There were 55 competitors from 33 nations, with each nation having up to three swimmers. The medals were swept the United States, the only time there has been a medal sweep in the men's 200 metre freestyle. Bruce Furniss took gold, John Naber silver, and Jim Montgomery bronze. It was the second consecutive and third overall victory by an American swimmer.

The Cayman Islands sent a delegation to compete at the 2008 Summer Olympics in Beijing, China. The delegation included fifteen people; of the delegation, four athletes participated under the Caymanian flag. In the track and field events was Ronald Forbes, who also served as the Cayman Islands' flag-bearer during the Opening Ceremony and reached quarterfinals in the 110m hurdles, and Cydonie Mothersille, who reached finals and ranked eighth in the 200m dash. Brian Brinkley is an English former competitive swimmer who represented Great Britain in the Olympic Games, FINA world championships and European championships, and England in the Commonwealth Games, during the 1970s. During his elite swimming career, he won thirteen medals in major international swimming championships.

Dmytro Yuriyovych Vereitynov is a Ukrainian former swimmer, who specialized in freestyle events. He currently holds a Ukrainian record of 7:21.42 in the 4 × 200 m freestyle relay at the 2005 FINA World Championships in Montreal, Quebec, Canada. Vereitinov is also a member of SC Ukraina Kharkiv, and is trained by his father and lifelong coach Yuriy Vereitinov.

Blake John Pieroni is a retired American professional swimmer who specialized in freestyle events. He is a two time Olympian and gold medalist in the 4x100-meter freestyle relay at both the 2016 Summer Olympics and the 2020 Summer Olympics. On the relay in 2016, he swam in the prelims of the race, on the 2020 Olympics 4x100-meter freestyle relay he swam in both the prelims and the final. He also won a gold medal in the 4x100-meter medley relay at the 2020 Olympic Games, swimming the freestyle leg of the relay in the prelims.

Zachary "Zach" Douglas Apple is an American competitive swimmer who specializes in the sprint freestyle events. He currently swims for DC Trident in the International Swimming League. He won his first Olympic gold medal in the 4x100-meter freestyle relay at the 2020 Summer Olympics, swimming in the prelims and the final of the event, and later in the same Olympic Games won a gold medal and helped set a new world record and Olympic record in the 4x100-meter medley relay, swimming the freestyle leg of the relay in the final.
