United Air Lines Flight 4 was a Boeing 247D, registration NC-13317, operating on a scheduled flight from Salt Lake City, Utah, to Cheyenne, Wyoming, on October 7, 1935. The aircraft last contacted Cheyenne at 02:16 or 02:17 local time, reporting its position as Silver Crown beacon. Cheyenne called the flight at 02:21, and no further communications were received from the accident aircraft. At 2:00 a.m. the weather at the Cheyenne airport was recorded as clear with ceiling unlimited, air temperature of 41.7F (5.4C), and dew point 37.0F (2.8C), and winds out of the northwest at 9 miles per hour (14.4 km/h).
The wreckage was located 3 mi (4.8 km) east of Silver Crown, and 7.4 mi (12 km) northwest of the Cheyenne airport, at an elevation of 6,553 ft (1997 m). Note that the elevation of the crash site is only 370 ft (113 m) higher than the airport elevation. The aircraft struck the ground in a shallow descent just below a small knoll. Marks on the ground made by the propellers, fuselage, and engine showed the aircraft was in a normal flight attitude. Propeller marks and engine damage established the engines were developing normal power and the aircraft was at cruising speed. An untouched knoll 60 ft (18 m) further back on the flight path and 3 ft (0.9 m) higher established that the aircraft was descending. The accident claimed all souls aboard, which included 9 passengers and three flight crew.
The captain of this flight was highly experienced and familiar with the topography and unique features of the approach environment. Possible factors include darkness, sleep deprivation (captain), fog (speculative). The probable cause was determined to be pilot error in failing to monitor altitude or location. The crash was written about in newspapers around the world. In modern parlance, the cause of this accident was "Controlled Flight into Terrain". Captain Collison had flown this route over 100 times. According to the accident report, navigation along the route from Salt Lake City, Utah, to Cheyenne, Wyoming, relied on tower-mounted rotating beacons, and the last radio communications by the crew indicated that they knew their position relative to the "Silver Crown" beacon located approximately 10 mi (16 km) northwest of Cheyenne, Wyoming. For some reason the crew failed to maintain sufficient altitude, and failed to notice that they were approaching the ground because of the darkness. [1] According to the accident report, the cockpit lights were set to maximum intensity at the time of the crash, which reduces visibility outside the aircraft during night flight.
The captain for this flight, Halston A. "Colly" Collison, was a very experienced pilot. He flew for the U.S. Air Mail Service in Wyoming and accumulated 3482 hours flying mail in single-engine biplanes and survived multiple forced landings. In the book "Wyoming Air Mail Pioneers", [2] Collison is referred two as "the lucky one" because of the number of forced landings he survived while flying mail around Wyoming, which is noted for its cold temperatures, strong, gusty winds and sudden snowstorms. Some of Collison's exploits flying air mail were documented in the January, 1926, issue of National Geographic in an article written by J. Parker Van Zandt, Lt. U.S. Army Air Service pilot. Collison started flying for United Airlines sometime before 1933. Collison's co-pilot for this trip was 27 year-old George E. Batty, who was known as "Jeb". Batty was from Denver, Colorado, and was an Army Air Corps veteran with a reputation for fearlessness. The flight attendant on board the flight was Miss Leona Mason, who was trained as a nurse.
Published histories include details about the passengers on the accident aircraft. This is largely due to the efforts of Kent Hargraves and Gerald Adams. [1] Passenger Vincent Butler was a prominent attorney for a law firm that represented the Standard Oil Company of California, and was traveling to Chicago. Passenger Juliet Hillman was vacationing at a dude ranch and visiting her sister who lived in Reno. She disliked travel by train, claiming that it made her ill. She preferred to travel by air. Her father was the head of the J.H. Hillman Coke & Chemical Company. Passenger John F. Cushing was a Civil Engineer and at the time of the crash served as President of the Great Lakes Dredge and Dock Co. The Cushing Hall of Engineering at Notre Dame University is named after him. Passenger George H. Miner was trained as an automobile mechanic. Having obtained a patent in 1929 for an arcade game called "All-American Automated Baseball Game" he and his father operated this amusement machine business. He was an innovator and designer in the coin machine business, and was well regarded in the industry. Passenger Robert H. Renebome, used a ticket issued to someone else to board the flight. He was a fugitive from the law [3] Passenger Helen Warren was a stenographer employed by United Airlines. Passenger Carolyn Cathcart, was the mother of a United Airlines pilot named named Daryl Cathcart. Passenger Roy Bane recently moved to Cheyenne, Wyoming, where he hoped to build a washing machine business.
Notable missing passenger: Former U.S. President Herbert Hoover gave a speech in Oakland, California, on Oct. 6, 1935. While he planned to return east by train, he instructed his assistant to reserve a seat on this flight in case his speech went long and he missed his train. Luckily for Mr. Hoover, he made it to the train station just in time and was not a passenger on United Flight 4.
American Airlines Flight 965 was a regularly scheduled flight from Miami International Airport in Miami, Florida, to Alfonso Bonilla Aragón International Airport in Cali, Colombia. On December 20, 1995, the Boeing 757-200 flying this route crashed into a mountain in Buga, Colombia, around 9:40 pm killing 151 of the 155 passengers and all eight crew members.

