In aviation, a mid-air collision is an accident in which two or more aircraft come into unplanned contact during flight. [1] Owing to the relatively high velocities involved and the likelihood of subsequent impact with the ground or sea, very severe damage or the total destruction of at least one of the aircraft usually results. For this reason, accidents involving mid-air collisions especially during cruise frequently result in very few survivors or, more often, a total lack thereof. This is especially when the accident involves jet aircraft.

The potential for a mid-air collision is increased by miscommunication, mistrust, error in navigation, deviations from flight plans, lack of situational awareness, and the lack of collision-avoidance systems. Although a rare occurrence in general due to the vastness of open space available, collisions often happen near or at airports, where large volumes of aircraft are spaced more closely than in general flight.

First recorded collision

The first recorded collision between aircraft occurred at the "Milano Circuito Aereo Internazionale" meeting held between 24 September and 3 October 1910 in Milan, Italy. On 3 October, Frenchman René Thomas, flying the Antoinette IV monoplane, collided with British Army Captain Bertram Dickson by ramming his Farman III biplane in the rear. [2] Both pilots survived, but Dickson was so badly injured that he never flew again. [3] [4] [5]

The first fatal collision occurred over La Brayelle Airfield, Douai, France, on 19 June 1912. Captain Marcel Dubois and Lieutenant Albert Peignan, both of the French Army, crashed into one another in an early-morning haze, killing both pilots. [6] [7]

Traffic collision avoidance system

Almost all modern large aircraft are fitted with a traffic collision avoidance system (TCAS), which is designed to try to prevent mid-air collisions. The system, based on the signals from aircraft transponders, alerts pilots if a potential collision with another aircraft is imminent. Despite its limitations, it is believed to have greatly reduced mid-air collisions. [8]

On 1 July 2002, BALBashkirian Airlines Flight 2937, a Tupolev Tu-154 passenger jet, and DHL International Aviation ME Flight 611, a Boeing 757 cargo jet, collided in mid-air over Überlingen, a southern German town on Lake Constance, near the German-Swiss border. All of the passengers and crew aboard both planes were killed, resulting in a total death toll of 71.

<span class="mw-page-title-main">Traffic collision avoidance system</span> Aircraft collision avoidance system

A traffic alert and collision avoidance system (TCAS), pronounced TEE-kas), also known as an Airborne Collision Avoidance System (ACAS), is an aircraft collision avoidance system designed to reduce the incidence of mid-air collision (MAC) between aircraft. It monitors the airspace around an aircraft for other aircraft equipped with a corresponding active transponder, independent of air traffic control, and warns pilots of the presence of other transponder-equipped aircraft which may present a threat of MAC. It is a type of airborne collision avoidance system mandated by the International Civil Aviation Organization to be fitted to all aircraft with a maximum take-off mass (MTOM) of over 5,700 kg (12,600 lb) or authorized to carry more than 19 passengers. CFR 14, Ch I, part 135 requires that TCAS I be installed for aircraft with 10–30 passengers and TCAS II for aircraft with more than 30 passengers. ACAS/TCAS is based on secondary surveillance radar (SSR) transponder signals, but operates independently of ground-based equipment to provide advice to the pilot on potentially conflicting aircraft.

Aeroméxico Flight 498 was a scheduled commercial flight from Mexico City, Mexico, to Los Angeles, California, United States, with several intermediate stops. On Sunday, August 31, 1986, the McDonnell Douglas DC-9 operating the flight was clipped in the tail section by N4891F, a Piper PA-28-181 Cherokee owned by the Kramer family, and crashed into the Los Angeles suburb of Cerritos, killing all 64 on the DC-9, all 3 on the Piper and an additional 15 people on the ground. Eight on the ground also sustained minor injuries. Blame was assessed equally on the Federal Aviation Administration (FAA) and the pilot of the Cherokee. No fault was found with the DC-9 or the actions of its crew.

