Philippine Air Lines Flight S85 was a domestic flight that crashed shortly after taking off from Cebu-Lahug Airport, Cebu.
On December 22, 1960, the aircraft's left engine malfunctioned shortly after takeoff, prompting an emergency landing, however before the aircraft could return to the runway, it stalled, banked left and crashed in a huge explosion about 2.5 miles from Lahug Airport, killing 26 passengers including both pilots. The aircraft was a Douglas C-47A manufactured in the United States in June 1944 and registered as 43–16127, before it was given to Philippine Air Lines and converted into a DC-3C 40-seater and re-registered as PI-C126 in 1948. [2]
It had 18,611 flight hours at the time of the crash, and there was no evidence that the aircraft was not airworthy before the flight.[ citation needed ]
The left engine had a total operating time of over 12,300 hours, and was last overhauled on June 7, 1960, while the right engine had a total operating time of over 5,000 hours, and was last overhauled on February 5, 1960. [3]
The crew of three consisted of a captain, a first officer and a steward.[ citation needed ]
The captain held an airline pilot's license and logged 8,564 hours. He had flown the S-85 flight route 16 times. The first officer held a commercial pilot's license and a flight instructor's license, and logged 1,822 hours. He had flown the S-85 flight route 35 times.[ citation needed ]
The steward, who survived the crash, stated after the crash that the first officer occupied the left seat and the captain occupying the right, something that wasn't authorised by the airline.[ citation needed ]
Flight S85 originated from Manila at 00:10 PHT and arrived at Cebu at 02:40. The captain who flew this segment of the trip reported nothing unusual with the aircraft's engine and instruments.[ citation needed ]
A visual inspection was conducted by the PAL maintenance inspector followed by a further inspection by the first officer upon arrival at Lahug Airport. It was checked and refuelled ready for the next segment to Davao.[ citation needed ]
No maintenance work was done at Cebu prior to takeoff. All scheduled inspections and maintenance were done in accordance with the airline's procedures. [3]
Following a crew changeover, Flight S85 took off from Cebu at 03:30 PHT, and was estimated to arrive at Davao around 2 hours later. The controller-on-duty at Lahug reported that the aircraft's take-off appeared to be normal. However, a radio call was received that the left engine had malfunctioned shortly after it left the runway. An emergency landing was granted and while the aircraft attempted to turn back to the airport, the plane failed to gain altitude, and so it stalled, banked left and crashed in a huge explosion 2.5 miles from the runway threshold, in Talamban.[ citation needed ]
Both pilots and 26 passengers died in the crash, though the steward and 8 other passengers survived.[ citation needed ]
Investigations determined that Flight S85 crashed due to the malfunction of the left engine which led to the crash, and contributing factors involved the darkness of the night which limited the visibility during the emergency and the inappropriate emergency procedures that were carried out. [4]
There was no material failure of the propellers to indicate a runaway propeller in its assembly. However it was noted that while all three blades on the right propeller were symmetrically bent, the left propellers appeared unsymmetrical, with one blade showing an oblique scratch at the tip. Based on the findings of the left propeller blades, it was suggested that the left engine had feathered itself. [3]
The Kegworth air disaster occurred when British Midland Airways Flight 092, a Boeing 737-400, crashed onto the motorway embankment between the M1 motorway and A453 road near Kegworth, Leicestershire, England, while attempting to make an emergency landing at East Midlands Airport on 8 January 1989.

Atlantic Southeast Airlines Flight 529 was an Embraer EMB 120 Brasilia aircraft that crashed in the community of Burwell between the cities Bowdon, Georgia and Carrollton, Georgia, on August 21, 1995. Nine of the 29 passengers and crew on board were killed as a result of the accident. The accident bore similarities to Atlantic Southeast Airlines Flight 2311, which had occurred four years earlier, and resulted in the deaths of all 23 people on board. The inquiries of both crashes concluded that design flaws in the aircraft's propellers were to blame.
Pakistan International Airlines Flight 688 was a domestic passenger flight from Multan to Islamabad with a stopover in Lahore, operated by Pakistan's flag carrier Pakistan International Airlines. On 10 July 2006, the aircraft operating the route, a Fokker F27, crashed into a mango garden after one of its two engines failed shortly after takeoff from Multan International Airport. All 41 passengers and four crewmembers on board were killed.
Mohawk Airlines Flight 405, a Fairchild Hiller FH-227 twin-engine turboprop airliner registered N7818M, was a domestic scheduled passenger flight operated by Mohawk Airlines that crashed into a house within the city limits of Albany, New York, on March 3, 1972, on final approach to Albany County Airport, New York, killing 17 people. The intended destination airport lies in the suburban Town of Colonie, about 4 miles north of the crash site.

