Severe fever with thrombocytopenia syndrome (SFTS) is a tick-borne infection [1] caused by Dabie bandavirus also known as the SFTS virus, first reported between late March and mid-July 2009 in rural areas of Hubei and Henan provinces in Central China. [2]

SFTS has fatality rates ranging from 12% to as high as 30% in some areas due to multiple organ failure, thrombocytopenia (low platelet count), leucopenia (low white blood cell count), and elevated liver enzyme levels.

Virology

SFTS virus (SFTSV) is a virus in the order Bunyavirales , discovered in 2009. [2]

Transmission

The life cycle of the SFTSV most likely involves arthropod vectors like Haemaphysalis longicornis ticks and animal hosts. Humans appear to be largely accidental hosts. .[ citation needed ]

Person-to-person transmission was not initially noted in the 2011, but occurs via blood or mucus as documented in 2012. [3]

Symptoms and signs

SFTS is an emerging infectious disease causing fever, vomiting, diarrhea, loss of consciousness and heamorrhage. [1]

SFTS has fatality rates ranging from 12% to as high as 30% in some areas due to multiple organ failure, thrombocytopenia (low platelet count), leucopenia (low white blood cell count), and elevated liver enzyme levels.

In July 2013, South Korea reported a total of eight deaths since August 2012. [4]

In July 2017, Japanese doctors reported that a woman had died of SFTS after being bitten by a cat that may have itself been infected by a tick. The woman had no visible tick bites, leading doctors to believe that the cat — which died as well — was the transmission vector. [5] [6]

Epidemiology

SFTS occurs in China's rural areas from March to November with the majority of cases from April to July. In 2013, Japan and Korea also reported several cases with deaths. [7]

In early 2020 an outbreak occurred in East China, more than 37 people were found with SFTS in Jiangsu province, while 23 more were found infected in Anhui province in August 2020. [8] Seven people have died. [9]

Tick-borne diseases, which afflict humans and other animals, are caused by infectious agents transmitted by tick bites. They are caused by infection with a variety of pathogens, including rickettsia and other types of bacteria, viruses, and protozoa. The economic impact of tick-borne diseases is considered to be substantial in humans, and tick-borne diseases are estimated to affect ~80 % of cattle worldwide. Most of these pathogens require passage through vertebrate hosts as part of their life cycle. Tick-borne infections in humans, farm animals, and companion animals are primarily associated with wildlife animal reservoirs. Many tick-borne infections in humans involve a complex cycle between wildlife animal reservoirs and tick vectors. The survival and transmission of these tick-borne viruses are closely linked to their interactions with tick vectors and host cells. <i>Bunyavirales</i> Order of RNA viruses

Bunyavirales is an order of segmented negative-strand RNA viruses with mainly tripartite genomes. Member viruses infect arthropods, plants, protozoans, and vertebrates. It is the only order in the class Ellioviricetes. The name Bunyavirales derives from Bunyamwera, where the original type species Bunyamwera orthobunyavirus was first discovered. Dabie bandavirus, also called SFTS virus, is a tick-borne virus in the genus Bandavirus in the family Phenuiviridae, order Bunyavirales. The clinical condition it caused is known as severe fever with thrombocytopenia syndrome (SFTS). SFTS is an emerging infectious disease that was first described in northeast and central China 2009 and now has also been discovered in Japan, South Korea, Vietnam and Taiwan in 2015. SFTS has a fatality rate of 12% and as high as over 30% in some areas. The major clinical symptoms of SFTS are fever, vomiting, diarrhea, multiple organ failure, thrombocytopenia, leukopenia and elevated liver enzyme levels. Another outbreak occurred in East China in the early half of 2020.

Borrelia miyamotoi is a bacterium of the spirochete phylum in the genus Borrelia. A zoonotic organism, B. miyamotoi can infect humans through the bite of several species of hard-shell Ixodes ticks, the same kind of ticks that spread B. burgdorferi, the causative bacterium of Lyme disease. Ixodes ticks are also the primary vector in the spread of babesiosis and anaplasmosis.

<span class="mw-page-title-main">Bourbon virus</span> Species of virus

Bourbon virus is an RNA virus in the genus Thogotovirus of the family Orthomyxoviridae, which is similar to Dhori virus and Batken virus. It was first identified in 2014 in a man from Bourbon County, Kansas, United States, who died after being bitten by ticks. The case is the eighth report of human disease associated with a thogotovirus globally, and the first in the Western hemisphere. As of May 2015, a case was discovered in Stillwater, Oklahoma, and relatively little is known about the virus. No specific treatment or vaccine is available. The virus is suspected to be transmitted by ticks or insects, and avoidance of bites is recommended to reduce risk of infection. In June 2017 a 58-year-old female Missouri State Park employee died from an infection of the Bourbon virus after it had been misdiagnosed for a significant period of time.

The nonstructural S segment (NSs) created by Bunyaviridae virus family, are able to interact with the human immune system, in order to increase their replication in infected cells. Understanding this mechanism can have global health impacts.

Wetland virus or WELV is a tick borne Orthonairovirus which can infect humans. It can produce fever, headache, dizziness, malaise, arthritis and less commonly petechiae, localized lymphadenopathy. Complications may include neurological symptoms.
