On July 7, 2015, an F-16 fighter jet operated by the United States Air Force collided in-flight with a private Cessna 150 single-engine light aircraft over Moncks Corner, South Carolina, United States. Both occupants of the Cessna were killed; the pilot of the F-16 ejected safely.
The subsequent investigation found that the local air traffic control unit had failed to ensure adequate separation between the two aircraft.
At about 11:00 AM on July 7, 2015, the F-16 and Cessna 150 collided over Moncks Corner, South Carolina, about 30 miles (48 km) north of Charleston. [1] Witnesses state that the Cessna was climbing when the F-16 hit it broadside. [2] The Cessna (N3601V) was climbing out of the Berkeley County Airport under visual flight rules and was not in communication with FAA air traffic control. The F-16 was on an ATC-assigned heading of 260 degrees and issued a descent to maintain 1,600 feet for an ILS approach to runway 15 at Joint Base Charleston (CHS).
At 11:00:16, the CHS approach controller advised the F-16 pilot of "traffic 12 o'clock, 2 miles, opposite direction, 1,200 [feet altitude] indicated, type unknown." At 11:00:24, the F-16 pilot responded that he was "looking" for the traffic. At 11:00:26, the controller advised the F-16 pilot to "turn left heading 180 if you don't have that traffic in sight." At 11:00:30, the pilot asked, "Confirm 2 miles?" At 11:00:33, the controller stated, "If you don't have that traffic in sight turn left heading 180 immediately." Over the next 18 seconds, the radar-derived ground track of the F-16 began turning southerly toward the designated heading.
At 11:00:49, the radar target of the F-16 was 1⁄2 nm northeast of the Cessna, at an altitude of 1,500 ft, and was on an approximate track of 215 degrees. At that time, the Cessna reported an altitude of 1,400 ft and was established on an approximate ground track of 110 degrees. The two aircraft collided at an approximate altitude of 1,450 ft. [3]
The F-16 flew for a further three minutes before the pilot transmitted a mayday call. He then ejected safely. [4] [5] Both aircraft crashed in Lewisfield Plantation. [1] Both occupants of the Cessna 150 were killed. [6] [7] [8] The pilot of the F-16 was on an instrument training mission. Its destination was Joint Base Charleston. [9] The Cessna was reported to be on a flight from Berkeley County Airport to Myrtle Beach. [2]
The first aircraft was a General Dynamics F-16C Fighting Falcon registered 96-0085. [10] [11] It was based at Shaw Air Force Base, South Carolina, and was operated by the 20th Fighter Wing of the United States Air Force. [1] [9]
The second aircraft involved was a privately-owned Cessna 150M registered N3601V. [12] [13]
The National Transportation Safety Board conducted the investigation into the accident. [4] [14] The cause of the collision was found to be air traffic control errors in failing to provide an appropriate conflict resolution between the two aircraft. [15] The Federal Aviation Administration also opened an investigation. [2]
Pacific Southwest Airlines Flight 182 was a scheduled flight on September 25, 1978, by Pacific Southwest Airlines (PSA), from Sacramento to San Diego (SAN), with a stopover at Los Angeles (LAX). The aircraft serving the flight, a Boeing 727-214, collided mid-air with a private Cessna 172 over San Diego, California. It was Pacific Southwest Airlines' first fatal accident, and it remains the deadliest air disaster in California history. At the time, it was the deadliest air crash to occur in the United States, and remained so until the crash of American Airlines Flight 191 in May 1979.
The Cessna 150 is a two-seat tricycle gear general aviation airplane that was designed for flight training, touring and personal use. In 1977, it was succeeded in production by the Cessna 152, a minor modification to the original design.
Aeroméxico Flight 498 was a scheduled commercial flight from Mexico City, Mexico, to Los Angeles, California, United States, with several intermediate stops. On Sunday, August 31, 1986, the McDonnell Douglas DC-9 operating the flight was clipped in the tail section by N4891F, a Piper PA-28-181 Cherokee owned by the Kramer family, and crashed into the Los Angeles suburb of Cerritos, killing all 64 on the DC-9, all three on the Piper and an additional 15 people on the ground. Eight on the ground also sustained minor injuries. Blame was assessed equally on the Federal Aviation Administration (FAA) and the pilot of the Cherokee. No fault was found with the DC-9 or the actions of its crew.