A turbine engine failure occurs when a gas turbine engine unexpectedly stops producing power due to a malfunction other than fuel exhaustion. It often applies for aircraft, but other turbine engines can also fail, such as ground-based turbines used in power plants or combined diesel and gas vessels and vehicles.
On March 17, 1957, a Douglas C-47 Skytrain transport aircraft crashed on the slopes of Mount Manunggal on the island of Cebu, Philippines, killing 25 of the aircraft's 26 occupants, including the incumbent president of the Philippines, Ramon Magsaysay. Several high-ranking Philippine government officials, military officials, and journalists were also among the dead. The sole survivor was a reporter for the Philippine Herald, Nestor Mata.
Tuninter Flight 1153 was a Tuninter Airlines international flight from Bari International Airport in Bari, Italy, to Djerba-Zarzis Airport in Djerba, Tunisia. On 6 August 2005, the Tuninter ATR 72 ditched into the Mediterranean Sea about 18 miles (29 km) from the city of Palermo. Sixteen of the 39 people on board died. The accident resulted from fuel exhaustion due to the installation of fuel quantity indicators designed for the ATR 42 in the larger ATR 72. It was also Tuninter's first fatal accident in the 14-year history of the company.
On April 4, 1955, a United Airlines Douglas DC-6 named Mainliner Idaho crashed shortly after taking off from Long Island MacArthur Airport, in Ronkonkoma, Islip, New York, United States.
American Eagle Flight 5452, officially operating as Executive Air Charter Flight 5452, was a commuter flight between Luis Muñoz Marín International Airport in San Juan, Puerto Rico and Eugenio María de Hostos Airport in Mayagüez. The flight was operated by Executive Air Charter, doing business as American Eagle, and was operated by a CASA C-212 Aviocar aircraft. Visual meteorological conditions were present as the plane made its final approach to runway 9 at Mayagüez on May 8, 1987. The plane crashed 600 feet short of the runway, destroying the aircraft and killing both pilots, but leaving the four passengers with only minor injuries.
Sepahan Airlines Flight 5915 was a scheduled domestic passenger flight from Iranian capital Tehran Mehrabad International Airport to Tabas, South Khorasan province, Iran. On 10 August 2014, the HESA IrAn-140 twin turboprop serving the flight crashed shortly after takeoff from Mehrabad International Airport, falling into a boulevard near the Azadi Stadium. Of the 42 passengers and six crew on board, 40 people died.
On October 30, 2014, a Beechcraft King Air B200 twin turboprop crashed into a building hosting a FlightSafety International (FSI) training center shortly after taking off from Wichita Mid-Continent Airport in Wichita, Kansas. The pilot, the only person on board, was killed along with three people in the building; six more people in the building were injured.
Centurion Air Cargo Flight 164 was a chartered international cargo flight, flying from Bogota's El Dorado International Airport while en route to Miami International Airport. The flight was operated by Kalitta Air and the aircraft was wet leased by Centurion Air Cargo. On 7 July 2008, the aircraft, a Boeing 747-209BSF registered as N714CK, crashed shortly after takeoff. All aboard suffered injuries, but none were killed. Two people on the ground were killed after the plane slammed into a farm. The crash was the second crash of a Boeing 747 in 2008 in Kalitta Air service, after a previous accident at Brussels in May.
Pakistan International Airlines Flight 661 was a Pakistani domestic passenger flight from Chitral to Islamabad, operated by Pakistan's flag carrier Pakistan International Airlines. On 7 December 2016, the aircraft serving the route, an ATR 42-500 twin-turboprop, crashed near Havelian following an engine failure. All 47 people on board died, including singer-turned-preacher and entrepreneur Junaid Jamshed, and the Deputy Commissioner of the District of Chitral.
Khabarovsk Airlines Flight 463 was a scheduled domestic passenger flight from Nikolayevsk-on-Amur Airport to Nelkan Airport in Russia. On 15 November 2017 the Let L-410 Turbolet operating the flight crashed short of the runway at Nelkan Airport, killing all but one of the seven people on board. The sole survivor was a three-year-old girl who sustained serious injuries. The crash was caused by a malfunction of the right engine's propeller.
Korean Air Flight 631 (KE631/KAL631) was a scheduled passenger flight from Incheon International Airport near Seoul, South Korea to Mactan–Cebu International Airport in Metro Cebu, Philippines. On 23 October 2022, the Airbus A330-300 operating this flight overran the runway while landing in Cebu due to hydraulic failure. Despite what reports described as a "terrifying close call," all passengers and crew members survived without injuries. However, the aircraft was damaged beyond repair and written off as a result of the accident, making it the 14th hull loss of an Airbus A330.

Philippine Air Lines Flight 741 was a domestic flight operated by Philippine Air Lines that crashed shortly after takeoff from Manila International Airport, Manila.
