American Airlines Flight 711 was a scheduled flight departing from Newark, New Jersey to Tulsa, Oklahoma, with several intermediate stops, Springfield, Missouri being one of them. [1] On March 20, 1955, the aircraft operating the service, a Convair CV-240-0, registration N94234, [2] crashed into a muddy field on approach to land at Springfield-Branson Regional Airport near Springfield, Missouri, killing 13 of the 35 aboard (11 passengers, 2 crew members), and injuring the other 22 on board. [3] Of the three crew members, the pilot survived, and the co-pilot and stewardess did not. [4]
The pilot of the flight, Jack Pripish, suffered considerable injuries including the loss of his right eye, and was in a coma for some time after the crash. Almost two weeks after the crash, Pripish told reporters he could remember nothing of it. [3]
Findings of fact regarding the crash were published by the Civil Aeronautics Board on September 22, 1955. [5] The board determined that a complete instrument landing approach into Springfield-Branson was not made, as would have been required based on the deteriorating weather conditions at the time. Evidence showed that the crew didn't seem to be aware of the aircraft's altitude and that the aircraft was descending. The board believed it was probable that "the pilots were devoting their attention away from their instruments and outside the cockpit, possibly toward the distant airport lights." [5]

American Airlines Flight 320 was a scheduled flight between Chicago Midway Airport and New York City's LaGuardia Airport. On February 3, 1959, the Lockheed L-188 Electra performing the flight crashed into the East River during its descent and approach to LaGuardia Airport, killing 65 of the 73 people on board. Weather conditions in the area were poor, and the aircraft descended through dense clouds and fog. As it approached the runway, it flew lower than the intended path and crashed into the icy river 4,900 feet (1,500 m) short of the runway. American Airlines had been flying the newly-developed Lockheed Electra in commercial service for only about two weeks before the crash, and the accident was the first involving the aircraft type.

Southwest Airlines Flight 1248 was a scheduled passenger flight from Baltimore, Maryland, to Chicago, Illinois, continuing on to Salt Lake City, Utah, and then to Las Vegas, Nevada. On December 8, 2005, the airplane slid off a runway at Midway Airport in Chicago while landing in a snowstorm and crashed into automobile traffic, killing a six-year-old boy.

The aircraft crashed on final approach to the Cincinnati/Northern Kentucky International Airport located in Hebron, Kentucky, United States. Only three passengers and one flight attendant survived the accident.
Eastern Air Lines Flight 212 was a controlled flight into terrain accident of a McDonnell Douglas DC-9 during approach to Charlotte Douglas International Airport in North Carolina. The accident occurred on September 11, 1974, killing 72 of the 82 people on board. The scheduled flight was from Charleston Municipal Airport to Chicago O'Hare, with an intermediate stop in Charlotte.
Western Airlines Flight 2605, nicknamed the "Night Owl", was an international scheduled passenger flight from Los Angeles, California, to Mexico City, Mexico. On October 31, 1979, at 5:42 a.m. CST (UTC−06:00), the McDonnell Douglas DC-10 used on the flight crashed at Mexico City International Airport in fog after landing on a runway that was closed for maintenance. Of the 88 occupants on board, 72 were killed, in addition to a maintenance worker who died when the plane struck his vehicle.
On July 10, 1991, a L'Express Airlines Beechcraft C99, flying as Flight 508 originating in New Orleans, and in transit from Mobile to Birmingham, crashed while attempting to make an ILS approach to Runway 5 at Birmingham Municipal Airport in Birmingham, Alabama. The plane crashed in the Fairview area near Five Points West in the Ensley neighborhood and subsequently injured four persons on the ground, as well as destroying two homes. Of the 15 occupants on board, there were 13 fatalities. The cause of the crash was attributed to the captain's decision to attempt an instrument approach into severe thunderstorms resulting in a loss of control of the airplane. To date it is the deadliest commercial aviation accident in Alabama history.

Paradise Airlines Flight 901A was a scheduled passenger flight from San Jose Municipal Airport to Tahoe Valley Airport, both within California, USA. On March 1, 1964, the Lockheed L-049 Constellation serving the flight crashed near Genoa Peak, on the eastern side of Lake Tahoe during a heavy snowstorm, killing all 85 aboard. After the crash site was located, the recovery of the wreckage and the bodies of the victims took most of a month. Crash investigators concluded that the primary cause of the accident was the pilot's decision to attempt to land at Tahoe Valley Airport when the visibility was too low due to clouds and snowstorms in the area. After aborting the landing attempt, the flight crew lost awareness of the plane's location as it flew below the minimum safe altitude in mountainous terrain. The pilot likely tried to fly through a low mountain pass in an attempt to divert to the airport in Reno, Nevada, and crashed into the left shoulder of the pass. At the time, it was the second-deadliest single-plane crash in United States history, and remains the worst accident involving the Lockheed L-049 Constellation.
Delta Air Lines Flight 723 was a flight operated by a McDonnell Douglas DC-9 twin-engine jetliner, operating as a scheduled domestic passenger flight from Burlington, Vermont, to Logan International Airport in Boston, Massachusetts, with an intermediate stop in Manchester, New Hampshire. On July 31, 1973, at 11:08 a.m., while on an instrument landing system (ILS) instrument approach into Logan in low clouds and fog, the aircraft descended below the glidepath, struck a seawall, and crashed. All 89 of the occupants aboard were killed, including an initial survivor who died more than 4 months after the crash.

Ozark Air Lines Flight 982 was a regularly scheduled flight on December 27, 1968, originating from Sioux Falls Regional Airport to O'Hare International Airport connecting through Sioux Gateway Airport that crashed shortly after takeoff. The plane struck the ground about 500 feet beyond the end of the runway and came to a stop about 1200 ft from the end of the runway. Thirty-five of the flight's 64 passengers and four crew members were taken to area hospitals, mostly for treatment of minor cuts and scratches. The flight was piloted by Capt. Patrick G. Sweeney, 48, of Wood River, IL. The copilot was John T. Schmeltz, 33, of St. Louis.
American Airlines Flight 476 was a passenger flight operated by American Airlines from Tulsa Municipal Airport in Tulsa, Oklahoma to Laguardia Airport in New York City, New York. On August 4, 1955, the Convair CV-240-0 crashed while attempting an emergency landing in Fort Leonard Wood, Missouri, killing all 30 passengers and crew. The Civil Aeronautics Board determined that the accident was caused by the installation of a damaged cylinder.

On January 6, 1957, the Convair CV-240-0 performing the flight crashed while performing an instrument approach to Tulsa Municipal Airport, killing one of the occupants and injuring seven. Weather conditions in the area were poor, and the aircraft descended through dense clouds and fog. As it approached the runway, it flew lower than the intended path and crashed into the ground 3.6 miles (5.8 km) north of the approach end of the runway. The probable cause of the crash was determined to be the captain's lack of alertness in allowing the first officer to continue an instrument descent to an altitude too low to permit terrain clearance.