On the evening of February 1, 1991, USAir Flight 1493, a Boeing 737-300, collided with SkyWest Airlines Flight 5569, a Fairchild Swearingen Metroliner turboprop aircraft, upon landing at Los Angeles International Airport (LAX). As Flight 1493 was on final approach, the local controller was distracted, though air traffic was not heavy at LAX, by a series of abnormalities, including a misplaced flight progress strip and an aircraft that had inadvertently switched off the tower frequency. The SkyWest flight was told to taxi into takeoff position, while the USAir flight was landing on the same runway.
Essex County Airport, informally known as Caldwell Airport, is a public use airport located in Fairfield Township, Essex County, New Jersey, two nautical miles (4 km) north of the central business district of Caldwell, a borough of northwestern Essex County in the U.S. state of New Jersey. It is owned by the Essex County Improvement Authority. This facility is included in the National Plan of Integrated Airport Systems for 2011–2015, which categorized it as a general aviation reliever airport.
Allegheny Airlines Flight 853 was a regularly scheduled Allegheny Airlines flight from Boston, Massachusetts, to St. Louis, Missouri, with stops in Baltimore, Maryland, Cincinnati, Ohio, and Indianapolis, Indiana. On September 9, 1969, the aircraft serving the flight, a McDonnell Douglas DC-9, collided in mid-air with a Piper PA-28 light aircraft near Fairland, Indiana. The DC-9 was carrying 78 passengers and 4 crew members, and the Piper was leased to a student pilot on a solo cross-country flight. All 83 occupants of both aircraft were killed in the accident and both aircraft were destroyed.
Piedmont Airlines Flight 22 was a Piedmont Airlines Boeing 727-22 that collided with a twin-engine Cessna 310 on July 19, 1967, over Hendersonville, North Carolina, United States. Both aircraft were destroyed and all passengers and crew were killed, including John T. McNaughton, an advisor to U.S. Secretary of Defense Robert McNamara. The aircraft were both operating under instrument flight rules and were in radio contact with the Asheville control tower, though on different frequencies. The accident investigation was the first of a major scale conducted by the newly created National Transportation Safety Board. A review of the investigation conducted 39 years after the crash upheld the original findings that had placed primary responsibility on the Cessna pilot.
On 29 September 2006, Gol Transportes Aéreos Flight 1907, a Boeing 737-800 on a scheduled domestic passenger flight from Manaus, Brazil, to Brasília and Rio de Janeiro, collided mid-air with an Embraer Legacy 600 business jet flying on an opposite heading over the Brazilian state of Mato Grosso. The winglet of the Legacy sliced off about half of the 737's left wing, causing the 737 to break up and crash into an area of dense jungle, killing all 154 passengers and crew. Despite sustaining serious damage to its left wing and tail, the Legacy landed with its seven occupants uninjured.
A transponder is an electronic device that produces a response when it receives a radio-frequency interrogation. Aircraft have transponders to assist in identifying them on air traffic control radar. Collision avoidance systems have been developed to use transponder transmissions as a means of detecting aircraft at risk of colliding with each other.
Hughes Airwest Flight 706 was a regularly scheduled flight operated by American domestic airline Hughes Airwest from Los Angeles, California to Seattle, Washington, with several intermediate stops. On Sunday, June 6, 1971, the McDonnell Douglas DC-9 serving as Flight 706 departed Los Angeles just after 6 p.m. en route to Seattle as a McDonnell Douglas F-4 Phantom II of the United States Marine Corps was approaching Marine Corps Air Station El Toro near Irvine at the end of a flight from Naval Air Station Fallon in Nevada. The two aircraft collided in midair over the San Gabriel Mountains near Duarte, killing all 49 aboard the DC-9 and the F-4 pilot; the F-4 radar intercept officer ejected and survived.
On October 25, 1999, a chartered Learjet 35 business jet was scheduled to fly from Orlando, Florida, United States to Dallas, Texas, United States. Early in the flight, the aircraft, which was climbing to its assigned altitude on autopilot, lost cabin pressure, and all six on board were incapacitated by hypoxia, a lack of oxygen in the brain and body. The aircraft continued climbing past its assigned altitude, then failed to make the westward turn toward Dallas over North Florida and continued on its northwestern course, flying over the southern and midwestern United States for almost four hours and 1,500 miles (2,400 km). The plane ran out of fuel over South Dakota and crashed into a field near Aberdeen after an uncontrolled descent, killing all six on board.
