Federal Express Flight 1406 was an American domestic cargo flight from Memphis International Airport, Memphis, Tennessee, to Logan International Airport in Boston, Massachusetts, that suffered an in-flight cargo fire over New York on September 5, 1996. The three crew members and two passengers on board successfully evacuated after an emergency landing at Stewart International Airport in Newburgh, New York. After the evacuation, the DC-10 was consumed by fire. After an extensive investigation, the National Transportation Safety Board (NTSB) couldn't determine what caused the fire. Nevertheless, the Federal Aviation Administration (FAA) made recommendations to prevent similar incidents from occurring in the future.
The aircraft involved, manufactured in 1975, was a McDonnell Douglas DC-10-10CF registered as N68055. It had logged onto more than 38271 airframe hours and 17818 take-off and landing cycles and was equipped with three General Electric CF6-6D engines. [1] [2] : 24–25
There were three crew members and two passengers on Flight 1406. The Captain (47), had 12,344 flight hours; the First Officer (41), had 6,535 flight hours; and the Flight Engineer (45) had 3,704 flight hours. Also on board were two FedEx employees (including Vietnam veteran and Eastern Air Lines pilot Frederick S. Olmsted Jr.) sitting in jumpseats. [2] : 21–23
Flight 1406 departed from Memphis International Airport at 3:42am, en route to Logan International Airport, Boston, Massachusetts, with an estimated time of arrival (ETA) of 7:42am. [3] [2]
At 5:42am, Flight 1406 was cruising at 33,000 ft (10,000 m) above the state of New York when the Cabin Smoke Fire Alarm sounded in the cockpit. Smoke detectors for several zones of the aircraft's main deck cargo compartment alerted the pilots of suspected smoke, prompting the crew members and passengers to don their smoke masks. Aircraft systems began to fail, and the crew noticed smoke entering the cockpit. The crew informed Boston Air Traffic Control (ATC) about the fire situation; ATC suggested that Flight 1406 could make an emergency landing at Albany County Airport, 50 mi (80 km) ahead, or to land at Stewart International Airport in Newburgh, 25 mi (40 km) behind. The crew decided to land at Stewart.
At 5:49am the aircraft was on approach to Stewart International Airport, where airport fire services prepared for the DC-10 to land on Runway 27. [4] The aircraft successfully landed at the airport at 5:54am and turned onto a taxiway, where it came to a stop for the fire services to engage. The crew and the employees tried to evacuate the aircraft, but the doors and cockpit windows could not be opened because the fuselage was still pressurized. [4] The captain then de-pressurized the aircraft and all occupants then exited safely; the flight crew through a cockpit window and the passengers through one of the forward fuselage cabin doors. The fire crews found the cargo prevented them from accessing the source of the smoke from inside the cabin and forty minutes after the aircraft landed the fire burned through the fuselage skin; the fire was extinguished four hours after the aircraft landed. [5] All five people on board survived, though two members of the flight crew received minor injuries.
Because the fire damage was extensive, the National Transportation Safety Board (NTSB) investigation was unable to find a source of ignition. On 22 July 1998, the NTSB released its report, concluding, "the probable cause of this accident was an in-flight cargo fire of undetermined origin". [4]
The McDonnell Douglas DC-10 is an American trijet wide-body aircraft manufactured by McDonnell Douglas. The DC-10 was intended to succeed the DC-8 for long-range flights. It first flew on August 29, 1970; it was introduced on August 5, 1971, by American Airlines.
Turkish Airlines Flight 981 (TK981/THY981) was a scheduled flight from Istanbul Yeşilköy Airport to London Heathrow Airport, with an intermediate stop at Orly Airport in Paris. On 3 March 1974, the McDonnell Douglas DC-10 operating the flight crashed into the Ermenonville Forest, 37.76 kilometres (23.46 mi) outside Paris, killing all 335 passengers and 11 crew on board. It was the first aviation accident to exceed 200 and 300 fatalities. The crash was also known as the Ermenonville air disaster.
ValuJet Airlines Flight 592 was a regularly scheduled flight from Miami to Atlanta in the United States. On May 11, 1996, the ValuJet Airlines McDonnell Douglas DC-9 operating the route crashed into the Florida Everglades about ten minutes after departing Miami as a result of a fire in the cargo compartment caused by mislabeled and improperly stored hazardous cargo. All 110 people on board were killed.
Saudia Flight 163 was a scheduled Saudia passenger flight departing from Quaid-e-Azam Airport in Karachi, Pakistan, bound for Kandara Airport in Jeddah, Saudi Arabia, via Riyadh International Airport in Riyadh, Saudi Arabia, which caught fire after takeoff from Riyadh International Airport on 19 August 1980. Although the Lockheed L-1011-200 TriStar made a successful emergency landing at Riyadh, the flight crew failed to perform an emergency evacuation of the airplane, leading to the deaths of all 287 passengers and 14 crew on board the aircraft from smoke inhalation.

American Airlines Flight 96 (AA96/AAL96) was a regular domestic flight operated by American Airlines from Los Angeles to New York via Detroit and Buffalo. On June 12, 1972, the left rear cargo door of the McDonnell Douglas DC-10-10 operating the flight blew open and broke off above Windsor, Ontario, after takeoff from Detroit, Michigan; the accident is thus sometimes referred to as the Windsor incident, although according to the NTSB it is an accident, not an incident.
