The Test of Variables of Attention (T.O.V.A.) is a neuropsychological assessment that measures a person's attention while screening for attention deficit hyperactivity disorder. Generally, the test is 21.6 minutes long and is presented as a simple, yet boring, computer game. The test is used to measure a number of variables involving the test taker's response to either a visual or auditory stimulus. These measurements are then compared to the measurements of a group of people without attention disorders who took the T.O.V.A. This test should be used along with a battery of neuropsychological tests, such as a detailed history, subjective questionnaires, interviews, and symptom checklists before a diagnosis should be concluded.
The T.O.V.A. has been shown to accurately identify 87% of individuals without ADHD, 84% of non-hyperactive ADHD, and 90% of the hyperactive ADHD, but should never be used solely as a diagnostic tool for those testing for attention deficit disorders or with a traumatic brain injury.[ unreliable medical source? ] However, The TOVA generates high false positive rates (30%) in normal controls and children with other psychiatric disorders (28%). The original T.O.V.A. adult normative sample (1993) consisted of 250 subjects, age 20 and older and has not been updated to reflect current population characteristics. The sample consisted primarily of persons of Caucasian ethnicity (99%, 1% other), and consisted of undergraduate students enrolled in three Minnesota liberal arts colleges and persons residing in nearby communities. Subjects were excluded from the study based upon current use of psychoactive medication, history of CNS disorder, or history of CNS injury.
The T.O.V.A. was developed in the 1960s by Dr. Lawrence Greenberg, Head of Child and Adolescent Psychiatry at the University of Minnesota.[ unreliable medical source? ]
The first modality used diagnostically was the Test of Variability, Inattention, and Response Time (VIRTEST), a mechanical machine that measured response time. During the VIRTEST, a child would press a response button when a target was presented versus the non-target.
After individuals were diagnosed with ADHD from using the VIRTEST, Dr. Greenberg began experimentation with different medications to try developing an adequate treatment plan. The most common medications used in the trial included dextroamphetamine (a stimulant), chlorpromazine (an anti-psychotic), hydroxyzine (a minor tranquilizer), and a placebo. Upon the findings of this study Dr. Greenberg decided that using behavioral ratings, or the VIRTEST, alone was too subjective and that the ratings themselves would be influenced by the testing environment, the raters bias, and external variables.[ citation needed ]
With the advance of computers, the T.O.V.A. was made commercially available in 1991.
For individuals between the ages of 4–5, the T.O.V.A test is 10.9 minutes long, while for older individuals the test lasts 21.6 minutes. The test may be presented as either a Visual or Auditory test, but both measure the same variables. During the first section of the test, the objective is to measure attention during a boring task. For adults, this section is 10.8 minutes long and the non-target is presented 3.5 times for every 1 time a target is presented. The second section of the test is a measure of attention while attending to a stimulating task (target frequent). ADHD has three sub-types: Inattentive, hyperactive, or combined. The T.O.V.A test can test for each of these sub-types of ADHD. When the subject responds to a "non-target" it is noted as an error of commission, or impulsive. During the second half of the test, the inability for the subject to inhibit themselves is measured (error of omission). If the subject responds too frequently, they may be diagnosed with the hyperactivity type. If the subject displays both types of errors (commission and omission), they may be diagnosed with the combined type of ADHD. [3]
The visual T.O.V.A. uses two simple geometric figures and involves clicking the micro switch when the person taking the test sees the target figure and not clicking when it's the non-target figure. The visual T.O.V.A. may be presented in several different ways, but the most common test displays the target as a square with a second but smaller square inside of it near the upper border. The non-target is a square with the smaller square near the lower border.[ citation needed ]
The auditory test is the same process. The test taker clicks when they hear the target, which is presented as a single tone, usually "G" above "Middle C" (392.0 Hz). The test taker should inhibit their response when the non-target is presented, which is usually the tone of "Middle C" (261.6 Hz).
The test is monochromatic, non-sequential, language and culturally independent. It is presented in both clinical and screening versions. The clinical version is used by health professionals and assistants. The screening version has no diagnostic terms and is used by school-based professionals.
