China Northwest Airlines Flight 2119 (WH2119) was a flight from Yinchuan Xihuayuan Airport, Ningxia to Beijing Capital International Airport, People's Republic of China. On July 23, 1993, the aircraft crashed into a lake after it was unable to get airborne while attempting to take off at Yinchuan Airport, killing 54 passengers and 1 crew member on board. [1]
Just before rotation on takeoff, the right-side flap actuator failed, causing the flaps to retract. Unable to get the aircraft into the air, the crew had no other option but to abort the takeoff. [1] This failed however, the nose gear lifted off causing the tail to strike the runway. The aircraft then overran the runway and crashed into a lake. [2]
It was suspected that the crew failed to check if the flaps were deployed properly for takeoff. [2]
The Tenerife airport disaster occurred on March 27, 1977, when two Boeing 747 passenger jets collided on the runway at Los Rodeos Airport on the Spanish island of Tenerife. The collision occurred when KLM Flight 4805 initiated its takeoff run during dense fog while Pan Am Flight 1736 was still on the runway. The impact and resulting fire killed everyone on board KLM 4805 and most of the occupants of Pan Am 1736, with only 61 survivors in the front section of the aircraft, resulting in the worst aviation disaster in Spain. With 583 fatalities, the disaster is also the deadliest accident in aviation history.
Delta Air Lines Flight 1141 was a scheduled domestic passenger flight between Dallas/Fort Worth, Texas and Salt Lake City, Utah. On August 31, 1988, the flight, using a Boeing 727-200 series aircraft, crashed during takeoff, resulting in 14 deaths and 76 injuries of the 108 on board.
Akron–Canton Airport is a commercial airport in the city of Green, in southern Summit County, Ohio. The airport is located about 14 miles (23 km) southeast of Akron and 10 miles (16 km) northwest of Canton. It is jointly operated by Summit County and Stark County. The airport is a "reliever" airport for Northeast Ohio and markets itself as "A better way to go", emphasizing the ease of travel in comparison to Cleveland Hopkins International Airport. Just under 90% of its traffic is general aviation. It is included in the Federal Aviation Administration (FAA) National Plan of Integrated Airport Systems for 2019–2023, in which it is categorized as a small-hub primary commercial service facility.
On August 16, 1987 a McDonnell Douglas MD-82, operating as Northwest Airlines Flight 255, crashed shortly after takeoff from Detroit Metropolitan Airport, about 8:46 pm EDT, resulting in the deaths of all six crew members and 148 of the 149 passengers, along with two people on the ground. The sole survivor was a 4-year-old girl who sustained serious injuries. It was the second-deadliest aviation accident at the time in the United States. It is also the deadliest aviation accident to have a sole survivor.
Lufthansa Flight 540 was a scheduled commercial flight for Lufthansa, serving the Frankfurt–Nairobi–Johannesburg route.
USAir Flight 405 was a regularly scheduled domestic passenger flight between LaGuardia Airport in Queens, New York City, New York, and Cleveland, Ohio. On March 22, 1992, a USAir Fokker F28, registration N485US, flying the route, crashed in poor weather in a partially inverted position in Flushing Bay, shortly after liftoff from LaGuardia. The undercarriage lifted off from the runway, but the airplane failed to gain lift, flying only several meters above the ground. The aircraft then veered off the runway and hit several obstructions before coming to rest in Flushing Bay, just beyond the end of the runway. Of the 51 people on board, 27 were killed, including the captain and a member of the cabin crew.
Pilot error generally refers to an accident in which an action or decision made by the pilot was the cause or a contributing factor that led to the accident, but also includes the pilot's failure to make a correct decision or take proper action. Errors are intentional actions that fail to achieve their intended outcomes. The Chicago Convention defines the term "accident" as "an occurrence associated with the operation of an aircraft [...] in which [...] a person is fatally or seriously injured [...] except when the injuries are [...] inflicted by other persons." Hence the definition of "pilot error" does not include deliberate crashing.
Qantas Flight 1 was a Qantas passenger flight between Sydney and London that was involved in a runway overrun accident at Don Mueang International Airport in Bangkok on 23 September 1999 as it was landing for a stopover.
LANSA Flight 502 was a Lockheed L-188A Electra operated by Líneas Aéreas Nacionales Sociedad Anónima (LANSA) which crashed shortly after takeoff from Quispiquilla Airport near Cusco, Peru, on August 9, 1970, after losing all power from one of its four engines. The turboprop airliner, registered OB-R-939, was bound from Cusco to Lima, carrying 8 crew and 92 passengers. All but one of the occupants died from injuries sustained from impact forces and post crash fire. Two people on the ground were also killed. There were 49 American high school exchange students on board, all of whom perished. A Peruvian government investigation concluded that the accident was caused by improper execution of engine-out procedures by the flight crew and lack of proper maintenance. LANSA was fined and its operations were suspended for 90 days. At the time, the crash was the deadliest ever in Peruvian history before being surpassed by Faucett Perú Flight 251 in 1996.
Comair Flight 5191 was a scheduled United States domestic passenger flight from Lexington, Kentucky, to Atlanta, Georgia. On the morning of August 27, 2006, at around 06:07 EDT, the Bombardier Canadair Regional Jet 100ER crashed while attempting to take off from Blue Grass Airport in Fayette County, Kentucky, 4 miles west of the central business district of the city of Lexington.

Spanair Flight 5022 was a scheduled domestic passenger flight from Barcelona–El Prat Airport to Gran Canaria Airport, Spain, via Madrid–Barajas Airport that crashed just after take-off from runway 36L at Madrid Airport at 14:24 CEST (12:24 UTC) on 20 August 2008. The aircraft was a McDonnell Douglas MD-82, registration EC-HFP. Of the 172 passengers and crew on board, 154 died and 18 survived.
Air France has been in operation since 1933. Its aircraft have been involved in a number of major accidents and incidents. The deadliest accident of the airline occurred on June 1, 2009, when Air France Flight 447, an Airbus A330-203, flying from Rio de Janeiro to Paris crashed into the Atlantic Ocean with 228 fatalities. Centurion Air Cargo Flight 164 was a chartered international cargo flight, flying from Bogota's El Dorado International Airport while en route to Miami International Airport. The flight was operated by Kalitta Air and the aircraft was wet leased by Centurion Air Cargo. On 7 July 2008, the aircraft, a Boeing 747-209BSF registered as N714CK, crashed shortly after takeoff. All aboard suffered injuries, but none were killed. Two people on the ground were killed after the plane slammed into a farm. The crash was the second crash of a Boeing 747 in 2008 in Kalitta Air service, after a previous accident at Brussels in May.
Air Manila Flight 702 was an unscheduled passenger flight from Naval Air Station Agana in Guam to Ninoy Aquino International Airport in Manila, carrying 33 passengers and 12 crew members; most of whom were personnel from the base. The Lockheed L-188A Electra attempted takeoff from runway 6L but crashed near a residential area; the crash was caused by retracting the flaps at an altitude too low to clear the terrain after the propeller of engine number three feathered. All 45 people on board and one person on the ground perished in the crash. The investigation concluded that the pilot should have followed company policy by aborting takeoff in the event of an engine failure before reaching VR (takeoff) speed.
Kalitta Air Flight 207 (K4207/CKS207) was a scheduled cargo flight between John F. Kennedy Airport to Bahrain International Airport with a technical stopover at Brussels. On May 25, 2008, the Boeing 747-200 overran runway 20 during takeoff at Brussels Airport, causing the aircraft to split into three large pieces. The occupants sustained minor injuries.