United Airlines Flight 811 was a regularly scheduled international flight from Los Angeles to Sydney, with intermediate stops at Honolulu and Auckland. On February 24, 1989, the Boeing 747-122 serving the flight experienced a cargo-door failure in flight shortly after leaving Honolulu. The resulting explosive decompression blew out several rows of seats, killing nine passengers. The aircraft returned to Honolulu and landed without further incident.
Eastern Air Lines Flight 212 was a controlled flight into terrain accident of a McDonnell Douglas DC-9 during approach to Charlotte Douglas International Airport in North Carolina. The accident occurred on September 11, 1974, killing 72 of the 82 people on board. The scheduled flight was from Charleston Municipal Airport to Chicago O'Hare, with an intermediate stop in Charlotte.
FedEx Express Flight 647 was a flight between Metropolitan Oakland International Airport, Oakland, California and Memphis International Airport, Memphis, Tennessee in the United States, that crashed during landing on December 18, 2003.
FedEx Express Flight 14 was a scheduled cargo flight from Singapore to Newark, New Jersey, via Malaysia, Taiwan, and Alaska. On July 31, 1997, the aircraft flying this route crashed during landing on its final segment at Newark International Airport (EWR), inverting and catching fire, injuring all five people on board.
The 1994 St. Louis Airport collision occurred when TWA Flight 427, operated using a McDonnell Douglas MD-82, struck a Cessna 441 Conquest II during its take-off roll, killing both of its occupants. The collision took place on November 22, 1994, at St. Louis Lambert International Airport (STL) in Bridgeton, Missouri.
On December 3, 1990, two Northwest Airlines jetliners collided at Detroit Metropolitan Wayne County Airport. Flight 1482, a scheduled Douglas DC-9-14 operating from Detroit to Pittsburgh International Airport, taxied by mistake onto an active runway in dense fog and was hit by a departing Boeing 727 operating as Flight 299 to Memphis International Airport. One member of the crew and seven passengers of the DC-9 were killed.
Aeroflot Flight 2306 was a scheduled domestic passenger flight from Vorkuta to Moscow in the Soviet Union, with a stopover in Syktyvkar. The Tupolev Tu-134 operated by Aeroflot crashed on 2 July 1986 during an emergency landing after it departed Syktyvkar, killing 54 of 92 passengers and crew on board.
Delta Air Lines Flight 1288 was a regularly scheduled flight from Pensacola, Florida to Atlanta, Georgia. On July 6, 1996, the aircraft serving the flight, a McDonnell Douglas MD-88, was on takeoff roll from Runway 17 at Pensacola when it experienced an uncontained, catastrophic turbine engine failure that caused debris from the front compressor hub of the left engine to penetrate the left aft fuselage. The cause of the engine failure was found to have been a fault in the manufacture of the fan. The failure of the airline to spot the resulting crack in the blade was a contributing factor.
On October 28, 2016, FedEx Express Flight 910, a McDonnell Douglas MD-10-10F flying from Memphis International Airport to Fort Lauderdale–Hollywood International Airport was involved in a runway skid after a landing gear collapse, which resulted in a fire completely destroying the left engine and wing. Two crew members, the only people on board, were unharmed.
Pilgrim Airlines Flight 458 was a scheduled United States passenger air commuter flight from LaGuardia Airport in New York City to Logan Airport in Boston, Massachusetts, with stopovers in Bridgeport, New Haven, and Groton, Connecticut. On February 21, 1982, the de Havilland Canada DHC-6-100 operating the flight made a forced landing on the frozen Scituate Reservoir near Providence, Rhode Island, after a fire erupted in the cockpit and cabin due to leakage of flammable windshield washer/deicing fluid. One passenger was unable to escape the aircraft and died of smoke inhalation, and eight of the remaining nine passengers, as well as both crew members, received serious injuries from the fire and crash-landing.
Overseas National Airways (ONA) Flight 032 was a non-scheduled positioning flight operated by Overseas National Airways with a McDonnell Douglas DC-10-30CF. On November 12, 1975, the flight crew initiated a rejected takeoff after accelerating through a large flock of gulls at John F. Kennedy International Airport, resulting in a runway excursion. Of the 139 aircraft occupants, all survived, while the aircraft was destroyed by an intense post-crash fire. The National Transportation Safety Board concluded that the probable cause of the accident was bird ingestion into the right-hand engine, causing an uncontained engine failure that ruptured several landing gear tires and disabled the engine's hydraulic system, in turn partially disabling the spoilers and the landing gear brakes. Contributing to the accident was the resultant failure of the affected engine's thrust reverser and the wet runway. The accident aircraft is claimed to be the largest commercial airliner ever destroyed due to a bird strike.
American Airlines Flight 102 was a regularly scheduled flight operated by a McDonnell Douglas DC-10 from Honolulu International Airport to Dallas/Fort Worth International Airport. On 14 April 1993, upon landing, it was raining at Dallas-Ft Worth International Airport, and there were numerous thunderstorms in the area. Shortly after touchdown on runway 17L, the pilot lost directional control as the aircraft began to weathervane, and it departed the right side of the runway. All 202 occupants on board survived, with two passengers suffering serious injuries during the emergency evacuation. The aircraft was badly damaged and was written off.
In aviation, an in-flight fire is a type of aviation accident where an aircraft catches on fire in-flight. They are considered one of the most dangerous hazards in aviation, with a report from the British Civil Aviation Authority showing that after a fire on an aircraft starts, flight crews only have on average 17 minutes to land their aircraft before it becomes uncontrollable. Between 1981 and 1990, approximately 20% of all fatalities on US airlines were caused by in-flight fires.