The T.O.V.A. measures a set of different variables to determine whether or not response times and attention is at the normal range for the sex and age of the test taker. Over 2000 people without attention problems were measured to determine what is a normal response time for the sex and age of the test taker as a basis for the interpretation provided. [4]

Rehabilitation of sensory and cognitive function typically involves methods for retraining neural pathways or training new neural pathways to regain or improve neurocognitive functioning that have been diminished by disease or trauma. The main objective outcome for rehabilitation is to assist in regaining physical abilities and improving performance. Three common neuropsychological problems treatable with rehabilitation are attention deficit/hyperactivity disorder (ADHD), concussion, and spinal cord injury. Rehabilitation research and practices are a fertile area for clinical neuropsychologists, rehabilitation psychologists, and others.
Developmental disorders comprise a group of psychiatric conditions originating in childhood that involve serious impairment in different areas. There are several ways of using this term. The most narrow concept is used in the category "Specific Disorders of Psychological Development" in the ICD-10. Adult Attention Deficit Hyperactivity Disorder is the persistence of attention deficit hyperactivity disorder (ADHD) into adulthood. It is a neurodevelopmental disorder, meaning impairing symptoms must have been present in childhood, except for when ADHD occurs after traumatic brain injury. Specifically, multiple symptoms must be present before the age of 12, according to DSM-5 diagnostic criteria. The cutoff age of 12 is a change from the previous requirement of symptom onset, which was before the age of 7 in the DSM-IV. This was done to add flexibility in the diagnosis of adults. ADHD was previously thought to be a childhood disorder that improved with age, but recent research has disproved this. Approximately two-thirds of childhood cases of ADHD continue into adulthood, with varying degrees of symptom severity that change over time and continue to affect individuals with symptoms ranging from minor inconveniences to impairments in daily functioning.

Neurofeedback, a specific form of biofeedback, is based on the idea that human beings can consciously alter their brain function through training sessions in which they attempt to change the signal generated by their brain and measured via a neurological feedback mechanism. Higher-order executive functions require the simultaneous use of multiple basic executive functions and include planning and fluid intelligence.
Attention deficit hyperactivity disorder predominantly inattentive, is one of the three presentations of attention deficit hyperactivity disorder (ADHD). A continuous performance task, continuous performance test, or CPT, is any of several kinds of neuropsychological test that measures a person's sustained and selective attention. Sustained attention is the ability to maintain a consistent focus on some continuous activity or stimuli, and is associated with impulsivity. Selective attention is the ability to focus on relevant stimuli and ignore competing stimuli. This skill is associated with distractibility.
In psychology, impulsivity is a tendency to act on a whim, displaying behavior characterized by little or no forethought, reflection, or consideration of the consequences. Impulsive actions are typically "poorly conceived, prematurely expressed, unduly risky, or inappropriate to the situation that often result in undesirable consequences," which imperil long-term goals and strategies for success. Impulsivity can be classified as a multifactorial construct. A functional variety of impulsivity has also been suggested, which involves action without much forethought in appropriate situations that can and does result in desirable consequences. "When such actions have positive outcomes, they tend not to be seen as signs of impulsivity, but as indicators of boldness, quickness, spontaneity, courageousness, or unconventionality." Thus, the construct of impulsivity includes at least two independent components: first, acting without an appropriate amount of deliberation, which may or may not be functional; and second, choosing short-term gains over long-term ones.
The California Verbal Learning Test (CVLT) is one of the most widely used neuropsychological tests in North America. As an instrument, it represents a relatively new approach to clinical psychology and the cognitive science of memory. It measures episodic verbal learning and memory, and demonstrates sensitivity to a range of clinical conditions. The test does this by attempting to link memory deficits with impaired performance on specific tasks. It assesses encoding, recall and recognition in a single modality of item presentation (auditory-verbal). The CVLT is considered to be a more sensitive measure of episodic memory than other verbal learning tests. It was designed to not only measure how much a subject learned, but also reveal strategies employed and the types of errors made. The CVLT indexes free and cued recall, serial position effects, semantic clustering, intrusions, interference and recognition. Delis et al. (1994) released the California Verbal Learning Test for Children (CVLT-C). The California Verbal Learning Test-II (CVLT-II) is an updated version of the original CVLT, which has been standardized and provides normative data.
