Ozark Air Lines Flight 982 was a regularly scheduled flight on December 27, 1968, originating from Sioux Falls Regional Airport to O'Hare International Airport connecting through Sioux Gateway Airport that crashed shortly after takeoff. [1] The plane struck the ground about 500 feet beyond the end of the runway and came to a stop about 1200 ft from the end of the runway. [2] Thirty-five of the flight's 64 passengers and four crew members were taken to area hospitals, mostly for treatment of minor cuts and scratches. [3] [4] The flight was piloted by Capt. Patrick G. Sweeney, 48, of Wood River, IL. The copilot was John T. Schmeltz, 33, of St. Louis. [5]
Ozark Airlines began service in Sioux City on August 9, 1955. It was the first crash of a DC-9 jetliner for Ozark since the beginning of their use on July 8, 1966. According to the manager of Ozark's News Bureau in St. Louis, Charles R. Ehlert, it was the "first crash of any significance for the airline." [6] The US$3 million aircraft (equivalent to $26,300,000in 2023) was a total loss. [7]
According to the NTSB report: "At approximately 07:11 Local Time, the aircraft crashed a half-mile north of the airport in a wooded area. There were no fatalities. Among the 64 passengers and four crew members, 11 passengers were hospitalized with minor injuries and three crew members received serious injuries. The aircraft was destroyed in the crash. There was no fire. Flight 982 began its takeoff on Runway 35 with the flight crew aware that ice was present on the wings. The aircraft lifted off and the landing gear was selected to the up position by the first officer. The captain began turning off the landing and taxi lights. As the landing gear began to retract, the aircraft rolled abruptly and violently to the right to an angle of bank estimated by the flight crew to have reached 90°. The captain applied additional power and left rudder in an attempt to level the wings. When no immediate response was noted, he then applied left aileron. With the application of left aileron, the right wing came up; however, the roll continued to the left until the left wing contacted the runway. At this point, the captain discontinued the takeoff. He, approximately 110 feet beyond the departure threshold of Runway 35, succeeded in leveling the wings prior to final ground contact. The aircraft came to rest in a grove of trees approximately 1,181 feet beyond the departure end of Runway 35." [5]
According to the NTSB report, the probable cause was: "A stall near the upper limits of ground effect, with subsequent loss of control as a result of aerodynamic and weight penalties of airfoil icing. The flight crew failed to have the airfoil ice removed prior to the attempted takeoff from Sioux City. The board also finds that the crew selected an improper takeoff thrust for the existing gross weight condition of the aircraft." [5]
Anderson Construction Company took 12 hours to salvage and tow the 27.5-ton aircraft to the Air National Guard hangar for further examination by the NTSB. [7]
The McDonnell Douglas DC-10 is an American trijet wide-body aircraft manufactured by McDonnell Douglas. The DC-10 was intended to succeed the DC-8 for long-range flights. It first flew on August 29, 1970; it was introduced on August 5, 1971, by American Airlines.
United Airlines Flight 232 was a regularly scheduled United Airlines flight from Stapleton International Airport in Denver to O'Hare International Airport in Chicago, continuing to Philadelphia International Airport. On July 19, 1989, the DC-10 serving the flight crash-landed at Sioux Gateway Airport in Sioux City, Iowa, after suffering a catastrophic failure of its tail-mounted engine due to an unnoticed manufacturing defect in the engine's fan disk, which resulted in the loss of all flight controls. Of the 296 passengers and crew on board, 112 died during the accident, while 184 people survived. Thirteen of the passengers were uninjured. It was the deadliest single-aircraft accident in the history of United Airlines.
Air Florida Flight 90 was a scheduled U.S. domestic passenger flight operated by Air Florida from Washington National Airport to Fort Lauderdale–Hollywood International Airport, with an intermediate stopover at Tampa International Airport. On January 13, 1982, the Boeing 737-200 registered as N62AF crashed into the 14th Street Bridge over the Potomac River just after take off from Washington National Airport.
On the evening of Friday, February 1, 1991, USAir Flight 1493, a Boeing 737-300, collided with SkyWest Airlines Flight 5569, a Fairchild Swearingen Metroliner turboprop aircraft, upon landing at Los Angeles International Airport (LAX). As Flight 1493 was on final approach, the local controller was distracted, though air traffic was not heavy at LAX, by a series of abnormalities, including a misplaced flight progress strip and an aircraft that had inadvertently switched off the tower frequency. The SkyWest flight was told to taxi into takeoff position, while the USAir flight was landing on the same runway.
Eastern Air Lines Flight 66 was a regularly scheduled flight from New Orleans to New York City that crashed on June 24, 1975, while on approach to New York's John F. Kennedy International Airport, killing 113 of the 124 people on board. The crash was determined to be caused by wind shear caused by a microburst, but the failure of the airport and the flight crew to recognize the severe weather hazard was also a contributing factor.
Sioux Falls Regional Airport, also known as Joe Foss Field, is a public and military use airport three miles northwest of Sioux Falls, South Dakota, United States. It is named in honor of aviator and Sioux Falls native Joe Foss, who later served as the 20th Governor of South Dakota (1955–1959).
