On March 24, 2014, a Chicago "L" train crashed at O'Hare station, injuring 34 people. A Blue Line train entering the station, the line's terminus, crashed into a bumper block and ran up an escalator after the operator fell asleep at the controls. The crash caused over $11 million in damage and halted service to O'Hare station for 6 days.
The National Transportation Safety Board's investigation of the crash found that the train operator was affected by sleep debt from insufficent sleep over the course of multiple days. In its final report on the crash, the NTSB criticized the Chicago Transit Authority's scheduling policies, which it found contributed to train operators becoming fatigued. Additionally, the NTSB found that the train stop system at O'Hare station was incorrectly configured. The CTA responded by revising its scheduling policies and reconfiguring train stops at terminal stations throughout the "L" system. [1]
The crash occurred at O'Hare station, the northwestern terminus of the Blue Line of the Chicago "L". The underground station has three tracks with two island platforms. Passengers enter the station at the end of the platforms via a set of escalators and elevators, located in line with the center track.
The train involved in the crash was Blue Line train number 141, which was operated by an eight-car train of 2600-series cars. Blue Line service normally operates 24/7 between O'Hare and Forest Park, but scheduled maintenance on the night of March 23-24 resulted in trains operating a shorter route from O'Hare to Logan Square. Train 141 departed Logan Square at 2:23 a.m, and the operator reported that she began to feel drowsy near Harlem station. The operator continued along the route towards O'Hare after adjusting a heater in the train cab in an attempt to stay awake. 50 people were onboard the train, which was scheduled to terminate at O'Hare on the center track. As Train 141 was entering O'Hare station, the operator fell asleep. The train entered O'Hare station at a normal speed of approximately 25 miles per hour (40 km/h), and was routed onto the center track. The train operator woke up when the train hit a train stop approximately 45 ft (14 m) from the end of the track, and immediately applied the brakes. [1] : 6
At 2:49 a.m. local time (07:50 UTC), the train crashed into the bumper block at the end of the center track. The train slowed slightly to approximately 23 miles per hour (37 km/h) by the time it hit the bumper block, which failed to stop the train as designed. [1] : 3 The train then overran the bumper block and rode up onto an escalator in front of the center track, coming to rest with the front car partially up the escalator. [2] [3] The force of the collision separated the body of the leading car from its front truck, which remained on the track. [1] : 4
While a spokesman initially stated that it was likely that the train entered the station at too high a speed, [4] later estimates indicated that the train entered the station at 25 to 26 miles per hour (40 to 42 km/h), which was not an excessive speed. [3]
At least 50 firefighters and paramedics responded to the accident. [5] Thirty-four people were injured, including the train operator. [1] : 1 They were taken to the Advocate Lutheran General Hospital, the Our Lady of the Resurrection Medical Center, the Resurrection Hospital and the Swedish Covenant Hospital. [2]
Following the accident, the line between O'Hare and Rosemont was closed, with a replacement bus service in place. The CTA initially estimated that the station would be closed for 48 hours, and later revised its estimate to one week. [2] [3]
The front two cars of the train were damaged in the accident, with the cost of the damage initially estimated at $6 million. [2] On March 26, work began to scrap the lead car on site. [6] The derailed train was removed from the station on March 27, and the station reopened on March 30, 6 days after the crash. [7] The escalator damaged in the crash was replaced by stairs. [8] The final estimate of the cost of the damage was $11.2 million, equivalent to $14.2 million in 2023. [9] [1] : 1
The National Transportation Safety Board (NTSB) opened an investigation into the accident. [1] Investigators focused on the theory that the operator, 25-year-old Brittney Tysheka Haywood, [10] fell asleep at the controls. [2] She stated that she had recently performed "a lot of overtime". [11] When interviewed by the NTSB, Haywood admitted falling asleep at the controls and disclosed that she had done a similar thing the previous month, which resulted in an overshoot at Belmont (Blue) on February 1. Haywood did not reveal to CTA that she had fallen asleep when questioned about the overshoot. [12] [13]
