Normalization of deviance, according to American sociologist Diane Vaughan, is the process in which deviance from correct or proper behavior or rule becomes culturally normalized. [1]
Vaughan defines the process where a clearly unsafe practice becomes considered normal if it does not immediately cause a catastrophe: "a long incubation period [before a final disaster] with early warning signs that were either misinterpreted, ignored or missed completely". [2] [3]
The original example cited by Vaughan is the events leading to the Space Shuttle Challenger disaster in 1986, but the concept has also been applied to aviation safety, [4] [5] clinical practice in medicine, [6] and the public's deviance from health measures aimed to stop the COVID-19 pandemic.[ citation needed ] [7]
Normalization of deviance can exist in conjunction with corporate omerta where deviation from rules is held up by a code of silence surrounding the deviations or an unspoken agreement on rhetoric within a group of executives. One of the reasons Lion Air Flight 610 and Ethiopian Airlines Flight 302 crashed was normalization of deviance based on a criticism of corporate omerta with a "culture of silence". [8]

For the worst corporate crimes, corporations may face judicial dissolution, sometimes called the "corporate death penalty", which is a legal procedure in which a corporation is forced to dissolve or cease to exist.
The Abilene paradox is a collective fallacy, in which a group of people collectively decide on a course of action that is counter to the preferences of most or all individuals in the group, while each individual believes it to be aligned with the preferences of most of the others. It involves a breakdown of group communication in which each member mistakenly believes that their own preferences are counter to the group's, and therefore does not raise objections, or even states support for an outcome they do not want.

It was the first fatal accident involving an American spacecraft while in flight.
Crew resource management or cockpit resource management (CRM) is a set of training procedures for use in environments where human error can have devastating effects. CRM is primarily used for improving aviation safety and focuses on interpersonal communication, leadership, and decision making in aircraft cockpits. Its founder is David Beaty, a former Royal Air Force and a BOAC pilot who wrote "The Human Factor in Aircraft Accidents" (1969). Despite the considerable development of electronic aids since then, many principles he developed continue to prove effective.
Medicalization is the process by which human conditions and problems come to be defined and treated as medical conditions, and thus become the subject of medical study, diagnosis, prevention, or treatment. Medicalization can be driven by new evidence or hypotheses about conditions; by changing social attitudes or economic considerations; or by the development of new medications or treatments.
In aviation, pilot error generally refers to an action or decision made by a pilot that is a substantial contributing factor leading to an aviation accident. It also includes a pilot's failure to make a correct decision or take proper action. Errors are intentional actions that fail to achieve their intended outcomes. The Chicago Convention defines the term "accident" as "an occurrence associated with the operation of an aircraft [...] in which [...] a person is fatally or seriously injured [...] except when the injuries are [...] inflicted by other persons." Hence the definition of "pilot error" does not include deliberate crashing.
Safety culture is the element of organizational culture which is concerned with the maintenance of safety and compliance with safety standards. It is informed by the organization's leadership and the beliefs, perceptions and values that employees share in relation to risks within the organization, workplace or community. Safety culture has been described in a variety of ways: notably, the National Academies of Science and the Association of Land Grant and Public Universities have published summaries on this topic in 2014 and 2016.
Criticism of the Space Shuttle program stemmed from claims that NASA's Space Shuttle program failed to achieve its promised cost and utility goals, as well as design, cost, management, and safety issues. Fundamentally, it failed in the goal of reducing the cost of space access. Space Shuttle incremental per-pound launch costs ultimately turned out to be considerably higher than those of expendable launchers. In 2010, the incremental cost per flight of the Space Shuttle was $409 million, or $14,186 per kilogram to low Earth orbit (LEO). In contrast, the comparable Proton launch vehicle cost was $141 million, or $6,721 per kilogram to LEO and the Soyuz 2.1 was $55 million, or $6,665 per kilogram, despite these launch vehicles not being reusable.
A system accident is an "unanticipated interaction of multiple failures" in a complex system. This complexity can either be of technology or of human organizations and is frequently both. A system accident can be easy to see in hindsight, but extremely difficult in foresight because there are simply too many action pathways to seriously consider all of them. Charles Perrow first developed these ideas in the mid-1980s. Safety systems themselves are sometimes the added complexity which leads to this type of accident.
In aviation, a mid-air collision is an accident in which two or more aircraft come into unplanned contact during flight. Owing to the relatively high velocities involved and the likelihood of subsequent impact with the ground or sea, very severe damage or the total destruction of at least one of the aircraft usually results.
Downeast Airlines was a commuter airline based in Rockland, Maine, from 1960 to June 1, 2007, when it was acquired by Maine Atlantic Aviation, an arm of the Jordache Enterprises conglomerate. While the airline was closed, Downeast Air remains a fixed-base operator at its former home airfield.
A conspiracy of silence, or culture of silence, describes the behavior of a group of people that by unspoken consensus does not mention, discuss, or acknowledge a given subject. The practice may be motivated by positive interest in group solidarity or by negative impulses such as fear of political repercussion or social ostracism. Unlike a taboo subject or the use of euphemisms, a conspiracy of silence is limited to specific social and political contexts rather than to an entire culture.

Deviance or the sociology of deviance explores the actions and/or behaviors that violate social norms across formally enacted rules as well as informal violations of social norms. Although deviance may have a negative connotation, the violation of social norms is not always a negative action; positive deviation exists in some situations. Although a norm is violated, a behavior can still be classified as positive or acceptable.
Groupthink is a psychological phenomenon that occurs within a group of people in which the desire for harmony or conformity in the group results in an irrational or dysfunctional decision-making outcome. Cohesiveness, or the desire for cohesiveness, in a group may produce a tendency among its members to agree at all costs. This causes the group to minimize conflict and reach a consensus decision without critical evaluation.
Abusive supervision is most commonly studied in the context of the workplace, although it can arise in other areas such as in the household and at school. "Abusive supervision has been investigated as an antecedent to negative subordinate workplace outcome." "Workplace violence has combination of situational and personal factors". The study that was conducted looked at the link between abusive supervision and different workplace events.
Pilot decision making, also known as aeronautical decision making (ADM), is a process that aviators perform to effectively handle troublesome situations that are encountered. Pilot decision-making is applied in almost every stage of the flight as it considers weather, air spaces, airport conditions, estimated time of arrival and so forth. During the flight, employers pressure pilots regarding time and fuel restrictions since a pilots’ performance directly affects the company’s revenue and brand image. This pressure often hinders a pilot's decision-making process leading to dangerous situations as 50% to 90% of aviation accidents are the result of pilot error.
Culture can affect aviation safety through its effect on how the flight crew deals with difficult situations; cultures with lower power distances and higher levels of individuality can result in better aviation safety outcomes. In higher power cultures subordinates are less likely to question their superiors. The crash of Korean Air Flight 801 in 1997 was attributed to the pilot's decision to land despite the junior officer's disagreement, while the crash of Avianca Flight 052 was caused by the failure to communicate critical low-fuel data between pilots and controllers, and by the failure of the controllers to ask the pilots if they were declaring an emergency and assist the pilots in landing the aircraft. The crashes have been blamed on aspects of the national cultures of the crews.
Diane Vaughan is an American sociologist and professor at Columbia University. She is known for her work on organizational and management issues, in particular in the case of the space shuttle Challenger Disaster.
