Middletown was a hamlet in the Town of Orangetown, Rockland County, New York, United States, located north of Nauraushaun; east of the state of New Jersey; south of Nanuet and west of Blauvelt.
In the early 1870s, Pearl River was divided into five different sections: Sickletown, Nauraushaun, Middletown, Pascack and Muddy Brook.

New York State Route 17M (NY 17M) is an east–west state highway in Orange County, New York, in the United States. It extends for 26.63 miles (42.86 km) from west of the city of Middletown to what is currently the north–south section of NY 17 just southeast of the village of Harriman. It is a busy main street in Middletown and the village of Monroe; in the former, it divides into a parkway for several blocks and forms the city's major commercial strip, located between the downtown district and an interchange with Interstate 84 (I-84). The rest of the road is a two-lane rural route. Between New Hampton and Goshen, the highway overlaps with U.S. Route 6 (US 6). The easternmost section of that overlap near Goshen is routed on the Quickway, making a three-route concurrency with NY 17.

New York State Route 206 (NY 206) is a 74.57-mile-long (120.01 km) state highway in the Southern Tier of New York in the United States. It runs through some lightly populated regions along the state's southern border, from Central New York to the Catskills. It begins near a busy intersection with Interstate 81 (I-81) at Whitney Point and runs east from there through Greene. The eastern terminus is located at a junction with NY 17 at Roscoe in Sullivan County. It is one of the longest three-digit routes in New York, and the only long one not associated with a two-digit route or a former U.S. Route. Yet due to its location it sees little traffic, although for much of its length it follows the route of a main 19th century thoroughfare, the Catskill Turnpike. It is primarily a long shortcut around Binghamton.
New York State Route 59 (NY 59) is an east–west state highway in southern Rockland County, New York, in the United States. The route extends for 14.08 miles (22.66 km) from NY 17 in Hillburn to U.S. Route 9W (US 9W) in Nyack. In Suffern, it has a concurrency with US 202 for 0.05 miles (0.08 km). NY 59 runs parallel to the New York State Thruway its entire route. The routing of NY 59 became a state highway in 1911 and was signed as NY 59 in the late 1920s.
New York State Route 304 (NY 304), also known as "Rockland County Clerk Paul Piperato Memorial Highway", as well as Pearl Street for its first half a mile and Main Street for other parts, is a north–south state highway located in central Rockland County, New York, in the United States. The 10.38-mile (16.70 km) route begins at the New Jersey–New York border in Pearl River and ends at an intersection with U.S. Route 9W (US 9W) in the community of Congers. The route is a main route through Rockland County, intersecting NY 59 and indirectly connecting to the New York State Thruway and the Palisades Interstate Parkway (PIP) in Nanuet. NY 304 has three distinct sections: a freeway that extends from Pearl River to Nanuet, an at-grade section between Nanuet and New City, and a surface bypass linking New City to Haverstraw.
County Route 33 is a 12.7 miles (20.4 km) south–north county route in the central part of Rockland County, New York. Prior to 1970, portions of CR 33 were designated as the old route of New York State Route 304. Today, CR 33 serves as an alternative route for NY 304 from Pearl River to New City where NY 304 ends at US 9W. North of New City, CR 33 serves as an alternative route for the US 9W/US 202 multiplex in northern Rockland County.

Middletown State Homeopathic Hospital was a hospital for the treatment of mental disorders located in Middletown, New York. It opened on April 20, 1874, and was the first purely homeopathic hospital for mental disorders in the United States. The hospital, which served "mentally ill patients from Orange, Sullivan and Ulster Counties". employed a number of new techniques for the treatment of mental disorders, most notably the use of baseball as a therapy.

The Hoboken International Film Festival is an annual festival that promotes domestic and international television pilots, screenplays, non-studio films. Awards, with cash prizes, are given for the best submissions. The festival was founded by Kenneth del Vecchio, who is its chairman. First held in 2005, it is now held at the Thomas Morahan Waterfront Park in Greenwood Lake, New York. It was held first at the Frank Sinatra Park in Hoboken, New Jersey and later in Orange County, New York, including most recently in Middletown, New York.