<span class="mw-page-title-main">1996 Charkhi Dadri mid-air collision</span> Fatal mid-air plane collision in India

On 12 November 1996, Saudia Flight 763, a Boeing 747 en route from Delhi, India, to Dhahran, Saudi Arabia, and Kazakhstan Airlines Flight 1907, an Ilyushin Il-76 en route from Chimkent, Kazakhstan, to Delhi, collided over the city of Charkhi Dadri, around 100 km west of Delhi. The crash killed all 349 people on board both planes, making it the world's deadliest mid-air collision and the deadliest aviation accident ever in India. The final report from the investigation found that "[T]he root and approximate cause of the collision" was the failure of the Kazakh crew to maintain the correct altitude. Contributing factors included the poor English language skills in the Kazakh cockpit resulting in inadequate interpretation of directions provided by air traffic control, and three specific incidents of failures in crew resource management (CRM) by the Kazakh crew. The report also suggested technical enhancements that would provide assistance in preventing a future crew's mistakes being allowed to go unchecked in real time.

<span class="mw-page-title-main">1960 New York mid-air collision</span> Aviation disaster in New York City

On December 16, 1960, a United Airlines Douglas DC-8 bound for Idlewild Airport in New York City collided in midair with a TWA Lockheed L-1049 Super Constellation descending toward LaGuardia Airport. The Constellation crashed on Miller Field in Staten Island and the DC-8 in Park Slope, Brooklyn, killing all 128 aboard the two aircraft and six people on the ground. The accident was the world's deadliest aviation disaster at the time, and remains the deadliest accident in the history of United Airlines.

<span class="mw-page-title-main">TWA Flight 553</span> 1967 aviation disaster in Concord Township, Ohio, US

Trans World Airlines (TWA) Flight 553 was a McDonnell Douglas DC-9-15 jet airliner, registration N1063T, operated by Trans World Airlines on March 9, 1967 between Pittsburgh, Pennsylvania and Dayton, Ohio. While descending toward Dayton about 29 miles from the airport, the aircraft collided in midair with a Beechcraft Baron, a small, general-aviation airplane, near Urbana, Ohio. All 25 aboard the DC-9 and the sole occupant of the Beechcraft were killed.

<span class="mw-page-title-main">Allegheny Airlines Flight 853</span> 1969 mid-air collision

Allegheny Airlines Flight 853 was a regularly scheduled Allegheny Airlines flight from Boston, Massachusetts, to St. Louis, Missouri, with stops in Baltimore, Maryland, Cincinnati, Ohio, and Indianapolis, Indiana. On September 9, 1969, the aircraft serving the flight, a McDonnell Douglas DC-9, collided in mid-air with a Piper PA-28 light aircraft near Fairland, Indiana. The DC-9 was carrying 78 passengers and 4 crew members, and the Piper was leased to a student pilot on a solo cross-country flight. All 83 occupants of both aircraft were killed in the accident and both aircraft were destroyed.

<span class="mw-page-title-main">Gol Transportes Aéreos Flight 1907</span> 2006 mid-air plane collision in Brazil

On 29 September 2006, Gol Transportes Aéreos Flight 1907, a Boeing 737-800 on a scheduled domestic passenger flight from Manaus, Brazil, to Brasília and Rio de Janeiro, collided mid-air with an Embraer Legacy 600 business jet flying on an opposite heading over the Brazilian state of Mato Grosso. The winglet of the Legacy sliced off about half of the 737's left wing, causing the 737 to break up and crash into an area of dense jungle, killing all 154 passengers and crew on board. Despite sustaining serious damage to its left wing and tail, the Legacy landed with its seven occupants uninjured.

A transponder is an electronic device that produces a response when it receives a radio-frequency interrogation. Aircraft have transponders to assist in identifying them on air traffic control radar. Collision avoidance systems have been developed to use transponder transmissions as a means of detecting aircraft at risk of colliding with each other.