The investigation was hampered by the train's lack of a train event recorder, although it was fitted with a video camera. The recording from the camera was examined. [2] Images from 41 cameras within the station were also studied. [3] The NTSB's report ultimately confirmed the original findings. [14] [15]
In its final report, the NTSB concluded:
The National Transportation Safety Board determines that the probable cause of the accident was the failure of the train operator to stop the train at the appropriate signal due to falling asleep as a result of fatigue, which was the result of the challenges of working shiftwork, circadian factors, and acute sleep loss resulting from her ineffective off-duty time management. In addition, Chicago Transit Authority failed to effectively manage the operator’s work schedule to mitigate the risk of fatigue. Contributing to the severity of the accident was Chicago Transit Authority’s failure to identify the insufficient stopping distance and inadequate speed restriction at the center pocket track at O’Hare Station. As a direct consequence of the accident, the CTA reduced the speed limit into the O'Hare station from 25 miles per hour (40 km/h) to 15 miles per hour (24 km/h). The area of the speed limit was extended away from the station. [13] Similar modifications were made at other "L" terminal stations, including Midway, Linden, and Cottage Grove. [1] : 18 The CTA fired the train operator in April 2014. [16]

As of January 2024, the "L" had 1,480 rail cars operating across eight different routes on 224.1 miles of track. CTA trains make about 1,888 trips each day servicing 146 train stations. In 2023, the system had 117,447,000 rides, or about 416,200 per weekday in the third quarter of 2024.

The Blue Line is a 26.93-mile-long (43.34 km) Chicago "L" line which runs from O'Hare International Airport at the far northwest end of the city, through downtown via the Milwaukee–Dearborn subway and across the West Side to its southwest end in Forest Park, with a total of 33 stations. At about 27 miles, it is the longest line on the Chicago "L" system and second busiest, and one of the longest local subway/elevated lines in the world. It has an average of 72,475 passengers boarding each weekday in 2023.
The Green Line is a rapid transit line in Chicago, Illinois, operated by the Chicago Transit Authority (CTA) as part of the Chicago "L" system. It is the only completely elevated route in the "L" system. All other routes may have various combinations of elevated, subway, street level, or freeway median sections. This line is also the only route with three terminals: trains departing Harlem/Lake alternate destinations between Ashland/63rd and Cottage Grove.
Atlantic Terminal is the westernmost commuter rail terminal on the Long Island Rail Road's (LIRR) Atlantic Branch, located at Flatbush Avenue and Atlantic Avenue in Downtown Brooklyn, New York City. It is the primary terminal for the West Hempstead Branch, and a peak-hour terminal for some trains on the Hempstead Branch, Far Rockaway Branch, Port Jefferson Branch,Ronkonkoma Branch, and the Babylon Branch; most other service is provided by frequent shuttles to Jamaica station. The terminal is located in the City Terminal Zone, the LIRR's Zone 1, and thus part of the CityTicket program.
The 1977 Chicago Loop derailment occurred on February 4, 1977, when a Chicago Transit Authority elevated train rear-ended another on the northeast corner of the Loop at Wabash Avenue and Lake Street during the evening rush hour. The collision forced the first four cars of the rear train off the elevated tracks, killing 11 people and injuring at least 268 as the cars fell onto the street below.
O'Hare is a Chicago "L" station located at O'Hare International Airport, 17 miles (27 km) northwest of The Loop. The northwestern terminus of the Chicago Transit Authority's Blue Line, it is a subway station with two island platforms serving three tracks, situated under the parking garage for Terminals 1, 2, and 3. Trains are scheduled to depart from O'Hare every 2–7 minutes during rush-hour periods and take about 40 minutes to travel to the Loop. It is the westernmost station of the Chicago 'L' system. It is also the only station without coordinates in Chicago's grid system, the only underground terminus, and is the only terminal that does not directly connect to any CTA or Pace buses. Uniquely among "L" stations, it serves airport passengers and employees exclusively, and is not accessible by foot beyond airport terminals 1, 2, and 3. It is also one of three terminals that does not have a yard assigned to it.
