The National Transportation Safety Board (NTSB) is an independent U.S. government investigative agency responsible for civil transportation accident investigation. In this role,the NTSB investigates and reports on aviation accidents and incidents,certain types of highway crashes,ship and marine accidents,pipeline incidents,bridge failures,and railroad accidents. The NTSB is also in charge of investigating cases of hazardous materials releases that occur during transportation. The agency is based in Washington,D.C. It has four regional offices,located in Anchorage,Alaska;Denver,Colorado;Ashburn,Virginia;and Seattle,Washington. The agency also operated a national training center at its Ashburn facility.
Marion Clifton Blakey is an American businesswoman and former government official who served as president and CEO of Rolls-Royce North America. Prior to joining Rolls-Royce,she served as the eighth full-time chief executive of the Aerospace Industries Association,an American defense industry trade association from 2007 to 2015. Before this,she served a five-year term as the 15th Administrator of the Federal Aviation Administration. Blakey was the second woman to hold the position,serving as a successor to Jane Garvey,the first woman to hold the Administrator title. She was the second Administrator who was not a licensed pilot. She was awarded the Wright Brothers Memorial Trophy in 2013.
Mary Louise "Missy" Cummings is an American academic who is a professor at Duke University and director of Duke's Humans and Autonomy Laboratory. She was one of the United States Navy's first female fighter pilots. In November 2021,Dr. Cummings joined the National Highway Traffic Safety Administration (NHTSA). She currently teaches at George Mason University.

Hanjin Venezia,formerly named the Cosco Busan,is a 275 m (902 ft) container ship. On 7 November 2007,it collided with the protective fender of the Delta Tower of the San Francisco–Oakland Bay Bridge in heavy fog. The collision sliced open two of its fuel tanks and led to the Cosco Busan oil spill in San Francisco Bay. She was renamed the Hanjin Venezia after the accident.
Mark Victor Rosenker was an American aviation official who served as the 11th chairman of the National Transportation Safety Board (NTSB) from August 2006 through August 2008. He was nominated by President George W. Bush and unanimously confirmed by the U.S. Senate. Chairman Rosenker was renominated by President Bush for a second term as chair and was going through the Senate confirmation process,which basically ended when President Obama was elected in November 2008. He served as acting chair starting in March 2005 to August 2006 and then again from August 2008 until his resignation from the board in August 2009,when he returned to private life. He died on September 26,2020,from brain cancer at the age of 73.

Asiana Airlines Flight 214 was a scheduled transpacific passenger flight originating from Incheon International Airport near Seoul,South Korea. On the morning of July 6,2013,the Boeing 777-200ER operating the flight crashed on final approach into San Francisco International Airport in the United States. Of the 307 people on board,three died;another 187 were injured,49 of them seriously. Among the seriously injured were four flight attendants who were thrown onto the runway while still strapped in their seats when the tail section broke off after striking the seawall short of the runway. It was the first fatal crash of a Boeing 777 since the aircraft type entered service in 1995.
In rail transport,the U.S. DOT-111 tank car,also known as the TC-111 in Canada,is a type of unpressurized general service tank car in common use in North America. Tank cars built to this specification must be circular in cross section,with elliptical,formed heads set convex outward. He was confirmed to serve as chairman on February 5,2015. Robert Sumwalt succeeded him as chairman in August 2017.
