The intercostal nerves are part of the somatic nervous system, and arise from the anterior rami of the thoracic spinal nerves from T1 to T11. [1] [2] The intercostal nerves are distributed chiefly to the thoracic pleura and abdominal peritoneum, and differ from the anterior rami of the other spinal nerves in that each pursues an independent course without plexus formation. [1] [3]
The first two nerves supply fibers to the upper limb and thorax; the next four distribute to the walls of the thorax; the lower five supply the walls of the thorax and abdomen. [3] The 7th intercostal nerve ends at the xyphoid process of the sternum. [3] The 10th intercostal nerve terminates at the navel. [3] The 12th (subcostal) thoracic is distributed to the walls of the abdomen and groin. [3] Each of these fibers contains around 1300 axons. [2]
Unlike the nerves from the autonomic nervous system that innervate the visceral pleura of the thoracic cavity, the intercostal nerves arise from the somatic nervous system. This enables them to control the contraction of muscles, as well as provide specific sensory information regarding the skin and parietal pleura. This explains why damage to the internal wall of the thoracic cavity can be felt as a sharp pain localized in the injured region. Damage to the visceral pleura is experienced as an un-localized ache.
The anterior division of the first thoracic nerve divides into two branches. The larger branch leaves the thorax in front of the neck of the first rib, and enters the brachial plexus. The smaller branch, the first intercostal nerve, runs along the first intercostal space, and ends on the front of the chest as the first anterior cutaneous branch of the thorax. Occasionally, this anterior cutaneous branch is missing.
The first intercostal nerve rarely gives off a lateral cutaneous branch; but sometimes sends a small branch to communicate with the intercostobrachial.
From the second thoracic nerve it frequently receives a connecting twig, which ascends over the neck of the second rib. This nerve was first described by Kuntz in 1927. There is considerable anatomic variation, but Kuntz nerve may be present in 40-80% of the population. [4] [5]
The anterior divisions of the second, third, fourth, fifth, and sixth thoracic nerves, and the small branch from the first thoracic, are confined to the walls of the thorax, and are named thoracic intercostal nerves.
They pass forward in the intercostal spaces below the intercostal vessels. At the back of the chest they lie between the pleura and the posterior intercostal membranes, but soon they run between the internal intercostals and the innermost intercostals then anteriorly they lie between the pleura and the internal intercostals.
Near the sternum, they cross in front of the internal mammary artery and transversus thoracis muscle, pierce the intercostales interni, the anterior intercostal membranes, and pectoralis major, and supply the integument of the front of the thorax and over the mammary gland, forming the anterior cutaneous branches of the thorax; the branch from the second nerve unites with the anterior supraclavicular nerves of the cervical plexus.
The fourth intercostal nerve is innervated by cutaneous slowly-adapting and rapidly-adapting mechanoreceptors, especially by ones densely-packed under the areola; innervation subsequently triggers oxytocin release, which, when in the peripheral bloodstream, causes myoepithelial cell contraction and lactation: this is an example of a non-nerve-innervation muscular reflex.
Numerous slender muscular filaments supply the Intercostales, the Subcostales, the Levatores costarum, the Serratus posterior superior, and the Transversus thoracis. At the front of the thorax some of these branches cross the costal cartilages from one intercostal space to another.
The lateral cutaneous branch of the second intercostal nerve does not divide, like the others, into an anterior and a posterior branch; it is named the intercostobrachial nerve.
The lateral cutaneous branch of the last thoracic nerve is large, and undivided.
It perforates the internal and the external oblique muscles, descends over the iliac crest in front of the lateral cutaneous branch of the iliohypogastric nerve, and is distributed to the skin of the front part of the gluteal muscles, some of its filaments extending as low as the greater trochanter of the femur.

It innervates the medial and lateral heads of the triceps brachii muscle of the arm, as well as all 12 muscles in the posterior osteofascial compartment of the forearm and the associated joints and overlying skin.
The median nerve is a nerve in humans and other animals in the upper limb. It is one of the five main nerves originating from the brachial plexus.
