The diocese is led by a prelate bishop, who pastors the motherchurch in the City of Mayagüez, Catedral Nuestra Señora de la Candelaria in front of the Plaza Colón.

His predecessor, Gustavo Rodriguez Vega, was called away from the diocese by Pope Francis to serve as Archbishop of Yucatán, which Archdiocese is based in Mérida.
