On October 30, 1959, Piedmont Airlines Flight 349, a Douglas DC-3, crashed on Bucks Elbow Mountain near Crozet, Virginia, killing the crew of three and all but one of its twenty-four passengers. The sole survivor was seriously injured and lay on the ground near the wreckage, still strapped in his seat. [1]
The aircraft was on an instrument landing system (ILS) approach to Charlottesville–Albemarle Airport inbound from Washington National Airport. While performing an inbound turn, the aircraft crashed into Bucks Elbow Mountain at 2,600 feet (790 m). [2]
The subsequent investigation determined the cause of the accident to be:
A navigational omission which resulted in a lateral course error that was not detected and corrected through precision instrument flying procedures. A contributing factor to the accident may have been pre-occupation of the captain resulting from mental stress. [2]
The Air Line Pilots Association conducted its own investigation and came to a very different conclusion. Rather than missing the one turn on their flight, the pilot and co-pilot, according to ALPA, may have been led astray by faulty radio beacons. The ALPA report, citing numerous instances of an intermittent signal at the beacon for the Charlottesville airport, found that the beacon for a private field in Hagerstown, Maryland, could have overridden and caused the collision with the mountain. The accident aircraft, named Buckeye Pacemaker, was registered as N55V and had construction number 20447. The aircraft had previously flown with Meteor Air Transport as N53593 and was sold to Piedmont Airlines in December 1956.

On August 16, 1987, a McDonnell Douglas MD-82, operating as Northwest Airlines Flight 255, crashed shortly after takeoff from Detroit Metropolitan Airport, about 8:46 pm EDT, resulting in the deaths of all six crew members and 148 of the 149 passengers, along with two people on the ground. The sole survivor was a 4-year-old girl who sustained serious injuries. It was the second-deadliest aviation accident at the time in the United States. It is also the deadliest aviation accident to have a sole survivor, the deadliest plane crash in the history of the state of Michigan, and the worst crash in the history of Northwest Airlines.
Eastern Air Lines Flight 512 was a scheduled domestic passenger flight operated by Eastern Air Lines from Charlotte Municipal Airport in Charlotte, North Carolina, to Idlewild Airport in New York City. On November 30, 1962, while attempting to abort an attempted landing at its destination, the Douglas DC-7B operating the flight crashed after it failed to gain altitude and struck the ground. 25 of the 51 occupants of the plane died in the accident. Emergency crews responded, but rescuers were delayed by the thick fog and the soft terrain. An investigation launched after the crash found that the probable cause of the accident was that the pilots had made critical mistakes during the go-around that prevented the aircraft from gaining altitude.
Charlottesville–Albemarle Airport is an airport eight miles north of Charlottesville, in Albemarle County, Virginia, United States. CHO underwent major construction in summer 2006; an 800-foot runway extension began in summer 2010 and was completed in December 2012.
Piedmont Airlines was a local service carrier, a scheduled carrier in the United States that operated from 1948 until it merged with USAir in 1989. Its headquarters were at One Piedmont Plaza in Winston-Salem, North Carolina, a building that is now part of Wake Forest University.
BCPA Flight 304/44 was a scheduled flight operated by British Commonwealth Pacific Airlines from Sydney, Australia, to Vancouver, Canada, with scheduled stops at Fiji, Canton Island, Honolulu and San Francisco. On 29 October 1953, the flight was conducted by a Douglas DC-6 named Resolution and registered in Australia as VH-BPE. The propliner crashed during its initial approach towards San Francisco International Airport, killing all 19 people on board, including the American pianist William Kapell.
Linjeflyg Flight 267V was a controlled flight into terrain by a Convair 440-75 Metropolitan on 20 November 1964 at 21:14 in Ängelholm, Skåne, Sweden. The Linjeflyg pilots, misled by a non-conventional military runway light configuration, descended too early and on a faulty course during approach to Ängelholm–Helsingborg Airport. The crash killed 31 of 43 people on board, making it the deadliest aviation accident in Sweden.
Inex-Adria Aviopromet Flight 1308 was a McDonnell Douglas MD-81 aircraft operating a Yugoslavian charter flight to the French island of Corsica. On 1 December 1981, the flight crashed on Corsica's Mont San-Pietro, killing all 180 people on board. The crash was the deadliest and first major aviation accident involving a McDonnell Douglas MD-80.
Western Airlines Flight 2605, nicknamed the "Night Owl", was an international scheduled passenger flight from Los Angeles, California, to Mexico City, Mexico. On October 31, 1979, at 5:42 a.m. CST (UTC−06:00), the McDonnell Douglas DC-10 used on the flight crashed at Mexico City International Airport in fog after landing on a runway that was closed for maintenance. Of the 89 people on board, 72 were killed, in addition to a maintenance worker who died when the plane struck his vehicle.
Alitalia Flight 404 (AZ404/AZA404) was an international passenger flight scheduled to fly from Linate Airport in Milan, Italy, to Zürich Airport in Zürich, Switzerland, which crashed on 14 November 1990. The McDonnell Douglas DC-9-32, operated by Alitalia, crashed into the woodlands of Weiach as it approached Zurich Airport, killing all 46 occupants on board.
Invicta International Airlines Flight 435 (IM435) was a Vickers Vanguard 952, flying from Bristol Lulsgate to Basel-Mulhouse, which crashed into a forested hillside near Hochwald, Switzerland on 10 April 1973. The aircraft somersaulted and broke up, killing 108 people, with 37 survivors. To date, this is the deadliest accident involving a Vickers Vanguard and the deadliest aviation accident to occur on Swiss soil. Many of the 139 passengers on the charter flight were women, members of the Axbridge Ladies Guild, from the Somerset towns and villages of Axbridge, Cheddar, Winscombe and Congresbury. The accident left 55 children motherless and became known in the British media as the Basle air crash.
Transcontinental and Western Airways Flight 1 , a Douglas DC-2, crashed into Cheat Mountain, near Uniontown, Pennsylvania, approximately 10:20 a.m. Eastern Standard Time on April 7, 1936, killing 12 of the 14 passengers and crew aboard. Flight 1 was a regularly scheduled TWA Sun Racer flight from Newark, New Jersey, to Los Angeles, California, with almost a dozen intermediate stops between. Approaching the flight's second stop, Pittsburgh, Pennsylvania's Allegheny County Airport, pilot Otto Ferguson lost contact with the airport's radio navigation signal, and tracked several miles in a southwestern line off course. Fearing icing conditions, he descended in an attempt to find visual landmarks for navigation. Thick fog hindered him, and his descent continued until Flight 1 hit ice-covered trees atop Cheat Mountain, about 40 miles (64 km) south of Pittsburgh on the West Virginia line and near Uniontown, Pennsylvania. When the plane crashed it was aiming in a northern flight direction indicating that the pilot finally realized he had tracked south of his flightplan and may have been trying to correct it.
TWA Flight 15A , a Douglas DC-2 operated by Transcontinental and Western Air, crashed into a gully in Clifton, Pennsylvania, now Upper Saint Clair, a suburb approximately 7 miles (11 km) south of Pittsburgh. The crash occurred at approximately 6:40 p.m. Eastern Standard Time on March 25, 1937, killing all 13 passengers and crew members. Flight 15A was a regularly scheduled flight from Newark, New Jersey to Pittsburgh, Pennsylvania via Camden, New Jersey.
