This is a list of spouses of prime ministers of Japan .
They have all been women as wives of male prime ministers, and the media often refers to the spouse as the "First Lady of Japan", however as the prime minister is not head of state but head of government in Japan, this differs considerably from its equivalent in the United States.
The role of the prime minister's consort is not an official position, and so they are not given a salary or official duties.

The politics of Japan are conducted in a framework of a dominant-party bicameral parliamentary representative democratic constitutional monarchy. A hereditary monarch, currently Emperor Naruhito, serves as head of state while the Prime Minister of Japan, currently Shigeru Ishiba since 2024, serves as the elected head of government.
Junichiro Koizumi is a Japanese retired politician who served as Prime Minister of Japan and president of the Liberal Democratic Party (LDP) from 2001 to 2006. He retired from politics in 2009. He is the sixth-longest serving Prime Minister in Japanese history.
The Liberal Democratic Party, frequently abbreviated to LDP or Jimintō (自民党), is a major conservative and nationalist political party in Japan. Since its foundation in 1955, the LDP has been in power almost continuously—a period called the 1955 System—except between 1993 and 1994, and again from 2009 to 2012.
Inukai Tsuyoshi was a Japanese statesman who was prime minister of Japan from 1931 to his assassination in 1932. At the age of 76, Inukai was Japan's second oldest serving prime minister, after Kantarō Suzuki whose term ended at the age of 77.
Ichirō Hatoyama was a Japanese politician who served as Prime Minister of Japan from 1954 to 1956. During his tenure he oversaw the formation of the Liberal Democratic Party and restored official relations with the Soviet Union.
Kakuei Tanaka was a Japanese politician who served as Prime Minister of Japan from 1972 to 1974. He served in the House of Representatives from 1947 to 1990. As prime minister, Tanaka advocated large-scale infrastructure development and oversaw the normalization of diplomatic relations with China. Afterwards, he was arrested in connection to the Lockheed bribery scandals, but nevertheless exercised a dominant influence on Japanese politics until suffering a stroke in 1985.

Keizō Obuchi was a Japanese politician who served as Prime Minister of Japan from 1998 to 2000.
Ryutaro Hashimoto was a Japanese politician who served as the Prime Minister of Japan from 1996 to 1998. He was the leader of one of the largest factions within the ruling LDP through most of the 1990s and remained an influential player in Japanese politics until a controversy centered around undisclosed campaign contributions forced him to resign his leadership position in 2004. As a result of falling out of public favor, he chose not to stand in the general election of 2005, and effectively retired from politics.

Seiko Hashimoto is a Japanese politician, former speed skater and track cyclist. She has the most Olympic appearances of any Japanese athlete except Noriaki Kasai, representing her native country in four consecutive Winter Olympics from 1984 to 1994 and in three consecutive Summer Olympics from 1988 to 1996, making her a seven time Olympian. On top of her Olympic career, she is the mother of six children while working in politics and other leadership positions. She is currently a member of the House of Councillors from the Liberal Democratic Party, and served as the President of the Japan Skating Federation from 2006 to 2019.

Cho Kiyoko, better known as Takeda Kiyoko (武田清子), was a Japanese scholar of the history of ideas. In the 1950s, she contributed to the diplomacy that was hurt by World War II, aimed at restoring relations and understanding among Asian people, including Chinese, Filipino, Indian, and people of other Asian countries. She was the founder of the Social Studies Institute at the International Christian University (ICU) in Tokyo. Takeda Kiyoko was a professor emerita at ICU with a PhD in Literature from the University of Tokyo in 1961.