In psychology and neuroscience, executive dysfunction, or executive function deficit, is a disruption to the efficacy of the executive functions, which is a group of cognitive processes that regulate, control, and manage other cognitive processes. Executive dysfunction can refer to both neurocognitive deficits and behavioural symptoms. It is implicated in numerous psychopathologies and mental disorders, as well as short-term and long-term changes in non-clinical executive control. Executive dysfunction is the mechanism underlying ADHD paralysis, and in a broader context, it can encompass other cognitive difficulties like planning, organizing, initiating tasks and regulating emotions. It is a core characteristic of ADHD and can elucidate numerous other recognized symptoms.
The Pediatric Attention Disorders Diagnostic Screener (PADDS), created by Dr. Thomas K. Pedigo and Kenneth L. Pedigo, is a suite of computer administered neuropsychological tests of attention and executive functioning. The PADDS is used in the diagnosis of attention deficit hyperactivity disorder (ADHD) in children between the ages of 6 and 12 years. The PADDS software program represents a multi-dimensional, evidence-based approach to ADHD assessment, consisting of the Computer Administered Diagnostic Interview (CADI), the Swanson, Nolan, and Pelham—IV (SNAP-IV) Parent and Teacher rating scales, and the three computer-administered objective measures of the Target Tests of Executive Functioning (TTEF). It calculates a diagnostic likelihood ratio, where each data source is allowed to contribute to (or detract from) the prediction of the diagnosis, as well as normalized relative standard scores, t-scores, z-scores, and percentile ranks for comparison to the non-clinical reference group.
Inhibitory control, also known as response inhibition, is a cognitive process – and, more specifically, an executive function – that permits an individual to inhibit their impulses and natural, habitual, or dominant behavioral responses to stimuli in order to select a more appropriate behavior that is consistent with completing their goals. Self-control is an important aspect of inhibitory control. For example, successfully suppressing the natural behavioral response to eat cake when one is craving it while dieting requires the use of inhibitory control.
Executive functions are a cognitive apparatus that controls and manages cognitive processes. Norman and Shallice (1980) proposed a model on executive functioning of attentional control that specifies how thought and action schemata become activated or suppressed for routine and non-routine circumstances. Schemas, or scripts, specify an individual's series of actions or thoughts under the influence of environmental conditions. Every stimulus condition turns on the activation of a response or schema. The initiation of appropriate schema under routine, well-learned situations is monitored by contention scheduling which laterally inhibits competing schemas for the control of cognitive apparatus. Under unique, non-routine procedures controls schema activation. The SAS is an executive monitoring system that oversees and controls contention scheduling by influencing schema activation probabilities and allowing for general strategies to be applied to novel problems or situations during automatic attentional processes.
The Delis–Kaplan Executive Function System (D-KEFS) is a neuropsychological test used to measure a variety of verbal and nonverbal executive functions for both children and adults. This assessment was developed over the span of a decade by Dean Delis, Edith Kaplan, and Joel Kramer, and it was published in 2001. The D-KEFS comprises nine tests that were designed to stand alone. Therefore, there are no aggregate measures or composite scores for an examinee's performance. A vast majority of these tests are modified, pre-existing measures ; however, some of these measures are new indices of executive functions.

The Swanson, Nolan and Pelham Teacher and Parent Rating Scale (SNAP), developed by James Swanson, Edith Nolan and William Pelham, is a 90-question self-report inventory designed to measure attention deficit hyperactivity disorder (ADHD) and oppositional defiant disorder (ODD) symptoms in children and young adults.
The ADHD Rating Scale (ADHD-RS) is a parent-report or teacher-report inventory created by George J. DuPaul, Thomas J. Power, Arthur D. Anastopoulos, and Robert Reid consisting of 18–90 questions regarding a child's behavior over the past 6 months. The ADHD Rating Scale is used to aid in the diagnosis of attention deficit hyperactivity disorder (ADHD) in children ranging from ages 5–17.
Nesplora Aula is a psychological test in virtual reality that measures attentional processes in children from 6 to 16 years of age. It is a level III C Continuous Performance Test (CPT) that combines auditory and visual stimuli in a virtual environment: a school classroom, alive and organic, with the distractors of that environment. It evaluates in approximately 20 minutes, among other variables, attention, impulsivity, processing speed, the tendency to distraction and motor activity during the task. The person being evaluated is immersed in virtual reality through a helmet with movement sensors and lightweight 3D glasses.
