Herpes meningitis is inflammation of the meninges, the protective tissues surrounding the spinal cord and brain, [1] due to infection from viruses of the Herpesviridae family [2] - the most common of which is HSV-2. [3]

Epidemiology

Aseptic meningitis, meningitis caused by pathogens other than bacteria, is the most common form of meningitis with an estimate of 70 cases per 100,000 patients less than 1 year old, 5.2 cases per 100,000 patients 1 to 14 years of age, and 7.6 cases per 100,000 adults. When looking at the most common causes of meningitis, 8.3% are due to herpes simplex virus. [4] HSV-2 specifically is the most common cause of meningitis in adults. [5]

Herpesviral meningitis primarily affects people aged 35-40, the elderly, and women. [6]

Clinical presentation

Common symptoms include nausea, vomiting, neck-stiffness, photophobia, and severe frontal headaches. [7] Patients with meningitis secondary to the HSV-2 virus may also present with genital lesions, although most cases of HSV-2 meningitis occur without symptoms of genital herpes. [8] [9] Around one fifth of people infected with HSV-2 have symptoms of meningitis with their initial infection, more commonly men than women. [10]

HSV-2 is the most common cause of Mollaret's meningitis, a type of recurrent viral meningitis. [5] This condition was first described in 1944 by French neurologist Pierre Mollaret. Recurrences usually last a few days or a few weeks, and resolve without treatment. They may recur weekly or monthly for approximately 5 years following primary infection. [11]

Diagnosis

Patient presentation of fever, headache, stiff neck, and altered mental status is not sufficient information for diagnosis. As such, lumbar puncture must be performed to properly rule out meningitis. [12] CSF findings in herpes meningitis present with lymphocytic pleocytosis, normal glucose, and normal-to-elevated protein. [4]

DNA analysis techniques such as Polymerase chain reaction is the gold standard for detection of herpes virus in patient CSF fluid due to high specificity [12] and has been able to detect the HSV-2 virus in patients presenting without genital lesions as well as those experiencing recurrent meningitis. [13]

Treatment

Although guidelines strongly recommend acyclovir for treatment of herpes encephalitis, there are currently no such guidelines for managing herpes meningitis. [14] Herpes meningitis is typically self-limiting over 2 weeks without treatment. However, empirical use of antiviral medications such as acyclovir are considered in cases of suspected HSV meningitis to prevent progression to the more rapid and fatal HSV meningoencephalitis. [15]

HSV-2 is the most common herpes virus that causes meningitis. [3] This virus is transmitted via sexual contact and there are currently no vaccinations or cures for the disease. At the moment, there are no specific programs developed to prevent HSV-2 spread and prevention of disease is primarily done via behavioral modification via condom use or through application of antiviral medications upon infection. [16]

Aseptic meningitis is the inflammation of the meninges, a membrane covering the brain and spinal cord, in patients whose cerebral spinal fluid test result is negative with routine bacterial cultures. Aseptic meningitis is caused by viruses, mycobacteria, spirochetes, fungi, medications, and cancer malignancies. The testing for both meningitis and aseptic meningitis is mostly the same. A cerebrospinal fluid sample is taken by lumbar puncture and is tested for leukocyte levels to determine if there is an infection and goes on to further testing to see what the actual cause is. The symptoms are the same for both meningitis and aseptic meningitis but the severity of the symptoms and the treatment can depend on the certain cause.

<span class="mw-page-title-main">Herpetic whitlow</span> Medical condition

A herpetic whitlow is a herpes lesion (whitlow), typically on a finger or thumb, caused by the herpes simplex virus (HSV). Occasionally infection occurs on the toes or on the nail cuticle. Herpes whitlow can be caused by infection by HSV-1 or HSV-2. HSV-1 whitlow is often contracted by health care workers that come in contact with the virus; it is most commonly contracted by dental workers and medical workers exposed to oral secretions. It is also often observed in thumb-sucking children with primary HSV-1 oral infection (autoinoculation) prior to seroconversion, and in adults aged 20 to 30 following contact with HSV-2-infected genitals.

<span class="mw-page-title-main">Viral encephalitis</span> Medical condition

Viral encephalitis is inflammation of the brain parenchyma, called encephalitis, by a virus. The different forms of viral encephalitis are called viral encephalitides. It is the most common type of encephalitis and often occurs with viral meningitis. Encephalitic viruses first cause infection and replicate outside of the central nervous system (CNS), most reaching the CNS through the circulatory system and a minority from nerve endings toward the CNS. Herpes gladiatorum is one of the most infectious of herpes-caused diseases, and is transmissible by skin-to-skin contact. The disease was first described in the 1960s in the New England Journal of Medicine. It is caused by contagious infection with human herpes simplex virus type 1 (HSV-1), which more commonly causes oral herpes. Another strain, HSV-2 usually causes genital herpes, although the strains are very similar and either can cause herpes in any location.

Mollaret's meningitis is a recurrent or chronic inflammation of the protective membranes covering the brain and spinal cord, known collectively as the meninges. Since Mollaret's meningitis is a recurrent, benign (non-cancerous), aseptic meningitis, it is also referred to as benign recurrent lymphocytic meningitis. It was named for Pierre Mollaret, the French neurologist who first described it in 1944.

A cold sore is a type of herpes infection caused by the herpes simplex virus that affects primarily the lip. Symptoms typically include a burning pain followed by small blisters or sores. The first attack may also be accompanied by fever, sore throat, and enlarged lymph nodes. The rash usually heals within ten days, but the virus remains dormant in the trigeminal ganglion. The virus may periodically reactivate to create another outbreak of sores in the mouth or lip.
