The Oregon Health Plan is Oregon's state Medicaid program. It is overseen by the Oregon Health Authority.
The Oregon Health Plan was conceived and realized in 1993 by emergency room doctor (and subsequent Oregon governor) John Kitzhaber, then a state senator, [1] and Dr. Ralph Crawshaw, a Portland activist. [2] It was intended to make health care more available to the working poor, while rationing benefits. [1] During the 1990s, Oregon was considered a national leader in health care reform. [3]
The law passed in Oregon was not initially compatible with federal law, so a waiver was needed. President Bill Clinton approved the plan on March 20, 1993, though he required a revision to the plan due to a concern about whether disabled people would have equal access. [4] At the time, Medicaid covered 240,000 Oregonians. [4] In 1994, the plan's first year of operation, nearly 120,000 new members enrolled, and bad debts at Portland hospitals dropped 16%. [1]
The cost of the Oregon Health Plan increased from $1.33 billion in 1993–1995 to $2.36 billion in 1999–2001, leading to budget-tightening measures in the early 2000s; [1] significant cuts would follow in 2003. [5]
New enrollment in the program was closed from mid-2004 [6] until early 2008, when a lottery-based system was introduced. Tens of thousands of Oregonians signed up, competing for 3,000 openings. [7] [8] In 2008, the Oregon Health Authority began implementing changes to Medicaid policy to increase screening rates and provide more access to services for young children at risk of developmental disorders. Some of these changes included revising certification requirements for patient-centered medical home (PCMH) status to include developmental screenings for children at the 9-month, 18-month, and 30-month well-child visits. The Oregon Health Plan was expanded to cover 80,000 uninsured children through legislation that passed in 2009. [10] The program has enrolled 38,000 additional children through February, 2010. [11] As of 2012, there are more than 100,000 children enrolled.
Oregon's proposed Oregon Health Plan (Medicaid) transformation was initiated by Oregon House Bill 3650, which was passed by the state Legislature with bi-partisan support in June 2011. In March, Oregon Senate Bill 1580 was signed into law by Governor John Kitzhaber establishing Coordinated Care Organizations, which will focus on prevention and coordination of physical, mental and dental care for Oregon Health Plan clients.
Starting in 2012, Oregon Health Plan clients have a new type of health plan called Coordinated Care Organizations or CCOs. A CCO is a network of all types of health care providers who are working together for people who receive health care coverage under the Oregon Health Plan. CCOs integrate physical, mental and eventually dental care for better health, better care and lower costs. CCOs focus on prevention of illness and disease and improving care to keep patients healthy and to manage existing health conditions.
The legal foundation for the OHP is generally spelled out in Chapter 414 of the Oregon Revised Statutes. [12]
Basic eligibility requires that the applicant be a resident of Oregon, as a citizen or otherwise. The level of coverage is based on income, age, mental and physical condition.
In 2014, the state implemented the Affordable Care Act's Medicaid expansion, folding previous OHP eligibility requirements into a single income requirement; up to 138% of the federal poverty level. [13] By December 2014, enrollment in Oregon's Medicaid and CHIP programs had increased to 1,030,940 people (26% of the state population). [14]
Since a February 2003 adjustment to the Oregon Health Plan, [6] it consists of two main plans, OHP Plus and OHP Standard.
OHP Plus is a full benefit package offered to children and adults who are eligible for Medicaid or for the Children's Health Insurance Program. The OHP Plus package has no premiums, but some adults may be required to pay small copayments for outpatient services and some prescription drugs.
In January 2010, most vision and some dental benefits were cut from OHP Plus due to budget deficits with the benefits partially restored later in January 2014.
