On 9 September 2018, en route from Juba International Airport to Yirol Airport, a Let L-410 Turbolet aircraft crashed, carrying a total of 23 passengers and crew, of which 20 were killed on impact, including the Anglican Bishop of Yirol, Simon Adut Yuang. [1] [2] [3] The small plane crashed into Lake Yirol amid heavy fog and poor visibility. [1] [4]
The aircraft, registered UR-TWO, operated by Ukrainian carrier Slaver Kompani for South Sudan-based South West Aviation, had been delivered to Aeroflot in 1984, then transferred to various operators until 2006, when it was placed in storage in Rivne, Ukraine. In April 2018, the plane was acquired by Slaver Kompani and wet-leased since May. [5]
The Let 410 UVP had landed in Juba on the same morning as the accident, piloted by a different crew from the one that would then conduct the flight to Yirol. [6]
On 9 September 2018 at 08.00 local time (05.00 UTC), the Let 410 took off from Juba International Airport with destination Yirol Airport, at approximately 122 NM, for an estimated flight time of 45 minutes. The take-off weight was 5,700 kg, of which 3,800 kg was for the aircraft alone.[ citation needed ]
On-board the flight deck were two Sudanese crew members. Captain Sami Qeily (57), and First Officer Mohamed Shamseddine (27), were the pilot-in-command and second-in-command, respectively. While both of their flight time logs were destroyed in the crash, they were rated in flying the Let-410. [7]
Despite the Flight Data Recorder being recovered on 10 September and the Cockpit Voice Recorder on 30 September, reading out any information on the two devices was impossible as the recording tapes had run out and had not been replaced. The investigation conducted, based on analyses of the wreckage and interviews with witnesses, allowed the accident to be classified as a controlled flight into terrain (CFIT) and the causes of the accident to be traced back to a series of factors:

The aircraft involved, a Let L-410 Turbolet, was operating Central American Airlines' scheduled domestic service from San Pedro Sula to Tegucigalpa.
On 13 July 2011, Noar Linhas Aéreas Flight 4896, a Let L-410 Turbolet passenger aircraft on a domestic service from Recife to Mossoró, Brazil, crashed shortly after take-off in the Boa Viagem neighbourhood of Recife, after suffering an engine failure. All 16 people on board were killed.
Summit Air, formerly known as Goma Air, is an airline based in Kathmandu, Nepal. The airline operates from short takeoff and landing airstrips in the Western parts of Nepal. The airline received the first of two Let 410 aircraft, financed by the Czech Republic Export Bank., in October 2014, and initially only used them on flights to Lukla and Jomsom.
On 4 November 2015, an Antonov An-12 cargo aircraft crashed near the White Nile shortly after takeoff from Juba International Airport serving Juba, the capital city of South Sudan. At least 37 people were killed, including the crew of six. The crash is the deadliest aviation accident to have occurred in South Sudan since independence in 2011.
Tropical Airways Flight 1301 (TBG1301/M71301) was a domestic short-haul passenger flight, flying from Cap-Haïtien International Airport in Cap-Haïtien, Haiti to the commune of Port-de-Paix which crashed onto a sugarcane field less than 10 minutes after take off on the evening of 24 August 2003. The aircraft was a 19-seater Let L-410 Turbolet carrying 19 passengers and 2 crew. Witnesses stated that the aircraft caught fire during take-off and exploded when it hit the ground. All on board were killed.
Simon Adut Yuang was a South Sudanese Anglican bishop. He served as the Anglican Bishop of Yirol, located within the Province of the Episcopal Church of South Sudan, from 2015 until his death in a plane crash on 9 September, 2018.
West Caribbean Airways Flight 9955 was a scheduled flight between Isla de Providencia and San Andres Island, Colombia that crashed on 25 March 2005, killing 9 of the 14 passengers and crew on board.
Khabarovsk Airlines Flight 463 was a scheduled domestic passenger flight from Nikolayevsk-on-Amur Airport to Nelkan Airport in Russia. On 15 November 2017 the Let L-410 Turbolet operating the flight crashed short of the runway at Nelkan Airport, killing all but one of the seven people on board. The sole survivor was a three-year-old girl who sustained serious injuries. The crash was caused by a malfunction of the right engine's propeller.
Sakha Avia Flight 301 was a scheduled domestic passenger flight from Kutana to Aldan via Uchur in Russia. On 26 August 1993 the Let L-410 Turbolet operating the flight crashed on approach to Aldan Airport, killing all 24 people on board. It is the deadliest aviation disaster involving the Let L-410 Turbolet.
On 22 August 2020, a South West Aviation An-26 turboprop aircraft crashed upon taking off from Juba Airport in Juba, South Sudan, for a domestic cargo charter flight to Aweil and to Wau, South Sudan.

Siberian Light Aviation Flight 51 was a passenger flight on September 12, 2021, by a Let L-410 Turbolet plane, from Irkutsk north to Kazatjinskoje, Irkutsk Oblast, Siberia, near Lake Baikal. The plane crashed about 4 kilometers from the airport.
