The London Underground network carries more than a billion passengers a year. [1] It has one fatal accident for every 300 million journeys. [2] Five accidents causing passenger deaths have occurred due to train operation in nearly 80 years since the London Passenger Transport Board was formed, the last being at Moorgate in 1975; other fatalities have been due to wartime and terrorist bombings and station fires.

16 October 1986 | Bakerloo line | Kensal Green station | At Kensal Green station on 16 October 1986, a stationary southbound Bakerloo line train at Kensal Green station was struck in the rear by a Class 313 British Rail service on the Euston to Watford Junction line "The DC line". 23 passengers were injured. The driver of the class 313 passed the protecting stop signal as though it were at caution when it only showed a 'calling on' aspect. The (apparently distracted) driver of the Class 313 observed the train stop lower at the stop signal but failed to notice that the signal had only displayed a calling on aspect. A further protecting repeater signal had been temporarily removed while a retaining wall was being removed but the preceding stop signal had been modified to show a maximum 'caution' aspect for the duration. The accident report criticised the fact that the calling on aspect looked much the same as the yellow 'proceed at caution' aspect but for its position on the signal head. [9] The Euston to Watford Junction line was, at this time, signalled with an experimental system (installed in 1932–3) where a stop signal automatically changed to a calling on aspect after a 70-80 second time delay. The repeater signals, unusually, were able to show green, yellow, or red aspects but were not equipped with train stops. A driver was permitted to pass a red repeater after one minute. Although intended to overcome local signal failures, the more intensive service of the past occasionally produced the spectacle of several trains occupying the same section buffer-to-buffer. Normally, on this system, there were always at least two (and often three) red signals behind any train. Though not unique at the time of installation, the system was the sole example of its type at the time of the collision. The system has since been replaced by a Network Rail standard colour light system, work for which was actually taking place when the accident occurred. | ||

5 July 2007 | Central line | Between Bethnal Green and Mile End stations | On 5 July 2007, two cars of an eight-car westbound Central line train of 1992 stock derailed at 65 km/h between Bethnal Green and Mile End tube stations. 520 passengers were trapped below ground for two hours, until they were escorted from the derailed train by following one another along the tracks to Mile End tube station. Eight people required hospital treatment and a further thirteen were treated at the scene for minor injuries. Most of the injuries were caused while walking along the uneven surface in the tunnel. The Central line was suspended between Liverpool Street and Leytonstone until the end of the following day as a safety investigation was carried out and the derailed train was rerailed. The investigation found that the derailment was caused by a roll of fire-resistant material being blown onto the tracks from its storage place in a connecting passageway between the two tunnels. 13 August 2010 | Northern line | Archway station to Warren Street station | At around 07:00 on 13 August 2010, a broken-down maintenance wagon on the Northern line became uncoupled from the locomotive that was towing it, allowing it to roll southwards from Archway station. 30 September 2022 | Northern line | High Barnet station | On the night of 30 September 2022, Sarah de Lagarde fell asleep on a northbound Northern line, missed getting off at Camden Town, and did not wake up until the train terminated at High Barnet. Disorientated, she slipped on the wet platform and fell through the gap between cars. She was then run over by the departing train, as well as a subsequent incoming one, in the process of which she lost her right arm and right leg. Willesden Junction is an interchange station located in Harlesden, north-west London, for London Underground and London Overground services. The station also borders the London Borough of Ealing, near the Old Oak Lane conservation area in the East Acton ward.
Mile End is a London Underground station in Mile End, London. It is served by the Hammersmith & City, District and Central lines. This station features a cross-platform interchange in both directions; District and Hammersmith & City lines stop on the inside tracks, and the Central line stops on the outside tracks. It is in Travelcard Zone 2.
There have been four railway accidents at Potters Bar in England. Those in 1898 and 1946 were signals passed at danger. The accident in 2002 led to substantial public debate and a national change in policy relating to maintenance of infrastructure.
The Ufton Nervet rail crash occurred on 6 November 2004 when a passenger train collided with a stationary car on a level crossing on the Reading–Taunton line near Ufton Nervet, Berkshire, England. The collision derailed the train, and seven people—including the drivers of the train and the car—were killed. An inquest found that all railway personnel and systems were operating correctly, and the crash was caused by the suicide of the car driver.
The Selby rail crash was a railway accident that occurred on 28 February 2001 near Great Heck, Selby, North Yorkshire when a passenger train collided with a car which had crashed down a motorway embankment onto the railway line. The passenger train then collided with an oncoming freight train. Ten people died, including the drivers of the two trains, and 82 were injured. It remains the worst rail disaster of the 21st century in the United Kingdom.
The Heart of Wessex Line, also known as the Bristol to Weymouth Line, is a railway line that runs from Bristol Temple Meads to Westbury and Weymouth in England. It shares the Wessex Main Line as far as Westbury and then follows the course of the Reading to Taunton Line as far as Castle Cary.
A buffer stop, bumper, bumping post, bumper block or stopblock (US), is a device to prevent railway vehicles from going past the end of a physical section of track.
The Dagenham East rail crash was a railway accident on the London, Tilbury and Southend line of British Railways which occurred at Dagenham, Essex, United Kingdom.
The Spa Road Junction rail crash was an accident on the British railway system which occurred during the peak evening rush hour of 8 January 1999 at Spa Road Junction in Bermondsey, in South East London.
The Falls of Cruachan derailment occurred on 6 June 2010 on the West Highland Line in Scotland, when a passenger train travelling between Glasgow and Oban hit boulders on the line and derailed near Falls of Cruachan railway station, after a landslide. There was a small fire and one carriage was left in a precarious position on the 50-foot-high (15-metre) embankment. Sixty passengers were evacuated, some with minor injuries; eight of those were hospitalised as a precaution. However, no people were killed. In addition to blocking the line, the incident also caused the closure of the A85 road below the rail line. Both road and rail were closed for a week.
The Little Cornard derailment occurred on 17 August 2010 when a passenger train collided with a road vehicle on a level crossing on the Gainsborough Line near Little Cornard, Suffolk, and partly derailed. The vehicle, a tanker lorry, had begun crossing over the track when the Class 156 train from Sudbury destined for Marks Tey struck it whilst travelling at a speed of approximately 40 miles per hour (64 km/h).
The Salisbury Rail Crash was a railway accident on 31 October 2021, at Salisbury, Wiltshire, United Kingdom. Two trains, travelling on converging lines, collided at Salisbury Tunnel Junction, approximately one mile northeast of Salisbury railway station. Fourteen people, including one of the train drivers, were taken to hospital.
On 13 March 2021, a Class 507 electric multiple unit operated by Merseyrail collided with the buffer stop at Kirkby railway station, Merseyside, United Kingdom. The only injury was the driver of the train. The cause was found to be that the driver was using a mobile phone whilst driving. The distraction led him to enter the station at excessive speed. He was dismissed from Merseyrail and later prosecuted, pleading guilty to a charge of endangering the safety of people on the railway, for which he received a 12-month suspended sentence.