The de Havilland Express, also known as the de Havilland D.H.86, was a four-engined passenger aircraft manufactured by the de Havilland Aircraft Company between 1934 and 1937.
Scandinavian Airlines System Flight 751 was a regularly scheduled Scandinavian Airlines passenger flight from Stockholm, Sweden, to Warsaw, Poland, via Copenhagen, Denmark. On 27 December 1991, a McDonnell Douglas MD-81 operating the flight, registration OY-KHO, piloted by Danish Captain Stefan G. Rasmussen (44) and Swedish first officer Ulf Cedermark (34), both experienced pilots with 8,000 and 3,000 flight hours, respectively, was forced to make an emergency landing in a field near Gottröra, Sweden. Ice had collected on the wings' inner roots before takeoff, broke off, and was ingested into the engines as the aircraft became airborne on takeoff, ultimately disabling both engines. All 129 passengers and crew aboard survived.

United Air Lines Flight 409 was a scheduled flight which originated in New York City, New York. The final flight destination was San Francisco, California, with stops in Chicago, Denver and Salt Lake City. The aircraft operating the service, a Douglas DC-4 propliner, registration N30062, crashed into Medicine Bow Peak, near Laramie, Wyoming, on October 6, 1955, killing all 66 people on board. The victims included five female members of the Mormon Tabernacle Choir and military personnel. At the time, this was the deadliest airline crash in the history of American commercial aviation. Another 66 lives had been lost earlier that year in the March 22 crash in Hawaii of a United States Navy Douglas R6D-1 Liftmaster military transport aircraft, and 66 had also died in the mid-air collision of two United States Air Force C-119G Flying Boxcars over West Germany on August 11, placing the three crashes in a three-way tie as the deadliest aviation incidents in 1955.
Varig Flight 254 was a Boeing 737-241, c/n 21006/398, registration PP-VMK, on a scheduled passenger flight from São Paulo, Brazil, to Belém, capital city of the state of Pará in the country's North Region, on 3 September 1989. The flight had several intermediate stopovers, the last being in Marabá, Pará. Prior to takeoff from Marabá, the crew entered an incorrect heading into the flight computer, flying deep into a remote area of the Amazon jungle. Attempts to reach an alternative airport were unsuccessful and the plane eventually ran out of fuel, crashing during a belly landing in the jungle 1,700 kilometres northwest of Rio de Janeiro. Of the 54 passengers and crew, 12 passengers died and many more sustained serious injuries. The survivors were rescued two days later.