The brachial plexus is a network of nerves formed by the anterior rami of the lower four cervical nerves and first thoracic nerve. This plexus extends from the spinal cord, through the cervicoaxillary canal in the neck, over the first rib, and into the armpit, it supplies afferent and efferent nerve fibers to the chest, shoulder, arm, forearm, and hand.
A spinal nerve is a mixed nerve, which carries motor, sensory, and autonomic signals between the spinal cord and the body. In the human body there are 31 pairs of spinal nerves, one on each side of the vertebral column. These are grouped into the corresponding cervical, thoracic, lumbar, sacral and coccygeal regions of the spine. There are eight pairs of cervical nerves, twelve pairs of thoracic nerves, five pairs of lumbar nerves, five pairs of sacral nerves, and one pair of coccygeal nerves. The spinal nerves are part of the peripheral nervous system.
The axillary nerve or the circumflex nerve is a nerve of the human body, that originates from the brachial plexus at the level of the axilla (armpit) and carries nerve fibers from C5 and C6. The axillary nerve travels through the quadrangular space with the posterior circumflex humeral artery and vein to innervate the deltoid and teres minor.
The long thoracic nerve is a branch of the brachial plexus derived from cervical nerves C5-C7 that innervates the serratus anterior muscle.
In human anatomy, the subclavian arteries are paired major arteries of the upper thorax, below the clavicle. They receive blood from the aortic arch. The left subclavian artery supplies blood to the left arm and the right subclavian artery supplies blood to the right arm, with some branches supplying the head and thorax. On the left side of the body, the subclavian comes directly off the aortic arch, while on the right side it arises from the relatively short brachiocephalic artery when it bifurcates into the subclavian and the right common carotid artery.
The cervical plexus is a nerve plexus of the anterior rami of the first four cervical spinal nerves C1-C4. The cervical plexus provides motor innervation to some muscles of the neck, and the diaphragm; it provides sensory innervation to parts of the head, neck, and chest.
The transversus thoracis muscle, also known as triangularis sterni, lies internal to the thoracic cage, anteriorly. It is usually a thin plane of muscular and tendinous fibers, however on athletic individuals it can be a thick 'slab of meat', situated upon the inner surface of the front wall of the chest. It is in the same layer as the subcostal muscles and the innermost intercostal muscles.
A nerve plexus is a plexus of intersecting nerves. A nerve plexus is composed of afferent and efferent fibers that arise from the merging of the anterior rami of spinal nerves and blood vessels. There are five spinal nerve plexuses, except in the thoracic region, as well as other forms of autonomic plexuses, many of which are a part of the enteric nervous system. The nerves that arise from the plexuses have both sensory and motor functions. These functions include muscle contraction, the maintenance of body coordination and control, and the reaction to sensations such as heat, cold, pain, and pressure. The medial pectoral nerve is (typically) a branch of the medial cord of the brachial plexus and is derived from spinal nerve roots C8-T1. It provides motor innervation to the pectoralis minor muscle, and the lower half of the pectoralis major muscle. It runs along the inferior border of the pectoralis minor muscle.
The femoral nerve is a nerve in the thigh that supplies skin on the upper thigh and inner leg, and the muscles that extend the knee. It is the largest branch of the lumbar plexus.
The lateral cutaneous nerve of the thigh is a cutaneous nerve of the thigh. It originates from the dorsal divisions of the second and third lumbar nerves from the lumbar plexus. It passes under the inguinal ligament to reach the thigh. It supplies sensation to the skin on the lateral part of the thigh by an anterior branch and a posterior branch.
The intercostobrachial nerve is the name applied to the lateral cutaneous branch of the second intercostal nerve. It arises anterior to the long thoracic nerve. It provides sensory innervation to the skin of the axilla, and a variable region of the medial side of the upper arm.
The intercostal arteries are a group of arteries passing within an intercostal space. There are 9 anterior and 11 posterior intercostal arteries on each side of the body. The anterior intercostal arteries are branches of the internal thoracic artery and its terminal branch - the musculophrenic artery. The posterior intercostal arteries are branches of the supreme intercostal artery and thoracic aorta.
The ventral ramus is the anterior division of a spinal nerve. The ventral rami supply the antero-lateral parts of the trunk and the limbs. They are mainly larger than the dorsal rami.
