Orbital x-ray or orbital radiography is an x-ray of both left and right eye sockets, to include the Frontal Sinuses and Maxillary Sinuses.

Positioning

The x-ray can be taken with the patient in either an erect or supine position, although most usually erect. The x-ray is taken PA (postero-antero), meaning that the patient faces towards the receiver and away from the x-rays source. The patients chin rests on the image receiver, which tilts the head up allowing the orbits to be clear of the internal structure of the Petrous ridge. This view is called Occipital-Mental or OM. [1] [2]

An orbital x-ray usually requires only one view unless the requester is looking for evidence of metallic fragments, in which case two projections can be made. One with the eyes looking up, one with the eyes looking down. These views will show any movement of fragments and helps rule out false positives / artefacts which may be present on the image receiver. Two other important views are the Water's view which helps visualise the anterior orbital floor and maxillary sinuses; and the Caldwell view which helps to visualise the frontal and ethmoid sinuses and posterior orbital floor. Uses

It is useful for detecting fractures of the surrounding bone arising from injury or disease. It is also commonly used for detecting foreign objects in the eye that an ophthalmoscope cannot detect and is sometimes given prior to an MRI where metal fragments could cause significant damage.

Maxilla

The maxilla in vertebrates is the upper fixed bone of the jaw formed from the fusion of two maxillary bones. In humans, the upper jaw includes the hard palate in the front of the mouth. The two maxillary bones are fused at the intermaxillary suture, forming the anterior nasal spine. This is similar to the mandible, which is also a fusion of two mandibular bones at the mandibular symphysis. The mandible is the movable part of the jaw.

The orbital contents comprise the eye, the orbital and retrobulbar fascia, extraocular muscles, cranial nerves II, III, IV, V, and VI, blood vessels, fat, the lacrimal gland with its sac and duct, the eyelids, medial and lateral palpebral ligaments, check ligaments, the suspensory ligament, septum, ciliary ganglion and short ciliary nerves.

Cavernous sinus thrombosis

This is a rare disorder and can be of two types–septic cavernous thrombosis and aseptic cavernous thrombosis. Most commonly the form is of septic cavernous sinus thrombosis. Orbital blowout fracture

An orbital blowout fracture is a traumatic deformity of the orbital floor or medial wall, typically resulting from impact of a blunt object larger than the orbital aperture, or eye socket. Most commonly the inferior orbital wall i.e. the floor is likely to collapse, because the bones of the roof and lateral walls are robust. Although the bone forming the medial wall is thinnest, it is buttressed by the bone separating the ethmoidal air cells. The comparatively thin bone of the floor of the orbit and roof of the maxillary sinus has no support and therefore it is the inferior wall that collapses mostly. So the medial wall blowout fractures are second most common, whereas superior wall i.e. the roof and lateral wall blowout fractures are uncommon & rare respectively. There are two broad categories of blowout fractures: open door, which are large, displaced and comminuted, and trapdoor, which are linear, hinged, and minimally displaced. They are characterized by double vision, sunken ocular globes, and loss of sensation of the cheek and upper gums due to infraorbital nerve injury.

A sinus is a sac or cavity in any organ or tissue, or an abnormal cavity or passage caused by the destruction of tissue. In common usage, "sinus" usually refers to the paranasal sinuses, which are air cavities in the cranial bones, especially those near the nose and connecting to it. Most individuals have four paired cavities located in the cranial bone or skull.

Projectional radiography Formation of 2D images using X-rays

Projectional radiography, also known as conventional radiography, is a form of radiography and medical imaging that produces two-dimensional images by x-ray radiation. The image acquisition is generally performed by radiographers, and the images are often examined by radiologists. Both the procedure and any resultant images are often simply called "X-ray". Plain radiography or roentgenography generally refers to projectional radiography. Plain radiography can also refer to radiography without a radiocontrast agent or radiography that generates single static images, as contrasted to fluoroscopy, which are technically also projectional.

Le Fort fracture of skull

A Le Fort fracture of the skull is a classic transfacial fracture of the midface, involving the maxillary bone and surrounding structures in either a horizontal, pyramidal or transverse direction. The hallmark of Lefort fractures is traumatic pterygomaxillary separation, which signifies fractures between the pterygoid plates, horseshoe shaped bony protuberances which extend from the inferior margin of the maxilla, and the maxillary sinuses. Continuity of this structure is a keystone for stability of the midface, involvement of which impacts surgical management of trauma victims, as it requires fixation to a horizontal bar of the frontal bone. The pterygoid plates lie posterior to the upper dental row, or alveolar ridge, when viewing the face from an anterior view. The fractures are named after French surgeon René Le Fort (1869–1951), who discovered the fracture patterns by examining crush injuries in cadavers.

Frontal sinus trephination

Frontal sinus trephination is a surgical procedure where in a small opening is made in the floor of the frontal sinus facilitating drainage of its contents. Drainage of the frontal sinus is done through the floor of frontal sinus above the inner canthus.

Oroantral fistula

Oroantral fistula (OAF) is an epithelialised oroantral communication (OAC). OAC refers to an abnormal connection between the oral cavity and antrum. The creation of an OAC is most commonly due to the extraction of a maxillary (upper) tooth closely related to the antral floor. A small OAC may heal spontaneously but a larger OAC would require surgical closure to prevent the development of persistent OAF and chronic sinusitis.
