It borders Koshi Pradesh to the east, Bagmati Province to the north, and India’s Bihar state to the south. It has an area of 9,661 km2 (3,730 sq mi) covering about 6.5% of the country's total area. It has a population of 6,126,288 as per the 2021 Nepal census, making it Nepal's most densely populated province and smallest province by area.

General elections were held in Nepal in two phases on 26 November and 7 December 2017 to elect the 275 members of the fifth House of Representatives, the lower house of the Federal Parliament of Nepal. The election was held alongside the first provincial elections for the seven provincial assemblies. A political deadlock between the governing Nepali Congress and the winning left-wing coalition over the system used to elect the upper house led to delay in forming the new government. Following the announcement of final result by the Election Commission, K.P. Oli of Communist Party of Nepal was sworn in as Prime Minister on 15 February 2018 by the President according to Article 76 (2) of the constitution. He passed a Motion of Confidence on 11 March 2018 with 208 votes.

In Nepal, the chief minister is the elected head of government of each of the seven provinces. The chief minister is appointed by the governor of the provinces according to Article 167 of the Constitution of Nepal.

The chief minister of Sudurpashchim Province is the chief executive of the Nepal state of Sudurpashchim Province. In accordance with the Constitution of Nepal, the governor is a state's de jure head, but de facto executive authority rests with the chief minister. Following elections to the Povincial Assembly of Sudurpashchim Province, the state's governor usually invites the party with a majority of seats to form the government. The governor appoints the chief minister, whose council of ministers are collectively responsible to the assembly. Hari Shankar Mishra is the current Governor of Madhesh Province. He was appointed Governor, as per the Article 163 (2) of the Constitution of Nepal by the President Bidya Devi Bhandari on the recommendation of the Council of Ministers of the Government of Nepal on 17 August 2021. He is a former member of House of representatives.

The Chief Minister of Madhesh is the head of government of Madhesh Province. The Chief Minister is appointed by the governor of according to Article 167 of the constitution of Nepal. The chief minister is the head of the council ministers of province and the chief adviser to the governor of Madhesh Province. Madhesh Province was formed after the adoption of the constitution of Nepal. On 17 January 2022, the meeting of provincial assembly declared Janakpur as Capital of Province no. 2 while the Province was rename to Madhesh Province.

The Governor of Madhesh Province is the nominal head of state of the Nepal state (province) of Madhesh Province and a representative of the President of Nepal. The governor is appointed by the term president for a term of five years. The governor's powers are mostly ceremonial and the executive powers of the governor are exercised by the chief minister of Madhesh Province, who is the head of the executive of the state government of Madhesh Province.
