The Tenerife airport disaster occurred on 27 March 1977, when two Boeing 747 passenger jets collided on the runway at Los Rodeos Airport on the Spanish island of Tenerife. The collision occurred when KLM Flight 4805 initiated its takeoff run in dense fog while Pan Am Flight 1736 was still on the runway. The impact and the resulting fire killed all 248 people on board the KLM plane and 335 of the 396 people on board the Pan Am plane, with only 61 survivors in the front section of the latter aircraft. With a total of 583 fatalities, the disaster is the deadliest accident in aviation history.
Delta Air Lines Flight 1141 was a scheduled domestic passenger flight between Dallas Fort Worth International Airport, Texas, and Salt Lake City International Airport, Utah. On August 31, 1988, the flight, using a Boeing 727-200 series aircraft, crashed during takeoff at Dallas Fort Worth International Airport, resulting in 14 deaths and 76 injuries among the 108 on board.
On August 16, 1987, a McDonnell Douglas MD-82, operating as Northwest Airlines Flight 255, crashed shortly after takeoff from Detroit Metropolitan Airport, about 8:46 pm EDT, resulting in the deaths of all six crew members and 148 of the 149 passengers, along with two people on the ground. The sole survivor was a 4-year-old girl who sustained serious injuries. It was the second-deadliest aviation accident at the time in the United States. It is also the deadliest aviation accident to have a sole survivor, the deadliest plane crash in the history of the state of Michigan, and the worst crash in the history of Northwest Airlines.

In aviation, pilot error generally refers to an action or decision made by a pilot that is a substantial contributing factor leading to an aviation accident. It also includes a pilot's failure to make a correct decision or take proper action. Errors are intentional actions that fail to achieve their intended outcomes. The Chicago Convention defines the term "accident" as "an occurrence associated with the operation of an aircraft [...] in which [...] a person is fatally or seriously injured [...] except when the injuries are [...] inflicted by other persons." Hence the definition of "pilot error" does not include deliberate crashing.
Mandala Airlines Flight 091 (RI091/MDL091) was a scheduled domestic passenger flight from Medan to Jakarta, operated by Mandala Airlines with a Boeing 737-200Adv. On September 5, 2005 at 10:15 a.m. WIB (UTC+7), the aircraft stalled and crashed into a heavily populated residential area seconds after taking off from Polonia International Airport. Of the 117 passengers and crews on board, only 17 survived. An additional 49 civilians on the ground were killed.
Qantas 1 was a Qantas passenger flight between Sydney and London that was involved in a runway overrun accident at Don Mueang International Airport in Bangkok on 23 September 1999 as it was landing for a stopover.
LANSA Flight 502 was a Lockheed L-188A Electra operated by Líneas Aéreas Nacionales Sociedad Anónima (LANSA) which crashed shortly after takeoff from Quispiquilla Airport near Cusco, Peru, on August 9, 1970, after losing all power from one of its four engines. The turboprop airliner, registered OB-R-939, was bound from Cusco to Lima, carrying 8 crew and 92 passengers. All but one of the occupants died from injuries sustained from impact forces and post crash fire. Two people on the ground were also killed. There were 49 American high school exchange students on board, all of whom perished. A Peruvian government investigation concluded that the accident was caused by improper execution of engine-out procedures by the flight crew and lack of proper maintenance. LANSA was fined and its operations were suspended for 90 days. At the time, the crash was the deadliest ever in Peruvian history before being surpassed by Faucett Perú Flight 251 in 1996.
Comair Flight 5191 was a scheduled United States domestic passenger flight from Lexington, Kentucky, to Atlanta, Georgia. On the morning of August 27, 2006, at around 06:07 EDT, the Bombardier Canadair Regional Jet 100ER crashed while attempting to take off from Blue Grass Airport in Fayette County, Kentucky, 4 miles west of the central business district of the city of Lexington.

Spanair Flight 5022 or (JK5022/JKK5022) was a scheduled domestic passenger flight from Barcelona–El Prat Airport to Gran Canaria Airport, Spain, with a stopover in Madrid–Barajas Airport that crashed just after take-off from runway 36L at Madrid-Barajas Airport to Gran Canaria Airport at 14:24 CEST (12:24 UTC) on 20 August 2008. The aircraft was a McDonnell Douglas MD-82, registration EC-HFP. Of the 172 passengers and crew on board, 154 died and 18 survived.
China General Aviation Flight 7552 was a China General Aviation flight from Nanjing Dajiaochang Airport to Xiamen Gaoqi International Airport. On July 31, 1992, the Yakovlev Yak-42D overran runway 06 during takeoff and impacted an embankment at 210 kilometres per hour, 420 metres (1,380 ft) from the threshold.
Air France has been in operation since 1933. Its aircraft have been involved in a number of major accidents and incidents. The deadliest accident of the airline occurred on June 1, 2009, when Air France Flight 447, an Airbus A330-203, flying from Rio de Janeiro to Paris crashed into the Atlantic Ocean with 228 fatalities. American Airlines Flight 514 was a training flight from Idlewild International Airport, to the Grumman Aircraft Corp. airfield. On the afternoon of August 15, 1959, the Boeing 707 operating the flight crashed near the Calverton airport, killing all five crew members aboard. This was the first accident to involve a Boeing 707, which had only gone into service in October of the previous year, and the first of three accidents involving American's 707s in the New York area within three years, followed by Flight 1502 and Flight 1.
Centurion Air Cargo Flight 164 was a chartered international cargo flight, flying from Bogota's El Dorado International Airport while en route to Miami International Airport. The flight was operated by Kalitta Air and the aircraft was wet leased by Centurion Air Cargo. On 7 July 2008, the aircraft, a Boeing 747-209BSF registered as N714CK, crashed shortly after takeoff. All aboard suffered injuries, but none were killed. Two people on the ground were killed after the plane slammed into a farm. The crash was the second crash of a Boeing 747 in 2008 in Kalitta Air service, after a previous accident at Brussels in May.
Air Manila Flight 702 was an unscheduled passenger flight from Naval Air Station Agana in Guam to Ninoy Aquino International Airport in Manila, carrying 33 passengers and 12 crew members; most of whom were personnel from the base. The Lockheed L-188A Electra attempted takeoff from runway 6L but crashed near a residential area; the crash was caused by retracting the flaps at an altitude too low to clear the terrain after the propeller of engine number three feathered. All 45 people on board and one person on the ground perished in the crash. The investigation concluded that the pilot should have followed company policy by aborting takeoff in the event of an engine failure before reaching VR (takeoff) speed.
Kalitta Air Flight 207 (K4207/CKS207) was a scheduled cargo flight between John F. Kennedy Airport to Bahrain International Airport with a technical stopover at Brussels. On May 25, 2008, the Boeing 747-200 suffered a bird strike and overran runway 20 during takeoff at Brussels Airport, causing the aircraft to split into three large pieces. The occupants sustained minor injuries.