OHP Standard is a limited benefit package covering a limited number of uninsured adults who are not eligible for Medicaid. In 2003, when OHP Standard began requiring small premiums of most adult participants, around 40,000 Oregonians (many homeless, destitute or mentally ill) were unable to pay the premium and were disenrolled from the program. [15] Significant cuts were made to the Oregon Health Plan's budget in 2003. Today, the monthly premiums are still required, but there are no copayments. [16]
Effective January 2014, the OHP Standard package was transitioned into OHP Plus as part of the state's implementation of the federal Patient Protection and Affordable Care Act. [17]
As a Medicaid managed care system, the plan has contracts with a number of private and nonprofit companies who provide care for a capitated fee. [18] In 2012, a Section 1115 Medicaid waiver designed a new type of plan called Coordinated Care Organizations, which included some of the preexisting organizations such as CareOregon, PacificSource, and FamilyCare as well as independent practice associations. [19]
In 2017, the second-largest Medicaid insurer, FamilyCare, shut down; [20] its 100k Portland-area will largely be absorbed by another Medicaid insurer, Health Share. [21] Health Share is related to CareOregon, which absorbed 80k members. [22]
From 2016 to 2018, CareOregon, which was founded in 1994 by providers with the Oregon Primary Care Association, Oregon Health Sciences University, and Multnomah County Health Department, [23] lost about $96m. [22] Various other plans showed losses as well. [24]
The Oregon Health Plan became the focus of national scrutiny in 2003, after deep budget cuts led to 100,000 people in mental health and/or substance abuse treatment losing prescription coverage under the program. [25]
During 2008 and 2009, the Oregon Health Plan stirred up controversy when enforcing 1994 guidelines [26] to only cover comfort care, and not to cover cancer treatment such as chemotherapy, surgery and radiotherapy for patients with less than a 5% chance of survival over five years. [27]
Springfield resident Barbara Wagner said her oncologist prescribed the chemotherapy drug Tarceva for her lung cancer, but that Oregon Health Plan officials sent her a letter declining coverage for the drug, and informing her that they will only pay for palliative care and physician-assisted dying. She appealed the denial twice, but lost both times. [28] Tarceva drugmaker Genentech agreed to supply her the $4,000-a-month [29] drug free of charge. [30] Wagner's plight garnered a flurry of attention from the media, [31] the internet, [32] [33] [34] [35] and triggered protest from religious groups. [36]
On August 26, 2008, former Oregon Governor John Kitzhaber and Somnath Saha, a Portland physician and then-chairwoman of the Oregon Health Services Commission, issued an op-ed [37] in The Oregonian newspaper in response to the twisting of Wagner's story in the media. According to Kitzhaber and Saha, the Oregon Health Plan covers nearly all chemotherapy prescribed for cancer patients, including the multiple rounds of chemotherapy that Wagner received; Wagner's request for second-line treatment was denied because of the drug's limited benefit and very high cost, which would have led to the denial of access to well-established, first-line treatments for other cancer patients. Kitzhaber and Saha stated that "the idea that treatment has ever been denied because death would be more 'cost effective'...is both abhorrent and a blatant distortion of the facts. [T]he fact that [physician aid in dying] is covered by the health plan has no bearing on the decision in this or any other case."
Wagner died in October 2008, three weeks after starting Tarceva. [38]
Following the end of two terms as Governor of Oregon, Kitzhaber established the Archimedes Movement, which aims to be a grassroots effort toward crafting legislation and solving Oregon's health care problems. The Archimedes Movement also has a close relationship with the Foundation for Medical Excellence. [39] [40]
The 2007 Oregon legislative session passed the Healthy Oregon Act (Senate Bill 329), which established the Oregon Health Fund Board. [41] This seven member advisory panel worked with former Governor Ted Kulongoski to propose legislation for the 2009 session. Among other challenges, the board has been advised that changes in federal requirements will affect funds that currently support 24,000 Oregonians on the OHP Standard plan. [42]
In June 2011, the Oregon Legislature with bi-partisan support created Medicaid (Oregon Health Plan) transformation with House Bill 3650. The goal of this legislation is to develop a health care delivery system managed by Coordinated Care Organization for Oregon Health Plan clients that are community-based, and have the flexibility to deliver better care, better health at lower costs. In March, 2012, Oregon Senate Bill 1580 was signed into law by Governor John Kitzhaber establishing Coordinated Care Organizations for Oregon Health Plan clients.
In August 2012, the first Coordinated Care Organizations opened to serve Oregon Health Plan clients.