Ozark Air Lines was a local service carrier in the United States that operated from 1950 until 1986, when it was purchased by Trans World Airlines (TWA). Ozark got a second chance to be an airline when the carrier that won the routes for which Ozark applied, Parks Air Lines, failed to start them in a timely manner. Parks had its rights revoked, Ozark won not only the routes it previously applied for, but others as well. Parks merged into Ozark and Ozark took over the Parks operation and the single route over which Parks had recently started service, thereby launching Ozark. Ozark over time became a jet carrier with a hub in St. Louis.
Continental Airlines Flight 1713 was a commercial airline flight that crashed while taking off in a snowstorm from Stapleton International Airport in Denver, Colorado, on November 15, 1987. The Douglas DC-9 airliner, operated by Continental Airlines, was making a scheduled flight to Boise, Idaho. Twenty-five passengers and three crew members died in the crash.
Sioux Gateway Airport, also known as Brigadier General Bud Day Field, is a public and military use airport in Woodbury County, Iowa, United States. It is located six nautical miles south of the central business district of Sioux City, just west of Sergeant Bluff. On May 25, 2002, the airport was named in honor of United States Air Force Colonel George Everette "Bud" Day, a Sioux City, Iowa, native who is the only person ever awarded both the Medal of Honor and the Air Force Cross.
The McDonnell Douglas DC-9 is an American five-abreast, single-aisle aircraft designed by the Douglas Aircraft Company. It was initially produced as the Douglas DC-9 prior to August 1967, after which point the company had merged with McDonnell Aircraft to become McDonnell Douglas. Following the introduction of its first jetliner, the high-capacity DC-8, in 1959, Douglas was interested in producing an aircraft suited to smaller routes. As early as 1958, design studies were conducted; approval for the DC-9, a smaller all-new jetliner, came on April 8, 1963. The DC-9-10 first flew on February 25, 1965, and gained its type certificate on November 23, to enter service with Delta Air Lines on December 8.
United Airlines Flight 227 (N7030U), a scheduled passenger flight from LaGuardia Airport New York City to San Francisco International Airport, California, crashed short of the runway while attempting a scheduled landing at Salt Lake City International Airport, Utah, on Thursday, November 11, 1965.
Western Airlines Flight 2605, nicknamed the "Night Owl", was an international scheduled passenger flight from Los Angeles, California, to Mexico City, Mexico. On October 31, 1979, at 5:42 a.m. CST (UTC−06:00), the McDonnell Douglas DC-10 used on the flight crashed at Mexico City International Airport in fog after landing on a runway that was closed for maintenance. Of the 88 occupants on board, 72 were killed, in addition to a maintenance worker who died when the plane struck his vehicle.
Ozark Air Lines Flight 650 was a regularly scheduled flight on December 20, 1983 from Sioux Gateway Airport in Sioux City, Iowa, to Sioux Falls Regional Airport in Sioux Falls, South Dakota. While landing in Sioux Falls, the aircraft struck a snow plow on the runway and burst into flames. The snow plow's driver was killed, and two flight attendants were injured.
Continental Airlines Flight 1404 was a Continental Airlines domestic flight from Denver International Airport in Denver, Colorado, to George Bush Intercontinental Airport in Houston, Texas. On the evening of December 20, 2008, the flight crashed while taking off from Denver, resulting in two critical injuries, 36 noncritical injuries, and a hull loss of the Boeing 737-524 aircraft.
On December 27, 1968, North Central Airlines Flight 458 crashed into a hangar while attempting a night landing in poor weather at O'Hare International Airport in Chicago, Illinois, in the United States. Of the 41 passengers and four crew members, only 17 passengers and one crew member survived. One person was killed and six were injured on the ground.
United Airlines Flight 2885 was a scheduled cargo flight from Cleveland to Los Angeles, with stopover in Detroit. On January 11, 1983, a DC-8 operating as Flight 2885 crashed after take-off from Detroit, killing all three crew members. The National Transportation Safety Board (NTSB) investigation determined that the cause for the crash was pilot error. A radioactive package was found on board, but no radioactive material was spilled.
Ryan International Airlines Flight 590 was a cargo flight carrying mail for the United States Postal Service from Greater Buffalo International Airport (BUF) in Buffalo, New York, to Indianapolis International Airport (IND) in Indiana, with a stopover at Cleveland Hopkins International Airport (CLE) in Cleveland, Ohio. On February 17, 1991, the McDonnell Douglas DC-9-15RC operating the flight crashed on takeoff from Cleveland during icing conditions. Both pilots, the aircraft's only occupants, were killed. The National Transportation Safety Board (NTSB) determined that the causes of the crash were the flight crew failing to deice their aircraft, and the inexperience of the Federal Aviation Administration (FAA), McDonnell Douglas, and Ryan International Airlines with icing condition on DC-9-10 aircraft.
Ozark Air Lines Flight 965 was a scheduled commercial flight from Chicago, Illinois, to Lambert Field in St. Louis, Missouri, with a scheduled intermediate stopover at Greater Peoria Regional Airport in Peoria, Illinois. On March 27, 1968, the Douglas DC-9-15 jetliner operating the flight, carrying 44 passengers and five crew, collided in mid-air with a single-engined Cessna 150F while both aircraft were on approach to the same runway at Lambert Field. The DC-9 landed safely with no injuries to any of its 49 occupants, while both pilots in the two-person Cessna died in the collision and subsequent ground impact.