The current rolling stock of the Chicago "L" rapid transit system consists of four series of railcars. The oldest series is the 2600-series which was built between 1981 and 1987 and refurbished between 1999 and 2002. The second series is the 3200-series, built between 1992 and 1994 and refurbished between 2015 and 2018. The third and newest series is known as the 5000-series; built between 2009 and 2015, they feature new technologies such as LED color signs, security cameras, new seating configuration, AC motors, and interior LED signs displaying date and time. The most recent order consists of the 7000-series cars that are planned to replace the 2600-series cars, with options for additional cars that would replace the 3200-series cars.
On May 28, 2008, shortly before 6 p.m., two westbound MBTA trains collided on the Green Line D branch between Woodland and Waban stations, behind 56 Dorset Road in Newton, Massachusetts. An investigation by the National Transportation Safety Board (NTSB) originally found the cause of the accident to be due to the operator texting while driving, but the NTSB later found that the operator of the rear train, Terrese Edmonds, had not been using her cell phone at the time of the crash, but rather went into an episode of micro-sleep, causing her to lose awareness of her surroundings and miss potential hazards up ahead. The collision killed Edmonds, and numerous others were injured. Fourteen passengers were taken to area hospitals; one was airlifted. This crash, along with another similar accident a year later, led the NTSB to set higher standards and regulations regarding the use of cell phones while operating a train.
During the afternoon rush hour of June 22, 2009, a subway train wreck occurred between two southbound Red Line Washington Metro trains in Northeast, Washington, D.C., United States. A moving train collided with a train stopped ahead of it; the train operator along with eight passengers died, and 80 people were injured, making it the deadliest crash in the history of the Washington Metro.
There have been numerous incidents on the Washington Metro over its history, including several collisions causing injuries and fatalities, and numerous derailments. The Washington Metropolitan Area Transit Authority (WMATA) has been criticized for disregarding safety warnings and advice from experts.
The 2600-series is a series of Chicago "L" car built between 1981 and 1987 by the Budd Company of Philadelphia, Pennsylvania. 600 cars were built, and 489 of them remain in service.
On the morning of December 1, 2013, a Metro-North Railroad Hudson Line passenger train derailed near the Spuyten Duyvil station in the New York City borough of the Bronx. Four of the 115 passengers were killed and another 61 injured; the accident caused $9 million worth of damage. It was the deadliest train accident within New York City since a 1991 subway derailment in Manhattan, and the first accident in Metro-North's history to result in passenger fatalities. The additional $60 million in legal claims paid out as of 2020 have also made it the costliest accident in Metro-North's history.
On April 3, 2016, Amtrak train 89, the southbound Palmetto, struck a backhoe while travelling through Chester, Pennsylvania, killing two track workers and derailing the locomotive, as well as damaging the first two cars.
On September 29, 2016, an NJ Transit commuter train crashed at Hoboken Terminal in Hoboken, New Jersey. The accident occurred during the morning rush hour, at one of the busiest transportation hubs in the New York metropolitan area. One person died, and 114 others were injured. The train operator, who was in the cab car, was among the injured.
On December 18, 2017, Amtrak Cascades passenger train 501 derailed near DuPont, Washington, United States. The National Transportation Safety Board's (NTSB) final report said regional transit authority Sound Transit failed to take steps to mitigate a curve at the accident location, and inadequately trained the train engineer. The train was making the inaugural run of the Point Defiance Bypass, a new passenger rail route south of Tacoma, Washington, operated by Amtrak in partnership with state and local authorities in Oregon and Washington, on right-of-way owned and operated by Sound Transit. The bypass was intended to reduce congestion and separate passenger and freight traffic, and was designed for faster speeds and shorter travel times, saving ten minutes from Seattle to Portland compared with the previous route used by Cascades.
On November 16, 2023, a Chicago Transit Authority (CTA) passenger train collided with maintenance equipment north of Howard station, injuring 16 people.