Theodore Ralph Kulongoski is an American politician, judge, and lawyer who served as the 36th Governor of Oregon from 2003 to 2011. A member of the Democratic Party, he served in both houses of the Oregon Legislative Assembly and also served as the state Insurance Commissioner. He was the Attorney General of Oregon from 1993 to 1997 and a justice of the Oregon Supreme Court from 1997 to 2001. Kulongoski has served in all three branches of the Oregon state government.
John Albert Kitzhaber is an American former politician and physician who served as governor of Oregon from 1995 to 2003 and again from 2011 to 2015. In February 2015, shortly after beginning his fourth term, Kitzhaber resigned from office and was replaced by Secretary of State Kate Brown. A member of the Democratic Party, Kitzhaber was the longest-serving governor in the state's history.
Oregon Health & Science University (OHSU) is a public research university focusing primarily on health sciences with a main campus, including two hospitals, in Portland, Oregon. The institution was founded in 1887 as the University of Oregon Medical Department and later became the University of Oregon Medical School. In 1974, the campus became an independent, self-governed institution called the University of Oregon Health Sciences Center, combining state dentistry, medicine, nursing, and public health programs into a single center. It was renamed Oregon Health Sciences University in 1981 and took its current name in 2001, as part of a merger with the Oregon Graduate Institute (OGI), in Hillsboro. The university has several partnership programs including a joint PharmD Pharmacy program with Oregon State University in Corvallis.
Vicki Lynn Walker is an American politician. She served as the Oregon State Director for Rural Development with the U.S. Department of Agriculture. A member of the Democratic Party, Walker previously served in both houses of the Oregon Legislature and briefly served as chair of the state's parole board. Walker is known as an advocate for utility customers and as an outspoken critic of influential former Governor Neil Goldschmidt.
Oregon State Hospital is a public psychiatric hospital in the U.S. state of Oregon, located in the state's capital city of Salem with a smaller satellite campus in Junction City opened in 2014. Founded in 1862 and constructed in the Kirkbride Plan design in 1883, it is the oldest operating psychiatric hospital in the state of Oregon, and one of the oldest continuously operated hospitals on the West Coast.
Mary Therese Nolan is a Democratic politician from the U.S. state of Oregon. She represented District 36 in the Oregon House of Representatives from 2001 to 2013, and served as the majority leader from 2008 to 2010. She ran unsuccessfully for the Portland City Council in 2012. She came back into office in 2020, when she was elected as one of the councilors for the Portland-area regional government, Metro.
The 2002 Oregon gubernatorial election took place on November 5, 2002. Incumbent Democratic Governor of Oregon John Kitzhaber was unable to seek a third consecutive term as governor, therefore creating an open seat. To replace him, former Oregon Supreme Court Associate Justice Ted Kulongoski won a crowded and competitive Democratic primary, while former State Representative Kevin Mannix emerged from an equally competitive Republican primary. The campaign between Kulongoski and Mannix, who were joined by Libertarian nominee Tom Cox, was close and went down to the wire. Ultimately, Kulongoski eked out a narrow margin of victory over Mannix, which was slightly smaller than Cox's total vote share, allowing Kulongoski to win what would be the first of two terms as governor. As of 2023, this is the last time that Oregon voted for a gubernatorial nominee and a U.S. Senate nominee of different political parties.
In the first decade of the 2000s, the Confederated Tribes of Warm Springs sought to build a casino in the Columbia River Gorge. They ended their pursuit of the project in 2013. They considered various sites, as early as 1999; the most extensive plan called for a 60-acre (24 ha) facility with 250 hotel rooms in Cascade Locks, Oregon. The proposed site is within the Columbia River Gorge National Scenic Area, and adjacent to a federally designated wilderness area, but within the city limits of Cascade Locks..
The Seventy-fourth Oregon Legislative Assembly was the Oregon Legislative Assembly (OLA)'s period from 2007 to 2008. There was a regular session in 2007, and a shorter special session in 2008.
