The phrase originated in gay male culture in the United States and was historically an insult. Some women who associate with gay men object to being called fag hags while others embrace the term. The male counterpart, for heterosexual men who have similar interpersonal relationships with gay and bisexual men, is fag stag.

He was taken by rescuers to Poudre Valley Hospital in Fort Collins, Colorado, where he died six days later from severe head injuries received during the attack.

The term has its roots in self-contained English toilet blocks resembling small cottages in their appearance; in the English cant language of Polari this became a double entendre by gay men referring to sexual encounters.