United Airlines Flight 173 was a scheduled flight from John F. Kennedy International Airport in New York City to Portland International Airport in Portland, Oregon, with a scheduled stop in Denver, Colorado. On December 28, 1978, the aircraft flying this route ran out of fuel while troubleshooting a landing gear problem and crashed in a suburban Portland neighborhood near NE 157th Avenue and East Burnside Street, killing 10 people on board.
Indian Airlines Flight 605 was a scheduled domestic passenger flight from Bombay to Bangalore. On 14 February 1990, an Airbus A320-231 registered as VT-EPN, crashed onto a golf course while attempting to land at Bangalore, killing 92 of 146 people on board.
The Grand Canyon mid-air collision occurred in the western United States on June 30, 1956, when a United Airlines Douglas DC-7 struck a Trans World Airlines Lockheed L-1049 Super Constellation over Grand Canyon National Park, Arizona. The first plane fell into the canyon while the other slammed into a rock face. All 128 on board both airplanes perished, making it the first commercial airline incident to exceed one hundred fatalities. The airplanes had departed Los Angeles International Airport minutes apart from each other and headed for Chicago and Kansas City, respectively. The collision took place in uncontrolled airspace, where it was the pilots' responsibility to maintain separation. This highlighted the antiquated state of air traffic control, which became the focus of major aviation reforms.

Dan-Air Flight 1008 was a fatal accident involving a Boeing 727-46 jet aircraft operated by Dan Air Services Limited on an unscheduled international passenger service from Manchester to Tenerife. The accident occurred on 25 April 1980 in a forest on Tenerife's Mount La Esperanza when the aircraft's flight deck crew wrongly executed an unpublished holding pattern in an area of very high ground; it resulted in the aircraft's destruction and the deaths of all 146 on board. Flight 1008 was Dan-Air's second major accident in ten years and the worst accident involving the deaths of fare-paying passengers in the airline's entire history.
Invicta International Airlines Flight 435 (IM435) was a Vickers Vanguard 952, flying from Bristol Lulsgate to Basel-Mulhouse, which crashed into a forested hillside near Hochwald, Switzerland on 10 April 1973. The aircraft somersaulted and broke up, killing 108 people, with 37 survivors. To date, this is the deadliest accident involving a Vickers Vanguard and the deadliest aviation accident to occur on Swiss soil. Many of the 139 passengers on the charter flight were women, members of the Axbridge Ladies Guild, from the Somerset towns and villages of Axbridge, Cheddar, Winscombe and Congresbury. The accident left 55 children motherless and became known in the British media as the Basle air crash.

Aviastar-TU Flight 1906 was a Tupolev Tu-204 that crashed while attempting to land at Domodedovo International Airport, Moscow, Russia, in heavy fog on 22 March 2010. The aircraft was on a ferry flight from Hurghada International Airport, Egypt to Moscow, and had no passengers on board; all eight crew survived the accident, four with serious injuries requiring hospitalization and four with minor injuries. The accident was the first hull loss of a Tu-204 and the first hull loss for Aviastar-TU.
The 1934 United Air Lines Boeing 247 crash was an accident involving a Boeing Air Transport-operated United Airlines scheduled flight of a Boeing 247, which crashed in bad weather shortly after departing Salt Lake City, Utah, on February 23, 1934, killing all eight on board. The cause was not immediately determined, but poor weather was considered a factor.
Western Air Lines Flight 34 was a scheduled flight between Los Angeles International Airport in Los Angeles and Minneapolis–Saint Paul International Airport in Minneapolis with intermediate stops in Las Vegas, Cedar City, Utah, Salt Lake City, Casper, Wyoming, and Rapid City, South Dakota. On February 26, 1954, the Convair CV-240 performing the flight crashed while flying through storms over Wyoming, killing all nine occupants of the plane. The region was experiencing severe blizzards and winds, and it took searchers three days to find the crash site. The aircraft crashed into the ground at a high rate of speed, leaving a five-foot deep impact crater in the frozen ground. The wreckage was left scattered and broken into tiny pieces. Crash investigators conducted an investigation of the events that led up to the crash, but were unable to find a clear cause. The final accident report identified the probable cause as "a sudden emergency of undetermined origin under adverse weather conditions resulting in rapid descent and impact with the ground at high speed".
