El Salvador sent a delegation to compete in the 2020 Summer Paralympics in Tokyo in Japan originally scheduled to take place in 2020 but postponed to 23 July to 8 August 2021 because of the COVID-19 pandemic. [1] This was the country's sixth successive appearance in the Summer Paralympics since debuting at the 2000 Summer Paralympics.

El Salvador first competed in the Paralympic Games at the 2000 Summer Paralympics in Sydney, Australia. It has participated in the Summer Paralympic Games every four years since that time. El Salvador has never taken part in the Winter Paralympics, and until Tokyo 2020, no Salvadorian had won a Paralympic medal. In 2021, Herbert Aceituno became the first athlete to win a medal, earning bronze in powerlifting at the 59 kg category.

Aleksandar Kukolj is a Serbian judoka. He won gold medals at the 2016 Abu Dhabi Grand Slam and the 2016 Tokyo Grand Slam. Kukolj started with bronze in the 2016 Havana Grand Prix and silvers at the 2014 Zagreb Grand Prix, 2015 Samsun Grand Prix and 2016 Baku Grand Slam. Kukolj won World Cups in Lisbon in 2012 and won the Panam Open in San Salvador in 2014 and the 2013 Mediterranean Games. He won European medals as Junior and U23. Kukolj competed at the 2016 Summer Olympics in Rio de Janeiro, in the men's 90 kg.
El Salvador sent a delegation to compete in the 2016 Summer Paralympics in Rio de Janeiro in Brazil from 7 to 18 September 2016. This was the country's fifth successive appearance in the Summer Paralympics since debuting at the 2000 Summer Paralympics. The Salvadoran delegation to Rio consisted of one athlete, powerlifter Herbert Aceituno, who qualified for the Games through his world ranking of 13th and his participation was confirmed by the International Paralympic Committee in August 2016. He failed to lift 185 kilograms (408 lb) in three attempts during the men's 72 kg tournament and was therefore not ranked in the final standings by the judges.

Celina Márquez is a Salvadoran swimmer. She competed in the women's 50 metre backstroke event at the 2017 World Aquatics Championships. She competed at the 2020 Summer Olympics. She and compatriot sailor Enrique Arathoon were named El Salvador's flag bearers for the 2020 Olympic Games opening ceremony in Tokyo.
