Garuda Indonesia Flight 865 (GA865/GIA865) was a scheduled international flight from Fukuoka, Japan, to Jakarta, Indonesia via Bali, Indonesia. [2] On 13 June 1996, Flight 865 crashed on takeoff from runway 16 at Fukuoka Airport. Out of the 275 occupants on board, 3 were killed.
Flight 865 was cleared for takeoff from Runway 16. Suddenly, the crew of the McDonnell Douglas DC-10-30, Captain Ronald Longdong (38), First Officer Yudhia Putra (31) and Flight Engineer Dwi Prayitno (34), attempted to abort take-off after the failure of the number 3 (right) engine. The abort occurred at speeds nearing V2, and after rotation of the nose. Following the abort, attempts were made to stop the aircraft on the runway by use of brakes, ground spoilers and thrust reversers, but the crew was unable to stop the aircraft within the boundaries of the runway, which exited the airport premises. [2] The captain stated that he feared that the aircraft might hit buildings or objects if he did not abort the takeoff. [3]
In slowing down, the aircraft slid through a ditch, a fence and a road before finally coming to rest approximately 620 metres (2,030 ft) beyond the runway threshold. Damage done to the aircraft during the slide across the ground caused the landing gear to break off and both wing-mounted engines to be torn from the wings. The fuselage broke in two places, at about the wing root trailing edge, and at approximately 10.4 metres (34 ft) aft of the wing root trailing edge. The resultant fire destroyed the areas between the hull fractures, and other areas of the aircraft. The bodies of 3 men were later discovered in the back of the wreckage of the plane. Two of the passengers in seats 34K and 35K died as a result of the violent impact; the third fatality, seated in 35J was rendered unconscious by the impact and was unable to escape the spreading inferno. The final report concluded that pilot error and the failure of the maintenance and flight operation sections of the airline in properly coordinating matters resulted in the accident. [4]
The aircraft involved was a McDonnell Douglas DC-10-30, registration PK-GIE. It had its first flight on 24 April 1979 and was delivered to Garuda Indonesia on 27 July 1979. The aircraft was 17 years old at the time of the accident; it was the 284th DC-10 built and its MSN number was 46685. [5] The aircraft had three General Electric CF6-50C2 turbofan engines. [6] The cause of the engine failure leading to the crash was that the engine turbine blades had been in service for 6,182 cycles (take-offs and landings) when General Electric recommended discarding the blades after 6,000 cycles. [7]
The former aircraft registration number (PK-GIE) was later used on one of Garuda Indonesia's Boeing 777-300ERs. [8]

PT Merpati Nusantara Airlines, operating as Merpati Nusantara Airlines, was an airline in Indonesia based in Central Jakarta, Jakarta. It operated scheduled domestic services to more than 25 destinations in Indonesia, as well as scheduled international services to East Timor and Malaysia. The word merpati is Indonesian for "dove", and Nusantara is a Javanese word found in the Pararaton meaning "the outer islands", referring to the Indonesian archipelago. The airline was based at Soekarno-Hatta International Airport, Jakarta. It also maintained both a maintenance and simulator facility at Juanda International Airport, Surabaya. Lion Air Flight 538 (JT538/LNI538) was a scheduled domestic passenger flight from Soekarno–Hatta International Airport, Jakarta, to Juanda International Airport in Surabaya, with a stopover at Adi Sumarmo Airport, Surakarta, Indonesia. On 30 November 2004, the McDonnell Douglas MD-82 overran the runway of Adi Sumarmo Airport, and crashed onto a cemetery on landing; 25 people on board were killed in the crash, including the captain. At the time, the accident was Lion Air's only fatal hull loss until 2018. Investigation conducted by the Indonesian National Transportation Safety Committee concluded that the crash was caused by hydroplaning, which was aggravated by wind shear.
The McDonnell Douglas DC-9 is an American five-abreast, single-aisle aircraft designed by the Douglas Aircraft Company. It was initially produced as the Douglas DC-9 prior to August 1967, after which point the company had merged with McDonnell Aircraft to become McDonnell Douglas. Following the introduction of its first jetliner, the high-capacity DC-8, in 1959, Douglas was interested in producing an aircraft suited to smaller routes. As early as 1958, design studies were conducted; approval for the DC-9, a smaller all-new jetliner, came on April 8, 1963. The DC-9-10 first flew on February 25, 1965, and gained its type certificate on November 23, to enter service with Delta Air Lines on December 8.
Garuda Indonesia Flight 200(GA200/GIA 200) was a scheduled domestic passenger flight of a Boeing 737-400 operated by Garuda Indonesia between Jakarta and Yogyakarta, Indonesia. The aircraft overran the runway, crashed into a rice field and burst into flames while landing at Adisucipto International Airport on 7 March 2007. Twenty passengers and one flight attendant were killed. Both pilots survived, and were fired shortly after the accident occurred. It was the fifth hull-loss of a Boeing 737 in Indonesia within less than six months and was the most recent accident with fatalities involving the airline.

Spantax Flight 995 was a charter flight from Madrid–Barajas Airport to New York via Málaga Airport on September 13, 1982. When the DC-10 aircraft was rolling for take-off from Malaga, the pilot felt a strong and worsening vibration and aborted the take-off. The flight crew lost control of the aircraft and were unable to stop in the runway available and the aircraft overran the runway, hit an airfield aerial installation, losing an engine, then crossed the Malaga–Torremolinos Highway, hitting a number of vehicles before finally hitting a railway embankment and bursting into flames. An emergency evacuation of the aircraft was carried out but 50 on board died of both burns and other injuries. A further 110 people were hospitalized.
