidocranial dysplasia, and Gardner's syndrome. [5]

In mammalian oral anatomy, the canine teeth, also called cuspids, dog teeth, eye teeth, vampire teeth, or vampire fangs, are the relatively long, pointed teeth. In the context of the upper jaw, they are also known as fangs. They can appear more flattened however, causing them to resemble incisors and leading them to be called incisiform. They developed and are used primarily for firmly holding food in order to tear it apart, and occasionally as weapons. They are often the largest teeth in a mammal's mouth. Individuals of most species that develop them normally have four, two in the upper jaw and two in the lower, separated within each jaw by incisors; humans and dogs are examples. In most species, canines are the anterior-most teeth in the maxillary bone. The four canines in humans are the two upper maxillary canines and the two lower mandibular canines. The third molar, commonly called wisdom tooth, is the most posterior of the three molars in each quadrant of the human dentition. The age at which wisdom teeth come through (erupt) is variable, but this generally occurs between late teens and early twenties. Most adults have four wisdom teeth, one in each of the four quadrants, but it is possible to have none, fewer, or more, in which case the extras are called supernumerary teeth. Wisdom teeth may become stuck (impacted) against other teeth if there is not enough space for them to come through normally. Impacted wisdom teeth are still sometimes removed for orthodontic treatment, believing that they move the other teeth and cause crowding, though this is not held anymore as true.

<span class="mw-page-title-main">Hyperdontia</span> Condition of having extra teeth beyond the regular number of teeth

Hyperdontia is the condition of having supernumerary teeth, or teeth that appear in addition to the regular number of teeth. They can appear in any area of the dental arch and can affect any dental organ. The opposite of hyperdontia is hypodontia, where there is a congenital lack of teeth, which is a condition seen more commonly than hyperdontia. The scientific definition of hyperdontia is "any tooth or odontogenic structure that is formed from tooth germ in excess of usual number for any given region of the dental arch." The additional teeth, which may be few or many, can occur on any place in the dental arch. Their arrangement may be symmetrical or non-symmetrical.

Hypodontia is defined as the developmental absence of one or more teeth excluding the third molars. It is one of the most common dental anomalies, and can have a negative impact on function, and also appearance. It rarely occurs in primary teeth and the most commonly affected are the adult second premolars and the upper lateral incisors. It usually occurs as part of a syndrome that involves other abnormalities and requires multidisciplinary treatment.

Deciduous teeth or primary teeth, also informally known as baby teeth, milk teeth, or temporary teeth, are the first set of teeth in the growth and development of humans and other diphyodonts, which include most mammals but not elephants, kangaroos, or manatees, which are polyphyodonts. Deciduous teeth develop during the embryonic stage of development and erupt during infancy. They are usually lost and replaced by permanent teeth, but in the absence of their permanent replacements, they can remain functional for many years into adulthood.

Tooth development or odontogenesis is the complex process by which teeth form from embryonic cells, grow, and erupt into the mouth. For human teeth to have a healthy oral environment, all parts of the tooth must develop during appropriate stages of fetal development. Primary (baby) teeth start to form between the sixth and eighth week of prenatal development, and permanent teeth begin to form in the twentieth week. If teeth do not start to develop at or near these times, they will not develop at all, resulting in hypodontia or anodontia.

The maxillary central incisor is a human tooth in the front upper jaw, or maxilla, and is usually the most visible of all teeth in the mouth. It is located mesial to the maxillary lateral incisor. As with all incisors, their function is for shearing or cutting food during mastication (chewing). There is typically a single cusp on each tooth, called an incisal ridge or incisal edge. Formation of these teeth begins at 14 weeks in utero for the deciduous (baby) set and 3–4 months of age for the permanent set.

The maxillary lateral incisors are a pair of upper (maxillary) teeth that are located laterally from both maxillary central incisors of the mouth and medially from both maxillary canines. As with all incisors, their function is for shearing or cutting food during mastication, commonly known as chewing. There are generally no cusps on the teeth, but the rare condition known as talon cusps are most prevalent on the maxillary lateral incisors. The surface area of the tooth used in eating is called an incisal ridge or incisal edge. Though relatively the same, there are some minor differences between the deciduous (baby) maxillary lateral incisor and that of the permanent maxillary lateral incisor. The maxillary lateral incisors occlude in opposition to the mandibular lateral incisors.

<span class="mw-page-title-main">Mandibular first molar</span>

The mandibular first molar or six-year molar is the tooth located distally from both the mandibular second premolars of the mouth but mesial from both mandibular second molars. It is located on the mandibular (lower) arch of the mouth, and generally opposes the maxillary (upper) first molars and the maxillary 2nd premolar in normal class I occlusion. The function of this molar is similar to that of all molars in regard to grinding being the principal action during mastication, commonly known as chewing. There are usually five well-developed cusps on mandibular first molars: two on the buccal, two lingual, and one distal. The shape of the developmental and supplementary grooves, on the occlusal surface, are described as being M-shaped. There are great differences between the deciduous (baby) mandibular molars and those of the permanent mandibular molars, even though their function are similar. The permanent mandibular molars are not considered to have any teeth that precede it. Despite being named molars, the deciduous molars are followed by permanent premolars.

<span class="mw-page-title-main">Tooth eruption</span> Process in tooth development

Tooth eruption is a process in tooth development in which the teeth enter the mouth and become visible. It is currently believed that the periodontal ligament plays an important role in tooth eruption. The first human teeth to appear, the deciduous (primary) teeth, erupt into the mouth from around 6 months until 2 years of age, in a process known as "teething". These teeth are the only ones in the mouth until a person is about 6 years old creating the primary dentition stage. At that time, the first permanent tooth erupts and begins a time in which there is a combination of primary and permanent teeth, known as the mixed dentition stage, which lasts until the last primary tooth is lost. Then, the remaining permanent teeth erupt into the mouth during the permanent dentition stage.

Dental anatomy is a field of anatomy dedicated to the study of human tooth structures. The development, appearance, and classification of teeth fall within its purview. Tooth formation begins before birth, and the teeth's eventual morphology is dictated during this time. Dental anatomy is also a taxonomical science: it is concerned with the naming of teeth and the structures of which they are made, this information serving a practical purpose in dental treatment.

Serial extraction is the planned extraction of certain deciduous teeth and specific permanent teeth in an orderly sequence and predetermined pattern to guide the erupting permanent teeth into a more favorable position.

Pre-eruption guidance is an orthodontic treatment method that allows for expansion of existing erupting teeth long before they appear in the mouth. The use off pre-eruption guidance appliances and the timing of extractions of certain deciduous teeth aligns the teeth naturally as opposed to orthodontic mechanical movement of permanent teeth into alignment after they have erupted. Research shows that pre-eruption guidance produces far more stable tooth alignment than alternative treatments.

Orthodontic indices are one of the tools that are available for orthodontists to grade and assess malocclusion. Orthodontic indices can be useful for an epidemiologist to analyse prevalence and severity of malocclusion in any population.