Willamette Valley Medical Center is a for-profit Level III acute care hospital in McMinnville, Oregon, United States, adjacent to the McMinnville Airport on Oregon Route 18. Opened at a different location as McMinnville Community Hospital, the four-story medical center has 60 licensed hospital beds. It is owned by Lifepoint.
We Can Do Better is a grassroots movement based in the U.S. state of Oregon, which aims to bring citizens and stakeholders together to design a health care system that "works better for everyone." It was founded by former Oregon governor John Kitzhaber, M.D. in January 2006. Kitzhaber's goal was to find a way to put money used for health care, which as of 2006 amounted to about $6.3 billion in Oregon, to better use. That year, it was one of three organizations that drew substantially increased funding levels from the Northwest Health Foundation; its grant funds totaled $82,000.
The 2010 Oregon gubernatorial election was held on Tuesday, November 2, 2010 to elect the Governor of Oregon to a four-year term beginning on January 10, 2011. The incumbent governor, Democrat Ted Kulongoski, was ineligible to run due to term limits barring him from being elected to more than two consecutive terms.
Healthcare rationing in the United States exists in various forms. Access to private health insurance is rationed on price and ability to pay. Those unable to afford a health insurance policy are unable to acquire a private plan except by employer-provided and other job-attached coverage, and insurance companies sometimes pre-screen applicants for pre-existing medical conditions. Applicants with such conditions may be declined cover or pay higher premiums and/or have extra conditions imposed such as a waiting period.
The Oregon Health Authority (OHA) is a government agency in the U.S. state of Oregon. It was established by the passage of Oregon House Bill 2009 by the 75th Oregon Legislative Assembly, and split off from Oregon Department of Human Services, OHA oversees most of Oregon's health-related programs including behavioral health, public health, Oregon State Hospital for individuals requiring secure residential psychiatric care, and the state's Medicaid program called the Oregon Health Plan. Its policy work is overseen by the nine member Oregon Health Policy Board.
Lesbian, gay, bisexual, and transgender (LGBT) people in the U.S. state of Oregon have the same legal rights as non-LGBT people. Same-sex sexual activity is legal in Oregon, and same-sex marriage has been legal in the state since May 2014 when a federal judge declared the state's ban on such marriages unconstitutional. Previously, same-sex couples could only access domestic partnerships, which guaranteed most of the rights of marriage. Additionally, same-sex couples are allowed to jointly adopt, and discrimination based on sexual orientation and gender identity in the areas of employment, housing and public accommodations is outlawed in the state under the Oregon Equality Act, enacted in 2008. Conversion therapy on minors is also illegal.
Cover Oregon was the health insurance marketplace for the U.S. state of Oregon, established as Oregon's implementation of the Patient Protection and Affordable Care Act. Cover Oregon was intended to enable Oregonians and small businesses to purchase health insurance at federally subsidized rates.
The 2014 Oregon gubernatorial election was held on November 4, 2014, to elect the Governor of Oregon, concurrently with other elections in Oregon and across the United States.
The Oregon health insurance experiment was a research study looking at the effects of the 2008 Medicaid expansion in the U.S. state of Oregon, which occurred based on lottery drawings from a waiting list and thus offered an opportunity to conduct a randomized experiment by comparing a control group of lottery losers to a treatment group of winners, who were eligible to apply for enrollment in the Medicaid expansion program after previously being uninsured.
Oregon Ballot Measure 44 was brought to the November, 1996 general election in the U.S. state of Oregon by the initiative process. It passed with 55.9% of the vote. With strong backing from Governor John Kitzhaber, PeaceHealth, a hospital in Eugene, teamed with the Oregon Association of Hospital and Health Systems (OAHHS) to bring about the initiative. Measure 44 would raise tobacco taxes in the state. The primary purpose for raising tobacco taxes was to raise money to fund the Oregon Health Plan (OHP). Additionally some of the money from the tobacco tax would go towards programs to prevent tobacco use and educate people about the harmful effects of tobacco, primarily aimed at children. Measure 44 became highly politicized on both sides.
A coordinated care organization (CCO) is a community based, integrated care organization created by the state of Oregon to allow for local and regional distribution and coordination of healthcare to segments of the state's population covered under the Oregon Health Plan.