The 1994 St. Louis Airport collision occurred when TWA Flight 427, operated using a McDonnell Douglas MD-82, struck a Cessna 441 Conquest II during its take-off roll, killing both of its occupants. The incident took place on November 22, 1994, at St. Louis Lambert International Airport (STL) in Bridgeton, Missouri.
On August 8, 2009, at 11:53 a.m. (15:53 UTC), nine people died when a tour helicopter and a small private airplane collided over the Hudson River near Frank Sinatra Park in Hoboken, New Jersey, United States. The aircraft were in an area known as the "Hudson River VFR Corridor", which extends from the surface of the river to altitudes of 800 to 1,500 ft at various locations along the Hudson River in the immediate area of New York City. Within this corridor, aircraft operate under visual flight rules (VFR), under which the responsibility to see and avoid other air traffic rests with the individual pilots rather than with the air traffic controller (ATC).
On April 9, 1990, Atlantic Southeast Airlines Flight 2254, a scheduled passenger flight from Muscle Shoals, Alabama, to Gadsden, Alabama, to Atlanta, Georgia, was involved in a mid-air collision with a Cessna 172 over Gadsden. The collision resulted in the death of the pilot and passenger of the Cessna 172.
On Thursday, January 15, 1987, SkyWest Airlines Flight 1834, a Swearingen SA-226TC, and a Mooney M20 were involved in a midair collision at 12:52 MST (UTC−7) near Kearns, Utah, a suburb southwest of Salt Lake City. All ten aboard the two aircraft were killed: two pilots and six passengers aboard the METRO II and two aboard the Mooney.
On February 8, 2000, a Zlin 242L light aircraft piloted by Chicago radio personality Bob Collins was involved in a mid-air collision with a Cessna 172P over Zion, Illinois, while both aircraft were on approach to land at the nearby Waukegan National Airport. The Zlin dived and crashed through the roof of a five-story medical building. The Cessna spiraled down and crashed onto a residential street three blocks away. All three people on board both aircraft were killed and five people on the ground were injured.
On July 7, 2017, an Airbus A320-211 operating as Air Canada Flight 759 was nearly involved in an accident at San Francisco International Airport in San Mateo County, California, United States. The flight, which originated at Toronto Pearson International Airport, had been cleared by air traffic control to land on runway 28R and was on final approach to land on that runway; however, instead of lining up with the runway, the aircraft had lined up with the parallel taxiway, on which four fully loaded and fueled passenger airplanes were stopped awaiting takeoff clearance. The flight crew initiated a go-around prior to landing, after which it landed on 28R without further incident. The aircraft on the taxiway departed for their intended destinations without further incident. The subsequent investigation by the National Transportation Safety Board (NTSB) determined that the Air Canada airplane descended to 59 feet (18 m) above the ground before it began its climb, and that it missed colliding with one of the aircraft on the taxiway by 14 feet (4.3 m).
On May 13, 2019, a de Havilland Canada DHC-2 Beaver floatplane operated by Mountain Air Service collided with a Taquan Air de Havilland Canada DHC-3 Turbine Otter floatplane over George Inlet, Alaska, United States. The DHC-2 broke up in mid-air with the loss of all four passengers and the pilot. The DHC-3 pilot was able to maintain partial control, but the aircraft sustained substantial damage in the collision and the subsequent forced landing; the pilot suffered minor injuries, nine passengers suffered serious injuries, and one passenger was killed. Both aircraft were conducting sightseeing flights under visual flight rules, which state that the pilot of each aircraft is responsible for visually ensuring adequate separation from other air traffic, commonly known as "see and avoid".
Ozark Air Lines Flight 965 was a scheduled commercial flight from Chicago, Illinois, to Lambert Field in St. Louis, Missouri, with a scheduled intermediate stopover at Greater Peoria Regional Airport in Peoria, Illinois. On March 27, 1968, the Douglas DC-9-15 jetliner operating the flight, carrying 44 passengers and five crew, collided in mid-air with a single-engined Cessna 150F while both aircraft were on approach to the same runway at Lambert Field. The DC-9 landed safely with no injuries to any of its 49 occupants, while both pilots in the two-person Cessna died in the collision and subsequent ground impact.
On November 12, 2022, two World War II–era aircraft, a B-17 Flying Fortress and a Bell P-63 Kingcobra, collided mid-air and crashed during the Wings Over Dallas air show at Dallas Executive Airport in Dallas, Texas, United States. The air show, which coincided with Veterans Day commemorations, was organized by the Commemorative Air Force.