Hughes Airwest Flight 706 was a regularly scheduled flight operated by American domestic airline Hughes Airwest from Los Angeles, California to Seattle, Washington, with several intermediate stops. On Sunday, June 6, 1971, the McDonnell Douglas DC-9 serving as Flight 706 departed Los Angeles just after 6 p.m. en route to Seattle as a McDonnell Douglas F-4 Phantom II of the United States Marine Corps was approaching Marine Corps Air Station El Toro near Irvine at the end of a flight from Naval Air Station Fallon in Nevada. The two aircraft collided in midair over the San Gabriel Mountains near Duarte, killing all 49 aboard the DC-9 and the F-4 pilot; the F-4 radar intercept officer ejected and survived.

The navigation paradox states that increased navigational precision may result in increased collision risk. In the case of ships and aircraft, the advent of Global Positioning System (GPS) navigation has enabled craft to follow navigational paths with such greater precision, that, without better distribution of routes, coordination between neighboring craft and collision avoidance procedures, the likelihood of two craft occupying the same space on the shortest distance line between two navigational points has increased.

<span class="mw-page-title-main">1956 Grand Canyon mid-air collision</span> Mid-air collision on June 30, 1956 over the Grand Canyon

The Grand Canyon mid-air collision occurred in the western United States on June 30, 1956, when a United Airlines Douglas DC-7 struck a Trans World Airlines Lockheed L-1049 Super Constellation over Grand Canyon National Park, Arizona. The first plane fell into the canyon while the other slammed into a rock face. All 128 on board both airplanes perished, making it the first commercial airline incident to exceed one hundred fatalities. The airplanes had departed Los Angeles International Airport minutes apart from each other and headed for Chicago and Kansas City, respectively. The collision took place in uncontrolled airspace, where it was the pilots' responsibility to maintain separation. This highlighted the antiquated state of air traffic control, which became the focus of major aviation reforms.

The Federal Aviation Act of 1958 was an act of the United States Congress, signed by President Dwight D. On August 8, 2009, at 11:53 a.m. (15:53 UTC), nine people died when a tour helicopter and a small private airplane collided over the Hudson River near Frank Sinatra Park in Hoboken, New Jersey, United States. The aircraft were in an area known as the "Hudson River VFR Corridor", which extends from the surface of the river to altitudes of 800 to 1,500 ft at various locations along the Hudson River in the immediate area of New York City. Within this corridor, aircraft operate under visual flight rules (VFR), under which the responsibility to see and avoid other air traffic rests with the individual pilots rather than with the air traffic controller (ATC).

<span class="mw-page-title-main">SkyWest Airlines Flight 1834</span> 1987 mid-air collision

On Thursday, January 15, 1987, SkyWest Airlines Flight 1834, a Swearingen SA-226TC, and a Mooney M20 were involved in a midair collision at 12:52 MST (UTC−7) near Kearns, Utah, a suburb southwest of Salt Lake City. All ten aboard the two aircraft were killed: two pilots and six passengers aboard the METRO II and two aboard the Mooney.

<span class="mw-page-title-main">2015 Senegal mid-air collision</span> Aviation accident over Senegal

On 5 September 2015, CEIBA Intercontinental Flight 071, a Boeing 737 passenger jet en route from Dakar, Senegal, to Malabo, Equatorial Guinea, collided mid-air with a Hawker Siddeley HS-125 air ambulance jet operated by Senegalair. The 737 was slightly damaged and managed to land safely at Malabo, but the HS-125, after remaining airborne for almost an hour with the crew unresponsive, eventually crashed into the ocean, killing all seven people on board.

<span class="mw-page-title-main">1997 Namibia mid-air collision</span> Collision between USAF C-141B and German Air Force Tu-154M

On 13 September 1997, a German Air Force Tupolev Tu-154M observation aircraft and a United States Air Force Lockheed C-141B Starlifter transport aircraft were destroyed in a mid-air collision while cruising at 35,000 feet (11,000 m) off the coast of Namibia. All 33 people onboard both aircraft were killed. At the time of the collision, the Tupolev was flying on a southerly route from Niamey, Niger, to Cape Town, South Africa, while the C-141 was heading northwest from Windhoek, Namibia, to Ascension Island.
